Objective: This study aims to estimate the cost-effectiveness of combined physical and cognitive programs designed to prevent community-dwelling healthy young-old adults from developing dementia. Methods: The analysis was conducted from a public healthcare and long-term care payer’s perspective. Quality-adjusted life years (QALYs) and expenses for health services and long-term care services were described in terms of effectiveness and cost, respectively. A thousand community-dwelling healthy adults aged 65 years were generated through simulation and analyzed. The incremental cost-effectiveness ratio (ICER) of adults with preventive program intervention compared to those with nonintervention was simulated with a 10-year cycle Markov model. The data sources for the parameters to build the Markov models were selected with priority given to higher levels of evidence. The threshold for assessing cost-effectiveness was set as less than 5,000,000 Japanese yen/QALY. Results: The ICER was estimated as −5,740,083 Japanese yen (US$−57,400)/QALY. Conclusion: A program targeting community-dwelling healthy young-old adults could be cost-effective.

Perovskite-type mixed protonic and oxide ionic conductors for electrolyte material of solid oxide fuel cells were investigated, focusing on BaZr0.1Ce0.7Y0.1Yb0.1O3−δ (BZCYYb) due to its high ionic conductivity and chemical stability. BZCYYb and NiO-added BZCYYb were evaluated using electrolyte-supported cell (ESC) and anode-supported cell (ASC) samples. 2 wt.% NiO was solid solute into the BZCYYb, resulting in improvement in the sinterability and thermal-expansion behavior. The addition of NiO, however, lead to the deterioration of cell performance. Compared with the ESC, power density of the ASC was much higher due to thin electrolyte, whereas its open-circuit voltage (OCV) was lower. This is due to Ni diffusion from the NiO–BZCYYb anode into the BZCYYb electrolyte during high-temperature co-sintering process at 1350°C. From the results of OCV measurements, 0–2 wt.% NiO was considered to be dissolved in the BZCYYb electrolyte of the ASC, suggesting that controlling Ni diffusion during co-sintering process is essential to achieve higher-performance ASCs using BZCYYb.

In older adults, social participation is an important component of rehabilitation and health promotion. Several studies have attempted to describe the definition and concepts of social participation, and there were many outcomes to measure social participation. This overview provides information about representative social participation and related concepts that have been defined in the literature. A standardized definition of social participation has not been developed; commonly, recognition for social participation was proposed as focused on involvement in social activities that provide interaction with others in a society or community. Many instruments assess the various aspects of social participation. Because of operational definition and diversity in social participation, performance in social participation was adopted as an aspect of assessment. Further discussions are needed to clarify the definition of social participation and evaluate the instruments used to assess social participation for it to be useful for rehabilitation and health promotion. In doing so, determining and developing assessment and intervention based on the purpose or perspective of social participation in older adults with and without disabilities is important.

Background: Few studies have examined whether declines over time in hand-grip strength (HGS) and fast walking speed (FWS) differ by sex and age among non-Western community-dwelling older adults. This study aimed to quantify changes in HGS and FWS over the 6-year period from 1994 to 2000 and examine whether these changes differed by sex and baseline age among older individuals in a Japanese community.Methods: We conducted a community-based prospective cohort study. The participants were 513 nondisabled men and women aged 67 to 91 years at the 1994 survey. Independent variables regarding time since baseline, in addition to various time-dependent and time-independent covariates, were obtained in 1994, 1996, 1998, and 2000. The outcome variables were HGS and FWS assessed at each survey. All data on independent and dependent variables that were collected at each survey were simultaneously analyzed using a linear mixed-effects model.Results: The linear mixed-effects model revealed significant declines in both HGS (−0.70 kg/year, P < 0.001) and FWS (−0.027 m/sec/year, P < 0.001) among nondisabled older participants who had analyzable data in any survey during the 6-year period. Sex was significantly associated with the rate of decline in HGS (P < 0.001), but not FWS (P = 0.211).Conclusions: In this analysis of nondisabled older Japanese, a mixed-effects model confirmed a significant effect of aging on declines in HGS and FWS and showed that men had a significantly steeper decline in HGS than did women during a 6-year period.
