The purpose of the current study is to clarify the structure of the care management practice based on social work. The research design is a cross-sectional survey using mailed questionnaires. The participants in the study were 1,742 care managers. The response rate was 43.6%. The factor analysis with Promax rotation was conducted in order to clarify the structure of the care management practice based on social work.The factor analysis results indicated the structure of the care management practice was composed of the following 7 domains:(1) Person-centered services,(2) Primacy of client–social worker relationship,(3)Utilization of an environment that directly affects life,(4) Utilization of informal support,(5) Strength perspective,(6)Collaborative teamwork and(7) Intervention at micro, mezzo, and macro levels.The results indicate that care management practice based on social work consists of 7 elements. It has been shown that it is important for care management based on social work not only to provide support that respects individual independence, but also to work on various environmental aspects.
