C-armガイド下に刺入した腸骨仙骨スクリュー(iliosacral screw)と経腸骨経仙骨スクリュー(transiliac- transsacral screw)の刺入精度と安全性を検討したので報告する.2017年12月から2019年4月の期間に不安定型骨盤骨折に対して,スクリューで後方固定を行った8例(男性5例,女性3例,平均64.8歳),11本(IS 6本,TITS 5本)を対象とした.全例が高エネルギー外傷で,5例にTAEを行った.骨折型はAO/OTA分類61B1:2例,B2:3例,B3:1例,C1:2例であった.Smith分類で正確性の評価を行い,Grade0,1,2,3が,それぞれ9,1,0,1本であり,術後合併症は認めなかった.術後整復位はRommensとHessmannの評価法を使用し,anatomic:3例,nearly anatomic:4例,moderate:1例であった.逸脱率は18.2%とC-armガイド下に安全にスクリューを刺入することが可能であったが,より正確な刺入を行うためには術中ナビゲーションの使用が望ましいと考えられる.

We Reported two cases of pyogenic arthritis of the knee which were obliged to undergo arthrodesis. Both cases had past histories of repeated steroid injection into the knee joints. Initially they were treated with closed irrigation-suction therapy, but did not improve at all, and finally underwent arthrodesis.Second case was a deep sepsis after total knee arthroplasty. In both cases bacterial isolation from specimens revealed pseudomonas pseudomonas aeruginosa, but bone union was obtained favorably by external fixation.
