We verified the coaptaion effect of the subscapuraris muscle when the shoulder was immobilized the external rotation after initial anterior dislocation of the glenohumeral joint, and the practical use of the immobilizer to keep the shoulder in external rotation position. We evaluated twelve shoulders of the 12 patients with traumatic initial anterior dislocation of the shoulder. All of the patients were male, and the in mean age was 22.2 years old(range,17 to 30). Fast-spin-echo T2-weighted axial and sagittal magnetic resonance images were made, with the arm held at the side and positioned first in the maximum internal rotation and then in 15°external rotation within one week after the dislocation. We examined the coaptaion effect of the subscapuralis muscle comparing the images in the both positions. The patients were asked about the compliance and the discomfort of immobilization. Bankart lesions were identified in 11/12 shoulders (91.6%). The coaptaion effect was observed in 10/12shoulders (83.3%). All of the patients put on immobilizers for 3 weeks, and answered that discomfort was within the bounds of their torelance. Better coaptaion of a Bankart lesion was observed in external rotation compared with that in internal rotation. The immobilizer was of practical use.

We measured the range of internal and external rotation of the shoulders and evaluated the effect of shoulder stretching in little league baseball players. Physical examinations were performed on fifty-two little league baseball players (range of age, nine to twelve years old), and measurements of the range of shoulder rotation were made with the patients supine and the shoulder at 90° abduction. Twenty-nine players, themselves and their partners were shown how to shoulder stretch and the effect of the stretching was evaluated. They demonstrated a significantly increased external rotation and a significantly decreased internal rotation range of motion compared with the opposite shoulders. The significant differences were found in all ages. Self stretching showed the least effect, while significant increased internal rotation was obtained by partner stretching. We found a significant loss of internal rotation of the throwing shoulders even in little league baseball players. Partner stretching was useful for recovery of the motion loss.

The effect of the throwing form correction for a baseball shoulder was evaluated. Materials and methods: Forty-five baseball pitchers with throwing pain in their shoulders were examined. All were males aged 10 to 31 years (mean 17.4), with a duration of shoulder pain on throwing of 32 months in average (range 2 weeks to 11 years). The throwing motions were recorded by a digital video camera and were evaluated as to whether there were any problems in the throwing form. The throwing form was corrected and grade of pain relief and return to pitching was inquired into. Results: Forty-two of the 45 cases (93%) had one or more problems in their throwing form. Problems in the throwing arm in 35 cases (83%), in the trunk or lower extremities motion in 32 cases (76%) and in the opposite arm in 23 cases (55%) were observed.Remarkable or moderate pain relief in 28 cases (67%) and complete return to pitching in 21 cases (50%) were gained by throwing form correction. The outcomes were better in the younger cases and the cases with the shorter periods form the onset of the pain. Conclusion: The throwing form correction was effective for baseball shoulder, especially for the younger cases.
