症例は74歳男性.2018年X月に大動脈弁置換術の際に輸血を施行した.術後2カ月頃から肝機能障害を認め,HEV-IgA抗体およびHEV-RNA陽性(genotype 3b)でE型肝炎と診断した.その後の解析で輸血前のHEV-IgG抗体陽性,HEV-RNA陰性であったことからE型肝炎既感染例と考えられた.また輸血に使用した凍結新鮮血漿からHEV-RNAが検出され,解析できた遺伝子配列が患者由来HEVとほぼ一致したため,輸血によるE型肝炎と判断した.本邦では2017年までに本例を含めると,少なくとも26例の輸血後E型肝炎が発症し,その報告数は増加しつつある.北海道地区では輸血製剤において核酸増幅検査によるHEVスクリーニング検査が行われているが,それ以外の地区では施行されていない.よって全国での核酸増幅検査によるHEVスクリーニングの早期導入が望まれる.

This report examined the following two points When the globalization of the dairy products market of Japan greatly progresses by TPP agreement, the domestic dairy industry comes under what kind of influence, and what kind of problem do they face in the near future? What kind of measures should be prepared for to maintain stability of the dairy farming and dairy industry of Japan when assume them a precondition? The basic point at issue of that case is three items. 1) Reinforcement of the dairy farming infrastructure. 2) Stabilization of dairy farming management. 3) The correspondence to the change of the external factor over dairy farming and the dairy industry to show a stronger volatility. In addition, this report overlooked the present situation of the Japanese dairy farming and gave an outline that was the basic condition.
