In this report we presented a case of emphysema of the parotid gland induced by abnormal habit.The patient was a 45-year-old man. In Sept. 1983, he noticed painless swelling of the left cheek. In Feb. 1985, concerned about the swelling which had continued to grow at times, he visited a clinic of an otorhinolaryngologist. However he received no cure because no swelling was found at that time. On Dec. 3, 1986, when he was admitted to a hospital for a physical examination, the swelling of the same region was pointed out by the doctor. And on Dec. 12, he was referred to our university clinic.In the clinical examination, a diffuse swelling of the left cheek was observed. W hen we pressed the bilateral parotid gland regions, we felt as if we had pressed a mass of flour. And foamy saliva was ejected from the parotid papilla with air. CT scan and sialogram revealed air in the bilateral parotid glands and ducts.Clinical diagnosis of emphysema of the bilateral parotid glands was made. This emphysema was considered to be induced by the abnormal habit that he had puffed out his cheeks strongly with his breath many times since he was 11 years old.

Sialadenosis, characterized by uniform hypertrophy of the acinar parenchyma of the salivary glands, is associated with a variety of systemic diseases of functional disorders.This paper describes two cases of sialadenosis, a 20-year-old woman with submandibular gland swelling and a 31-year-old woman with parotid gland swelling. Both of them had endocrine disorders that may be associated with anorexia nervosa.

The efficacy of combined treatment with amfenac sodium (150 mg/day for 14 days) and ethyl loflazepate (2 mg/day for 14 days) was clinically evaluated in 44 patients with temporomandibular disorders in comparison with 77 patients with temporomandibular disorders who were given amfenac sodium (150 mg/day for 14 days) alone.<BR>The 121 temporomandibular disorder patients were classified into four types (I-IV) according to the criteria proposed by the Japanese Society for the Temporomandibular Joint.<BR>The usefulness of combined treatment with amfenac sodium and ethyl loflazepate was excellent in 14 cases (31.8%), good in 14 cases (31.8%), and fair in 12 cases (27.3%).<BR>The usefulness of amfenac sodium alone was excellent in 6 cases (7.8%), good in 34 cases (44.1%), and fair in 31 cases (40.3%).

下顎歯肉癌に対する手術は, 従来より区域切除を主体として行われ, 良好な治療成績が得られているが, それを基本としつつ患者のQOLを考慮し辺縁切除の適応の拡大に努めてきたので報告した。下顎骨吸収深度 Grade2 (骨体部, 下顎管上の骨吸収) で, 骨吸収様式が平滑型 (Pressure type) の症例までは下顎辺縁切除が可能であった。腫瘍の軟組織進展範囲別では頬側進展型に原発巣再発の頻度が高く, 区域切除の適応が多かったが, 下顎歯肉癌全体で術前治療後の縮小手術は14.9%に行われた。画像診断特に Dental CTにより, 下顎下縁の保存に関する判断が容易となった。

The presence of sebaceous glands in the oral mucosa is not uncommon, however, sebaceous elements are rarely found on the tongue. Only six cases of ectopic sebaceous glands of the tongue have been reported previously. And four cases of them which have formed duct-like structure were discussed in relation to the thyloglossal duct.<BR>We have recently encountered a case which was clinically and microscopically similar to the prior cases. In this article, a case report was presented with the review of the literature.

本年度においては、昨年発表されたヘパラネース遺伝子のシークエンスをもとに、培養口腔癌細胞株および、患者から得られた口腔癌組織内におけるヘパラネースの発現について検索を行い、その転移能との相関についても調べた。さらに、研究計画どおり、培養扁平上皮癌細胞株におけるヘパラナーゼの活性とMMP2、9、MT-MMPの発現との相関も調べた。用いた培養細胞それぞれのヘパラネースmRNAの発現量は、活性レベルとほぼ相関していることがわかった。ヘパラナーゼの酵素活性を、定量PCR法を用いて、簡便に測定できることが示唆された。術前からリンパ節転移が存在した症例、原発が制御されても術後9ヶ月以内にリンパ節転移を確認した症例のなかでヘパラナーゼ陽性例数を見ると、前者には57.1%で陽性、また、後者では、100%陽性であった。また、発現レベルとともに転移率の上昇が見られた。以上より、ヘパラネースが、口腔癌において、リンパ節転移能と関連する重要なマーカーの1つになる可能性が期待できる。さらに、培養扁平上皮癌細胞株における、MMP2,MMP9,MT1-MMP,TIMP2の発現を定量し、そのヘパラネース活性とマトリジェルにおける浸潤能の相関についてしらべたところ、それぞれの細胞の浸潤能に対し、独立したMMPの発現レベルとヘパラネース活性を有しており、とくに大きな相関は見出されなかった。現在、昨年クローニングしたラットヘパラネースの抗体の精製を完了させ、また、in situハイブリダイゼーション法による、組織内のヘパラネース発現パターンを検索しており、マウス実験転移モデルと組み合わせて、がん転移機構における、ヘパラネースの機能の分析を続けていく予定である。
