Abstract : We experienced a case of accidental ingestion of a root canal instrument. At first, surgical extraction was suggested by a medical doctor, however, the therapeutic strategy was changed upon presentation of a previously published paper. After ingesting fibrous foods, the patient excreted the instrument naturally on the seventh day. The maintenance of guidelines on accidental ingestion would enable treatment policies to be shared with other medical departments.

Abstract : Before the acceptance of endodontic treatment, a huge caries tooth has been often left without any treatment for a long time. A few patients first visit a dental office due to the appearance of intolerable strong toothache. In these cases, the biologic width of tooth is frequently invaded by subgingival caries. These caries cause various troubles for attaining the final prosthesis after root canal treatment. Furthermore these teeth took not only dental movement (inclination, extrusion) but also inadequate occlusal plane. In this case report, we reported a patient who was treated with a comprehensive approach in a narrow sense, where MTM, periodontal surgical treatment and implant were performed for preservation to arrange the periodontal environment and occlusal plane from the root canal treatment until the final prosthesis. These treatment recovered the purpose of esthetic and the occlusal reconstruction on a oral unit.

Abstract : The implant periapical lesion (IPL), which is a possible cause of implant failure, may occur due to the presence of a pre-existing microbial pathology such as endodontic/periodontal lesions. A 27-year-old female patient underwent the placement of a screw-shaped submergible titanium dental implant in the maxillary anterior region. Six months after implant insertion, the patient visited our hospital and presented with a persistent pain resistant to analgesics. A periapical x-ray showed the presence of a radiolucency at the apical portion of the implant ; this image was confirmed by cone beam CT. Root canal treatment was performed to restore the bone defect in the neighborhood of the fixture. This paper presents a case history related to IPL, which was caused by a pre-existing endodontic lesion.

Abstract : This case report describes the root development following apexification of an immature tooth. The patient was a 9-year-old girl. The upper right central incisor suffered apical periodontitis with gingival abscess after rapid orthodontic treatment with a removable appliance. The root canal was cleaned by both biomechanical preparation and biochemical preparation with 6% sodium hypochlorite and 3% hydrogen peroxide. After disinfection of the root canal, calcium hydroxide paste was applied. The medicaments were replaced several times during the follow-up term. After five years and six months, radiographic images showed disappearance of the apical lesion and continuous root development. The shape of the root-end was an irregular, sharp-pointed outline with narrow canal. Following confirmation of the calcified apical barrier at the root end by palpation with a K-file, the lateral condensation procedure with a custom cone and canal sealer was done. This report suggests that root development after apexification occurs because of the existence of apical vital tissue even if the tooth is a non-vital tooth with open apex.

Abstract : Cone-beam computed tomography (CBCT) and dental microscope are useful for diagnosis and treatment in endodontic therapy. CBCT is designed to produce three-dimensional information about undistorted tooth root canals and the shape of periapical lesions. A dental microscope is effective for identifying the subvisible narrow root canal orifice, location of root perforation and fractured instrument in the root canal. This report describes the treatment by CBCT and microscope of an endodontic lesion caused by perforation. The patient was a 49-year-old woman. X-ray photographs of the mandibular right first molar showed a periapical lesion and a perforation near the intracanal post on the middle of the distal surface of the distal root. The closed orifice of the distal root canal was searched for on CBCT images, and located under a dental microscope. The root canals and perforation area were filled by gutta-percha after checking sterilization with a bacterial examination. The prognosis was good 18 months after the root canal filling. This case suggests that an exact diagnosis with CBCT and fine treatment under a dental microscope improve the success rate of complex cases of infected root canal and infected root perforation.
