本研究の目的は,広汎子宮全摘出術を経験する子宮がん患者が,がんと共に安寧に生きるための強靱さを獲得していくことを促進する看護援助を検討することである。研究対象は,癌専門病院で広汎子宮全摘出術を経験する初発の子宮がん患者19名で,対象者の術前から術後6ヶ月間にわたり面接法および参加観察法を用いたデータ収集を行い,質的帰納的分析により,対象者の安寧に生きるための取り組みの様相と,関連する看護援助を明らかにし,以下を得た。1.広汎子宮全摘出術を経験する子宮がん患者は,【元気になりたい】という願いを原動力として,安寧に生きるためには【おしっこのコツをつかむ】【余計な消耗を避ける】【求めた情報を自分流に解釈して自らを救う】【生きるために懸命に食べる】【新しい価値観を獲得する】ことに取り組み,この取り組みの過程で【つまるところは自分次第】という意識を形成し,全ての問題解決に立ち向かう強靱な特性を培う。2.強靱さ獲得の源泉は,己の再生に欠かせない【おしっこのコツをつかむ】という必然的欠乏欲求であると言える。3.広汎子宮全摘出術を経験する子宮がん患者が,がんを抱えながら安寧に生きるための強靱さを獲得していくことを促進する看護援助は,【共有】【共感】【肯定】【支持】【強化】【尊重】を援助の基幹とし,(1)面接する(2)生活環境を快適に整える(3)食事を整える(4)身体の不快感や苦痛を緩和する,である。The purpose of this study was to identify how nursing care facilitated the efforts of cervical cancer patients who wanted to get well and to cultivate hardiness after a radical hysterectomy. Subjects were 19 patients with newly diagnosed cervical cancer who faced radical hysterectomy in a hospital specializing in cancer treatment. Data were collected by interviewing the patients and through a participant observation before and for 6 months after the operation. The following results were obtained by qualitative inductive analysis to reveal how the subjects faced the challenges of getting well and to clarify how nursing care was related to this effort. 1. Cervical cancer patients who experienced radical hysterectomy were motivated by the desire to be well and challenged the tasks of learning the technique of urination, avoiding unnecessary fatigue, acquiring and understanding information and using it to help themselves, eating well to build strength, and acquiring new values. During the process of those challenges to maintain well-being, they realized that "in the end, it is up to myself and cultivated the hardiness to face all problems. 2. The source of cultivating hardiness comes from maintaining ones' natural functions, such as learning the technique of urination. 3. Nursing care for cervical cancer patients with radical hysterectomy involved: a) interviewing patients, b) making their living environment comfortable, c) preparing meals and d) decreasing physical discomfort and pain. Such care was based on "Sharing," "Sympathy," "Affirmation," "Support," "Enhancement" and "Respect."
