We observed a nine-year-old boy who suffered from a so-called Brodie's abscess involving the adjacent metaphysis and epiphysis of the distal tibia, communicating through the growth cartilage of the epiphyseal plate. The patient presented himself with the mild symptoms and subtle clinical findings that are characteristic of Brodie's abscess, which is usually confined to the metaphyseal, and occasionally to the epiphyseal, bone. Initially, he was treated by surgical evacuation of the abscess and with intravenous antibiotics, and then by oral andtibiotics. The pathogenic organism was Staphylococcus aureus. At follow-up 10 months after treatment, he had normal results without any evidence of a growth disturbance. As far as we know, there have been 27 previously reported cases of Brodie's abscess traversing the epiphyseal plate. We believe that surgical evacuation of the lesion, followed by antibiotics, is an appropriate treatment for some children presenting themselves with this condition.
