Since domestic air lines commenced their service in Japan, the department Otorhynolaryngogy, the Jikei University School of Medicine has been participating to the health controls of flight crews of the 'domestic air lines and students of the Aviation Academy.Highly developed airplanes, vastly increased number of flight crews and serial domestic air accidents prompted us to establish the details of medical standards in accepting new crews and students.The Otorhynolaryngological standards have been set to prevent individual health hazards in long duties, to eliminate the accidents due to human errors and to save resultant losses of the air line companies.The standards were prescribed based mainly on the experience and date in our dapartment, which were accumulated previously, in conjunction with the Japanese domestic flight rules and the medical standards of Air France and F. A. A.

Although aerotitis is a disorder of the middle ear precipitated by rapid barometric changes, it is rarely seen in modern air passengers despite the tremendous increase in air travel because of the smooth and automatic control of changes in pressurization in large passenger planes.However, among airplane personnel it still occurs in a fairly high incidence and is a hazard to which those whose occupation involves flying are exposed.The author has compiled a list of 403 cases of aerotitis media among commercial airplane personnel in our country during a recent 3-year-period and has analysed the data concerning any difference in incidence between cockpit and cabin crews, tendencies to onset, predisposing factors, and has reviewed in detail the flight patterns and pressure changes in the interi Qr of the planes, in orderto pinpoint someol the factors that may be responsible tor the reiativeiy nign inclaence.The literature on this subject is reviewed and the main points and problems in the therapy of this disorder are discussed.

Audiometric examination was performed on 120 subjects who engaged in speaker research or in administrative works in speaker production, 10 subjects who used in their work insert-type receivers, and 30 recording engineers.The results were analized in reference to the number of years in works.Present study revealed that:1) there was no tendency that hearing was impaired in speaker engineers or in other people exposed to sound from speakers.2) the mechanism of hearing loss at 4000Hz found in recording engineers was appeared either different from the ones seen among sheet-metal workers or in speaker engineers.3) the reproduced sound through speakers seemed not to cause hearing loss in human.4) the direct sound produced by instruments may cause hearing loss as in the case with noise, however, further observation is to be done to prove definite effect of the sound from the instruments.
