インターネットをはじめとするコンピュータネットワークの整備により種々の情報がオンラインで利用可能になり、情報収集が容易になってきた。しかしその反面、情報の量が多すぎるため有用な情報を見逃す危険性も増大してきている("情報洪水"の問題)。特に電子メール・Net News・新聞等の"フロー情報"では情報の"新しさ"が重要であり、時間が経過してしまうことによりユーザにとっての情報の価値が失われてしまうことも少なくない。こうした問題への対処として、我々は、フロー情報からの情報収集・整理を支援するシステムの研究開発を進めている。本システムは、フロー情報の中からユーザにとって有用な情報を選別し、大意・キーワード等を付与し、ユーザの設定した分類体系に振り分けて、適切なタイミングで提示する。柔軟な処理を行なうために、マルチエージェントモデルに基づくアーキテクチャーを採用した。本稿ではこのシステムの概要について述べる。

The purpose of this study is to quantitatively evaluate the effectiveness of the components of existing PVC windows in the country in providing thermal insulation, to identify the optimal conditions of the components and the insulation performance of windows with the components in the optimal conditions, and to conduct a performance test on samples under the practical specifications for production. We briefly summarize the results below. 1) For the frame, using a method of quality engineering, we examined the reduction in conductive heat flow across each cross-section for multiple regulatory elements, which would contribute to thermal insulation. We then derived the optimal condition (level) for each regulatory element and showed the reduction rate under the optimal condition (sash frame part 19%-21%, fix frame part 6%-10%, mullion frame part 22%). Furthermore, based on the SN ratio, we found that the reduction rate is highest with the following components: inner-frame insulator in the sash frame part and fix frame part, and physical properties of mullion reinforcement in the mullion frame part. 2) As for the glazing part, we examined the specifications of the Low-E film suspended air space of a insulated glass and observed the effect of a Low-E film, optimized the arrangement, and the optimal thickness of the air space for each gas fill. We also examined the thermal transmittance at the center of glass with xenon gas having 3 films (0.29 [W/(m2·K)]), and those at the center of glass with krypton gas having 2 films (0.39 [W/(m2·K)]). 3) We determined the thermal transmittance of the window (0.56 [W/(m2·K)]) with the combined optimal conditions for the frame and glazing parts. In actual manufacturing, however, there are two problems: a technical problem (of forming a metal film inside the frame cross-section and arranging phenolic foam insulator without leaving any space) and a high manufacturing cost (associated with filling xenon gas inside the insulated glass). We therefore examined the insulation performance of samples having practical specifications for production. As a result, we obtained a measured thermal transmittance of 0.63 [W/(m2·K)] in contrast to the calculated value of 0.65 [W/(m2·K)].

症例は70歳代女性.近医で多発骨転移を伴う乳癌と診断され当院紹介受診となった.パクリタキセル(PTX)による化学療法中にAST 201 U/L,ALT 35 U/LとAST優位のトランスアミラーゼ上昇を認めた.腹部超音波検査,腹部造影CT検査では異常所見は認めなかったため,PTXによる薬剤性肝障害を疑い薬剤の変更,中止をしたが肝機能障害は改善しなかった.超音波エラストグラフィによる肝硬度検査を施行したところ,TE(Transient elastography)39.1 kPa,VTQ(Virtual touch quantification)2.73 m/s,SWE(Shear wave elastography)2.54 m/sと著明な上昇を認め,肝生検を施行し,特殊型乳癌の1つである浸潤性小葉癌の肝転移と診断された.肝硬度が高値になる稀な原因として微小転移性肝癌があげられた.

グロコット染色は一般に,銀液の温度や反応時間の設定が難しく,施行者間での染色性に差が生じやすい。特にメセナミン銀液を用いる従来法ではその傾向が強いことから,銀液の反応時間の許容範囲が大幅に広く,また塩化金液による菌体の染色性を調節することができるクロム酸アンモニア銀法を用いることで施行者間での相違が少なくなることが期待される。しかし,これまでには温熱下での銀液反応時間の設定や,真菌ごとの至適条件の検討は十分には行われていない。今回我々は,従来法とクロム酸アンモニア銀法における各種真菌での溶融器と温浴槽を用いた銀液の反応時間および塩化金液の反応時間を比較検討した。検討した真菌はアスペルギルス,クリプトコッカス,ニューモシスチス・イロベチーの3種類で,いずれの真菌でも溶融器を用いた場合に,良好な染色性を示す銀液の反応時間の幅が最も広いことが確認された。また,いずれの真菌においても塩化金液の反応時間を変えることで菌体の色の濃さが調節でき,いずれも1~5分で十分な染色が行えることが明らかになった。溶融器を用いたクロム酸アンモニア銀法によるグロコット染色は,塩化金液での染色時間を真菌ごとに調節することで,施行者間の差の少ない安定した結果が得られるものと考えられる。

The thermal performance and equipment type specified for a newly built house were investigated as benchmarks to enable the selection of appropriate insulation and equipment specifications. The prevalence of electric heater installations for space heating and hot water supply decreased in 2012, compared to 2011. However, it is estimated that the primary energy consumption is high in particularly cold areas of Hokkaido, because this type of equipment remains common in these areas. Because there is an initiative to contractors when the equipment type is determined, it is important to provide appropriate information to these contractors.

