糖尿病患者の自己管理における自己モニタリング時の感情や思いの表現から,自己モニタリングの性質を明らかにすることを目的に,6名のインスリン依存性糖尿病患者に自己管理場面を想起してもらう方法で面接を行い,自己モニタリング時の感情や思い,状況,行動の特徴を分析した結果,以下の知見を得た。1.糖尿病患者の自己管理における自己モニタリング時の感情や思いの表現は,以下の9に分類できた。『あれっ,なんで,おかしい。』,『大丈夫かな。』,『こうなるようにしたい。』,『こういうこともあるから心配ない。』。『この時はどうなのかな。』,『やっぱりそうなんだなー。』,『私って,糖尿病ってこうなんだなー。』,『わかってきた。』,『見ちゃうと気分がよくない。』2.各々の表現には以下のような自己モニタリングの性質が示されていた。〈異常のないうまくいっている状態とのずれのキャッチ〉,〈安心・大丈夫・確かさだと感じる基準とのずれのキャッチ〉,〈規範や目標とのずれのキャッチ〉,〈不安への対処〉,〈血糖変化のバラエティーへの関心〉,〈思い起こされる判断や状態への確信〉,〈糖尿病を持つ自分への深い理解と納得へのきっかけ〉,〈自己管理に関連する事柄の関連性やパターンのはっきりした認識への貢献〉,〈悪い状態の予想とそれに伴う不快感情の生起〉The purpos of this study were to identify expression at the time of self monitoring of self-management in pataients with diabetes, and to identify quality of self monitoring. Data was collected from six outpatients with IDDM, through interviews by looking back at some past scenes of their diabetes self-management. Their expression of feeling and emotion, situation, behavior of those scenes were analyzed. Expression at the time of self-monitoring were as follows: 『Oh! Why! Unusual!』, 『OK?』, 『I want to be like this.』, 『No anxiety because it's same as usual.』, 『I want to know how high is my blood glucose in this situation.』, 『After all I become convinced that is correct.』, 『I am diabetes patient like this. My diabetes is like this.』, 『I become to understand.』, 『Feel bad if monitor such bad results.』 Quality of self-monitoring were as follows: 〈Catch the difference from good condition〉 , 〈Catch the difference from the standards about security, OK, certainty〉 , 〈Catch the difference from my rules and goal〉 , 〈Cope to anxiety〉 , 〈Attention to variety of floating blood glucose level〉 , 〈Be convinced of my judgement at past scenes when coming my mind〉 , 〈Provide a clue for realization and conviction about myself with diabetes〉 , 〈Contribute to understand some connection and patterns of self-management clearly〉
