We herein report the first case of glossopharyngeal nerve and vagus nerve palsies that appeared after an influenza vaccination. A 15-year-old boy developed dysphagia and dysarthria seven days after receiving an inoculation of the inactivated influenza vaccine. Massive intravenous immunoglobulin (IVIg) treatment was applied, as the patient's symptoms were considered to be immunological adverse effects of the influenza vaccine. He responded well to IVIg, and the symptoms immediately diminished. The mechanisms underlying the development of neurologic symptoms following vaccination are difficult to determine; however, providing immediate immunological treatment, such as IVIg, is effective and beneficial in countering these symptoms.

A 71-year-old man developed postural tremor and was treated as an essential tremor patient. Nine years after the tremor onset, he developed symptoms resembling Fragile-X-associated tremor/ataxia syndrome (FXTAS), including exacerbated (increased coarseness and amplitude) tremor in the right arm, ataxic gait, and brain MRI showed lesions in the bilateral middle cerebellar peduncles (MCP). Evidence of premutation in the form of 83 CGG repeats of the Fragile-X-mental retardation 1 (FMR1) gene confirmed the diagnosis of FXTAS. FXTAS causes various neurological symptoms including in some cases tremor resembling essential tremor in the early stages. FMR1 gene premutation should be checked when the patient develops intention tremor, cerebral dysfunction and/or a brain MRI shows MCP lesions.

The inhabitants living in a specific region of Kizaki area in Kamisu-town, Ibaraki Prefecture exhibited uncommon clinical central nervous system symptoms. A graphite furnace atomic absorption spectrophotometer detected markedly elevated concentration of arsenic (4.5 ppm) in their drinking well water. Further investigation using HPLC, GC/MS and HPLC/ICP/MS demonstrated that the structures of the arsenic were bis(diphenylarsine)oxide (BDPAO), diphenylarsinic acid (DPAA) and phenylarsonic acid (PAA), compounds that can be derived from the chemical warfare agents, diphenylchloroarsine (DA) and diphenylcyanoarsine (DC). The predominant form of the arsenic compound in the well water was DPAA (maximum 15 ppm), so that it was calculated that the inhabitants ingested 11-30 mg of DPAA daily. This is the first report of inhabitants that were injured by drinking well water contaminated with organic arsenic compounds that were likely derived from chemical weapons.
