Exposure is a well-known efficient therapeutic technique for anxiety disorders or irrational fear. The present article reviews the behavioral mechanisms of fear reduction in exposure-based therapies including simple exposure and systematic desensitization. The traditional and current theories of associative learning of Pavlovian conditioning, extinction, and response reemergence are applicable to acquisition, treatment, and relapse of fear. Despite recent transactions between basic animal research and human clinical studies on simple exposure therapy, little attention is paid to the counterconditioning process in systematic desensitization. By focusing on theoretical and experimental studies with animals and their implications for clinical settings, we propose further collaboration between basic and clinical studies.

As a method for mining offshore gas fields, a floating production, storage and offloading (FPSO) system is attracting attention. However, sloshing in the oil-gas separator installed in FPSOs excited by sea waves is expected to cause significant difficulties. To suppress sloshing wave heights, one possibility may be to install perforated plates in a tank. In this study, a method is proposed for the accurate estimation of the first resonant wave height in the horizontal cylindrical tank with a perforated plate under pitching excitation in less time. To accomplish this purpose, the pressure loss due to the perforated plate in the open channel must be estimated accurately. Therefore, the pressure loss is modeled using steady CFD calculations considering the effects of the distribution of the flow velocity and the distribution of the inflow angle. The first order sloshing wave height is calculated in the theoretical analysis by substituting the pressure loss calculated in steady CFD. The wave heights determined using the pressure loss utilized by steady CFD are compared with the experimental value measured with a small-scale model. Using the method proposed in this study, the first resonant wave height of sloshing wave height is calculated accurately in less time.

Clinical use of TNFα (tumor necrosis factor α) inhibitors, adalimumab and infliximab, for psoriasis began in January 2010 when an additional indication for this disease was approved. In January 2011, an interleukin-12/23 p40 (IL-12/23 p40) inhibitor, ustekinumab, was newly approved as the third biologic agent with an indication for psoriasis. All of these biologic agents are expected to exhibit excellent efficacy against not only psoriasis but also psoriatic arthritis, and to contribute to the improvement of quality of life (QOL) of psoriatic patients. At the same time, however, they require safety measures to prevent adverse drug reactions such as serious infections. We therefore decided to prepare this Guideline/Safety Manual for the Use of Biologic Agents in Psoriasis (The 2011 Version) by revising that for the use of TNFα Inhibitors prepared by the Biologics Review Committee of the Japanese Dermatological Association in February 2010. In this new unified version for all three biologic agents including ustekinumab, requirements for clinical facilities for the use of biologic agents, contents of safety measures against reactivation of tuberculosis and hepatitis B, and recommendable combination therapies with biologic agents, have been renewed and added. This guideline/safety manual has been prepared to assist dermatology specialists experienced in clinical practice of psoriasis to use biologic agents safely and properly.

症例は妊娠37週5日の25歳女性,発熱後に異常言動,記憶障害が出現し,辺縁系脳炎と診断した.後に奇形腫ではないことが判明したが,帝王切開の術中に両側卵巣腫瘍を認めた.不穏,口部ジスキネジア,薬剤抵抗性の全身性ミオクローヌス,中枢性無呼吸,自律神経障害を呈したが,免疫治療に良好に反応した.経過から抗N-methyl D-aspartate(NMDA)受容体脳炎が疑われたが抗NMDA受容体抗体は陰性,一方,抗SS-A抗体が陽性であり,唾液腺生検でシェーグレン症候群(Sjögren’s syndrome; SjS)と診断した.SjSに合併した辺縁系脳炎は過去に数例報告があるが,抗NMDA受容体脳炎様の経過を呈した報告はなく,辺縁系脳炎の鑑別を考える上で重要な1例と考え報告する.

TRIZなどに代表されるようにアイディア検索のために特許を利用する方法が提案されている。このような状況では、表面上の表記に注目した同分野の特許を検索するのではなく、類似の問題を扱っている特許を検索することが重要となる。本研究では、アイディアの根幹にある物理法則をキーとした検索を考え、学習により文書に対して関連物理法則のラベルをつけ、分類する方法を提案する。

Monthly deposition samples were collected at the Nuclear Fuel Cycle Engineering Laboratories located about 115 km south of the Fukushima Daiichi Nuclear Power Plant (NPP), and the radioactive nuclides were analyzed. Although radionuclides such as 131I, 129mTe-129Te and 132Te-132I with 134Cs and 137Cs were observed by gamma spectrometry at the early time of the Fukushima Daiichi NPP accident, only 134Cs and 137Cs were detected as of September 2011. The annual depositions of 134Cs and 137Cs were about 19,000 and 17,000 Bq m-2, respectively in March 2011-February 2012 after the accident. Moreover, 90Sr was detected in some samples. Monthly 90Sr deposition in March 2011 was 5.1 Bq m-2 month-1 and then decreased after that. On the other hand, 239, 240Pu concentration was the same level with the concentration before the accident.
