Vibration mode of the ossicles was investigated in twelve fresh human temporal bones using a video measuring system (VMS, Technical Insrtument). This system allows one to observe the ossicular vibration and to measure its vibration amplitude (up to 0. 2 micron) and phase angle. In this study the inner and middle ear was kept intact except for two small holes in the tympanic tegmen. These holes were for the observation of ossicular movement and were covered with a thin cover glass during the experiment. The vibration amplitude and phase angle of the umbo, malleus head, lenticular process and stapes head were measured at 19 frequencies between 0. 1 kHz and 4. 5 kHz. The umbo moved piston-like at 0. 1-0. 8 kHz and 2.6-4.5 kHz but in an ellipse at 1. 0-2.4 kHz. The malleus head showed elliptical movement with its long axis anteriorly tilted around 45 degrees from the direction of the umbo vibration at 0. 1 kHz. Both the lenticular process and stapes head showed similar movement ; piston-like in lower frequencies and elliptical in higher frequencies. The umbo, lenticular process and stapes head vibrated parallel at lower frequencies. The position, displacement and phase angle of the rotation axis of the ossicles was calculated based on the displacement and phase angle of the umbo, malleus head and lenticular process. The axis was around the level of the neck of the malleus in frequencies lower than the resonant frequency, beneath the level of the short process near the resonant frequency and at the top of the malleus head in higher frequencies. The average axis displacement was 0. 9 microns at 1 kHz but much smaller at frequecies lower than 0. 8 kHz or higher than 2 kHz. This suggests that such axis movement may reduce the efficiency of the middle ear sound transmission.

Three cases of laryngeal foreign bodies in infants were treated in our hospital over the past 10 years. Among them, a case of a 1-year-old boy was reported. His chief complaint was coughing after having eaten fish. Videolaryngoscopy revealed pieces of fish bones in his subglottic space. First we removed a part of the fish bones with forceps via a flexible fiberscope following placement of a laryngeal mask under general anesthesia. Because of edema in the larynx, we removed the bones piece by piece with forceps by way of a rigid ventilation bronchoscope.With infants it is difficult to diagnose a laryngeal foreign body especially in the subglottic space. Videolaryngoscopy is useful in such cases. Before removal of a laryngeal foreign body, careful observation is mandatory to avoid unnecessary damage to the trachea and the larynx. To inspect the larynx and the trachea, the flexible fiberscope is a choice instrument following placement of a laryngeal mask under general anesthesia.

We successfully developed the Taguchi method as the short version of Vocal Function Exercise (VFE). The Taguchi method involves only the following training contents included in VFE: Extension of phonation time, and phonation exercise with a certain voice pitch. We applied this method to a series of voice disorder cases. Patients treated with the short version of VFE were typically in their 60s and 70s, often with vocal cord atrophy and sulcus. The number of training sessions and the training period duration were ≤6 times and ≤3 months, respectively, so treatment was completed in a shorter period of time than with the original VFE method. We also noted an improvement in the auditory impression, maximum phonation time, and Voice Handicap Index (VHI) in most cases with the short version of VFE compared to before therapy. The short version of VFE can be accomplished with a simple procedure and is expected to shorten the treatment period.
