Background and Aims: For any committed athlete, getting back to conditioning and participation post-injury (return to play [RTP]) needs to be as swift as possible. The effects of near-infrared lightemitting diode (LED) therapy on pain control, blood flow enhancement and relaxation of muscle spasm (all aspects in the treatment of musculoskeletal injury) have attracted attention. The present pilot study was undertaken to assess the role of 830 nm LED phototherapy in safely accelerating RTP in injured university athletes.Subjects and Methods: Over a 15-month period, a total of 395 injuries including sprains, strains, ligament damage, tendonitis and contusions were treated with 1,669 sessions of 830 nm LED phototherapy (mean of 4.3 treatments per injury, range 2 - 6). Efficacy was measured with pain attenuation on a visual analog scale (VAS) and the RTP period compared with historically-based anticipated RTP with conventional therapeutic intervention.Results: A full set of treatment sessions and follow-up data was able to be recorded in 65 informed and consenting subjects who achieved pain relief on the VAS of up to 6 points in from 2-6 sessions. The average LED-mediated RTP in the 65 subjects was significantly shorter at 9.6 days, compared with the mean anticipated RTP of 19.23 days (p = 0.0066, paired two-tailed Student’s t-test).A subjective satisfaction survey was carried out among the 112 students with injuries incurred from January to May, 2015. Eighty-eight (78.5%) were either very satisfied or satisfied, and only 8 (7.2%) were dissatisfied.Conclusions: For any motivated athlete, RTP may be the most important factor postinjury based on the resolution of pain and inflammation and repair to tissue trauma. 830 nm LED phototherapy significantly and safely reduced the RTP in dedicated university athletes over a wide range of injuries with no adverse events. One limitation of the present study was the subjective nature of the assessments, and the lack of any control groups. However, further controlled studies are warranted to enable confirmation and generalization of the very good results in the present study.

Background and Objectives: Computational clinical trial (CCT) in the field of laser medicine promotes clinical application of novel laser devices, because this trial carried out based on numerical modeling of laser-tissue interactions and simulation of a series of treatment process. To confirm the feasibility of the computational clinical trial of skin treatment with a novel picosecond laser, this paper presents an evaluation method of the safety. Study Design/Materials and Methods: In this method, the light propagation and thermal diffusion process after ultrashort light pulse irradiation to a numerical skin model is calculated and the safety based on the photothermal damage is evaluated by computational modeling and simulation. As an example, the safety of a novel picosecond laser device was examined by comparing with several laser devices approved for clinical use. Results: The ratio of the maximum thermal damage induced by picosecond laser irradiation was 1.2 × 10-2 % at the epidermis, while that caused by approved laser irradiation was 99 % at the capillary vessels. The numerical simulation demonstrated that less thermal damage was observed compared with the approved devices. The results show the safety simulated by photothermal damage calculation was consistent with the reported clinical trials. Conclusions: This computational clinical trial shows the feasibility of applying computational clinical trials for the safety evaluation of novel medical laser devices. In contrast to preclinical and clinical tests, the proposed computational method offers regulatory science for appropriately and quickly predicting and evaluating the safety of a novel laser device.

Spasticity following cerebral vascular accidents (CVAs) is a common occurrence, but remains a problematic entity to treat and interferes with mobility and self-care activities which are critical for successful rehabilitative outcomes. Low reactive-level laser therapy (LLLT) has attracted attention in a number of areas including spasticity associated with cerebral palsy. In the case of post CVA therapy, LLLT has been reported for pain treatment, but not spasticity. The present study examined the efficacy of LLLT in attenuating triceps surae muscle spasticity in CVA patients. The study subjects comprised 15 chronic CVA patients with spasticity, treated at our university hospital between 2002 and 2006. The LLLT device we used was a near infrared (830 nm) semiconductor laser device delivering 1 W in continuous wave (irradiance, 670 mW/cm2) . The laser probe was applied with mild pressure to the skin over the tibial nerve on the affected side, 30 sec/point (dose/point 20.1 J/cm2) repeated 3 times with a 5 sec interval between irradiations. Two sessions were given per week for 1 week. At the end of the week, we recognized LLLT effects in 11 cases out of 15. The other 4 patients had little or no effect but were in their fifties, and had successive bouts of ankle clonus. LLLT is a promising medical treatment for the attenuation of CVA-related spasticity of the triceps surae muscle spasticity, and facilitate voluntary movements in such patients. Further studies are warranted to elucidate the mechanisms by which LLLT can attenuate spasticity.

This paper presents the results of a study on soft tissue injury treated with laser therapy. Sixty patients (40 males and 20 females. whose ages range 32-68 years) with sacrospinal muscle strain, external humeral epicondylitis, or lumbar muscle strain were selected for this study. After laserr therapy was received in these patients. their condition all bud an improvement immediately The results showed that the rate of recovery, very good, and good were 50%, 31.7%, and 15% respectively. We believe that the laser therapy for some soft tissue injury is an effective treatment.