Delanian et al. reported the efficacy of pentoxifylline-tocopherol-clodronate (PENTOCLO) treatment for osteoradionecrosis (ORN) of the mandible in 2011. Spontaneous sequestration and epithelialization of the mandibular lesion are induced by this method. We report a case of osteoradionecrosis of the mandible which responded to treatment with PENTOCLO. A 60-year-old man was referred to our department with persistent mandibular ORN after definitive chemoradiotherapy for oropharyngeal carcinoma (T2N0M0). Periodontitis of the right second molar caused ORN, followed by pathological fracture. PENTOCLO treatment was started following 4-week antibiotic therapy. The sequestrum spontaneously separated with mucosal epithelialization two months after the treatment started. The second molar was removed at 11 months because of its increasing mobility. The extraction socket had epithelized and computed tomography (CT) showed that the pathological fracture had healed. No adverse events occurred during this treatment course. PENTOCLO may be an organpreserving treatment option for ORN of the mandible.

顎骨切除後の下顎骨は骨の形態が非対称であり、特に正中部は応力がかかりやすく、咬合負荷をする場合には骨の強度を考慮する必要がある。チタンインプラントは顎骨機能の回復に有用であるが、顎骨切除後症例では残存骨やインプラント周囲への負荷を考慮する必要がある。本研究では、骨再建用スキャホールドによる再建を念頭におき、咬合応力をどのように分布させることが最適であるかインプラントシミュレーションを実施して検討した。インプラントを作用点とした下顎応力解析ではそれぞれを結合させることで3点分布においても4点分布と同等の応力軽減を図ることが可能であり、その分布は切除部から離れた片側1点、反対側2点が最適であった。

The aim of this study is to determine the criteria for the wall structures in cold region where fibrous thermal insulation materials are used, by measuring the hygrothermal condition of the full size specimen wall which modeling the execution defects of thermal insulation and vapor barrier. From the experiment results, the wall with air convection in the cavity has possibility to make condensation at the bottom of the wall. It is clarified that an attic space, wall cavities and a crawl space must be separated to avoid air flow connections among them. Vapor barrier installed continuously, and ventilated air space are also effective to keep from making condensation in the insulated wall cavity.

悪性腹膜中皮腫(malignant peritoneal mesothelioma:MPM)と原発性腹膜癌(primary peritoneal carcinoma:PPC)は同じ腹膜中皮細胞由来であるが,病態や治療方針が異なり早期に鑑別する必要がある.またMPMはPPCに比してまれで本邦の年間女性死亡者数は30人に満たず,肉眼的に大網に限局した早期のMPMの報告は海外を含めて数件が報告されているのみである.今回経腟的腹水穿刺を行い,腹水細胞診を免疫染色することでMPMを診断し,MPMに適応した術式が選択できた症例を経験したので報告する.症例は32歳,経産婦.月経不順にて前医を受診し腹水貯留を指摘されたため当科紹介となった.血清CA125は129 IU/ml,経腟超音波検査にて両側付属器およびダグラス窩腹膜は正常で,腹部造影CT検査にて肥厚した大網の脂肪組織が淡く造影された.消化管内視鏡検査にて異常なく,PET/CT検査にて18F-fluorodeoxygrucose (FDG)の有意な集積は認めなかった.ダグラス窩穿刺により淡褐色粘性腹水を採取し細胞診に提出したところ,重積性を示すマリモ状細胞集塊を認め,calretinin,CK5/6,D2-40による免疫化学染色法にていずれも陽性であった.これよりMPMと診断し腹腔鏡下に腹腔内を観察したところ,黄褐色で不整に肥厚した大網以外に異常を認めず大網生検を行った.生検組織によりMPMと確定診断した後に肉眼的完全摘出を目標として大網亜全切除術を行った.術後に化学療法を勧めたが拒否され, 6ヵ月後に全身倦怠感を認めPET/CT検査を施行したところ腹腔内に多発腫瘍を認めた.現在,pemetrexedとcisplatinによる全身化学療法を行っている.〔産婦の進歩63(2):106-111,2011(平成23年5月)〕

Sialoliths commonly occur in the submandibular gland, and parotid gland sialoliths, especially parenchymal sialoliths, are rare. As a minimally invasive surgery, we report here endoscopic removal of a parenchymal sialolith of the parotid gland under local anesthesia.A female in her 40s with swelling and pain in the left buccal region was referred to our department for endoscopic removal to avoid extraoral removal after diagnosis of parotid sialolith in another hospital. Computed tomography showed that the parenchymal sialolith of the left parotid gland was 1.7×1.4×1.0 mm in size. The patient underwent endoscopic removal of the sialolith under local anesthesia as minimally invasive surgery. Complete removal was performed and the postoperative course was uneventful without complications such as swelling of the parotid gland.

During World War I, the price of daily necessities rose dramatically in Japan, causing city residents to face many difficulties. Officials in each city established a municipal retail market to ease the burden on citizens and began selling foodstuffs at reasonable prices. However, the municipal retail market in the city of Tokyo (which was the capital and most populated city) was not sufficiently developed. This paper discusses the reasons why the municipal retail market was not sufficiently developed in Tokyo prior to World War II; specifically, (1) the absence of a mayor and resistance of the city council members delayed the establishment of the municipal retail market, (2) shopkeepers strongly opposed the municipal retail market, and (3) the system to operate the municipal retail market was in disarray because of regular interference in municipal administrative affairs by the city council members.
