In thirty-six cases with lumlar diseases, CT studies of the shape of the lateral recess (L5) were performed. A series of CT scans at L5 lateral recess showed the narrowest level of it and the L5 neve root condition in it. Three types of the shape were classified, which were triangular type, trefoil type and flat trefoil type. Especially the flat trefoil type was associated with LCS. We concluded that the narrowest level of the lateral recess of LCS was the entrance of the nerve root, which was trapped there by the configuration of the recess with the degenerative soft tissue, on the other hand, the lateral recess of OA was not narrowest at the entrance level, but narrowest at the pedicle level. The difference in the shape contributed to the pathogenesis of symptoms of the LCS.

The purpose of this paper is to report morphological features of the cells in the human posterior longitudinal ligament, including its attachment to the vertebral bodies, and of the cultured cells derived from PLL. We used the transmission electron microscopy to observe the cells. Far from its vertebral attachment, the main constituent cells in PLL are fibroblasts. The closer to the vertebral bodies fibroblasts become degenerative, and chondrocytes are observed in the irregular collagen fibers. The closer still more, calcium deposits on the collagen fibers, and osteocytes become the dominant cells. Compared with these three types of cells fibroblast, chondrocyte, and osteocyte in PLL, the cultured cells resemble fibroblasts.

Vascular tests for thoracic outlet syndrome are considered to be one of the important diagnostic aid in order to determine the localization and the severity of neurovascular lesion at the thoracic outlet region. However, the mechanism and significance of vascular tests are still unclear.This is to report our clinical and autopsial study to try to clarify the exact meanings of each vascular test.Materials are 72 cases in 48 patients with thoracic outlet syndrome (46 cases in 39 patients recieved first rib resection) and 21 cases of exploration in autopsy.Positive Adson's test, although the incidence is quite low, may suggest severe lesion at the scalen triangle. Eden's test indicate lesions at the costo-clavicular space. In this positive the clavicle moves most to the posteromedial direction among the various positions. Abduction-external rotation test and Wright's test show neurovascular compression mainly at the costo-clavicular region and partly at the scalen triangle. These positions make the neurovascular bundle tracted foward the lateral. Compression at the subcoracoideal region is not the main cause in Wright's test.

We experienced congenital anomalies of the cervical spine among 4961 patients in our clinic.These anomalies were analyzed 77 cases of congenital cervical vertebral fusion (C2-C3: 28, C5-C6: 13, C3-4: 12, Occ-C1: 9 and others), 14 cases of spina bifida occulta, 125 cases of posterior ponticle of atlas, and one case of os odontoideum.Among congenital cervical vertebral fusion 14 cases of myelopathy were observed, of which 4 cases were operated.Congenital fusion of lower cervical vertebrae affected commonly degeneration of adjacent vertebral disc.Congenital fusion of the occiput and the first vertebra is more frequently complicated with basilar impression and atlantoaxial subluxation.