In patients with cerebral palsy of the tension athetosis type, a number of symptoms may be observed, including not only the fairly constant involuntary athetotic movements but also myotonic disorders of the motor function of all four limbs and trunk, vocalization and motions associated with eating such as mastication and swallowing. Aggravation of involuntary movements and pain in the neck and back are also seen in many cases. Existing conservative treatment methodologies have proved to be more or less ineffective, and limitations in functional training in adults have made treatment extremely difficult. We first employed low reactive-level laser therapy (LLLT) in a case of spastic cerebral palsy in 1994 with good results. In the present study, we applied laser irradiation (830 nm. 60 mW continuous wave) to all myotonic sites around the face and neck region where myotonia was severe in 20 patients with cerebral palsy of the adult tension athetosis type for which there was no effective treatment for their neck and back pain. Improvement of myotonia was seen in 19 patients while improvement of pain in the neck and back was seen in all the 16 patients who had pain in these regions. Suppression of myotonia reduced tonic vocalization making words easily heard; moreover, suppression of myotonia and involuntary movements improved working efficiency enabling the patient to perform fairly complex tasks such as word processing. Insomnia and dysuria also improved. Even in the most severe cases, assisting the patient became easier and breathing improved. LLLT with the 830 nm diode laser provides a new and effective treatment modality in this extremely problematic condition, has no serious side effects, and has the potential to improve these patients’ quality of life.

Background and aims: The mortality rate from vascular diseases is one of the highest. The use of Intravenous Laser Blood Irradiation (ILBI) within the last 30 years has demonstrated high efficacy in the treatment of vascular, cardiac and other systemic diseases.Rationale: Laser energy at 630-640 nanometers is arguably the most effective for irradiation of blood and the vascular wall. Photons at this wavelength are absorbed by oxygen, improve microcirculation, can change the viscosity of the blood and affect vascular endothelium.Conclusions: In summary, more than 25 years of experience of using laser energy at 630-640 nm has shown that this waveband directly influences the parameters of all cells in the blood, blood plasma, the coagulation process and all the structural components of the vascular wall. Additionally, ILBI directly or indirectly affects the cells of the immune system, hormones, and exchange processes in an organism, thereby not only improving the function of the vascular system, but also the other systems of an organism. It can finally lead to lower the incidence and number of vascular diseases, and indirectly to the reduction of the number of diseases in other organs and even systemically, thus helping to prolong the lifespan.

In the period of approximately 11 years between 1985 and 1995 64 patients suffering from allergic rhinitis (AR) visited the Department of Anesthesia, Matsuyama Red Cross Hospital for treatment. From these 64 patients, 32 clinical cases were studied for the case history, type and severity of the AR, and the efficacy of stellate ganglion block (SGB, n=20) and laser irradiation over the stellate ganglion (SGL, n=12) was evaluated. Treatment sessions were limited to a maximum of 20 over a maximum treatment period of one month. The lasers used were GaAlAs diode systems (60 mW and 120 mW, irradiation times of 10 min and 7 min, respectively, applied in contact pressure mode to the area over the stellate ganglion). The mean overall efficacy rate, (excellent plus good results) based on the improvement calculated on the pretreatment clinical score, was 60% (12 patients) for the SGB group and 100% (12 patients) for the SGL Group. The mean number of treatment sessions for the SGB Group was 13.9 ± 7.1 sessions and 6.8 ± 2.6 for the SGL group. Thus a better efficacy was obtained in the SGL group in a shorter period with fewer treatment sessions required compared to the SGB group. SGL using the GaAlAs diode laser at the parameters used in the present study was therefore concluded to be effective as a therapy for allergic rhinitis, and offers the additional advantages of being painless and easy to apply, side effect free and well-tolerated by all ages of patient.

Inflammation of many different aetiologies is a condition affecting almost 100% of all patients presenting at dental clinics, and the complete management of inflammation is often extremely important before any orthodontic surgery can be carried out. The lymphatic system is one of the body’s main lines of defence against inflammation, with the lymph nodes playing an extremely major role. laser therapy, or LLLT, has become much more common in dental offices, and is associated with pain attenuation, wound healing and anti-inflammatory properties. The last of these is mediated amongst others by the effect of LLLT on the lymphatic system, which has been shown to enhance lymphatic drainage concomitantly with increasing local blood flow to the treated area. Infected areas and those with viral contamination have been suggested as contraindications for laser therapy. The possibility of treating these difficult-to-manage entities through laser therapy-accelerated drainage of the affected lymphatics offers another approach. Although lymphatic drainage is frequently used in the clinical setting, its application in dentistry is less common than it should be. An illustrated review of the lymphatic system of the head and neck is presented, with relevance to the use of infrared diode laser therapy in inducing lymphatic drainage in the management of inflammation associated with acute infectious processes such as pericoronitis, endodontic abscesses, alveolitis and herpes. A case report is presented illustrating the success of this therapeutic approach for labial herpes simplex. Infrared diode laser therapy-mediated lymphatic drainage offers an interesting and efficacious approach in the management of infectious and contaminated areas, without actually irradiating the areas themselves.

The World Federation of Societies for Laser Medicine and Surgery (WFSLMS) has achieved many things since its inception in 2002, but one of the most generous of the WFSLMS programs and goals is the donation of laser and other systems and training in their use to medical professionals in developing countries. WFSLMS has also established training programs both in the target country, and for successful applicants from the target company at selected sites of excellence in Japan. Having heard of a 3-month training program carried out in Vietnam, the author applied was lucky enough to be accepted for one such 3-month training program in the use of functional training and rehabilitation, including the use of laser therapy and light therapy, at the Shinano Handicapped Children's Hospital, Japan. This article summarizes the program and the author's experiences.
