A patient with an animate foreign body in the external auditory canal obviously needs immediate help. Before removal of insects from the external auditory canal the author has used 8% Xylocaine spray against them for several years. Most insects were paralyzed in a few seconds after two or three sprays of 8% Xylocaine. It is clinically confirmed that Xylocaine is insecticidal to most insects, but harmless to stato-acoustic organs of patients.An application of Xylocaine to an animate foreign body in the external auditory canal is strongly recommended.

The infected fistula auris congenita at the base of crus helicis makes its total removal difficult through the preauricular approach because of the location of the main lesion. The fistula sinus is usually located deep behind the auricle. Therefore, it is easier to extirpate the entire lesion through the retroauricular approach than the preauricular approach.The retroauricular approach enables a wider operative field and makes the lesion free easier from the surrounding tissues.The author describes a detailed technique of the retroauricular approach for the infected fistula auris congenita at the base of crus helicis.

突発難聴を主訴として来院し, 初診時に突発性難聴と診断されたが, その後の精査により肝細胞癌の側頭骨転移癌によることが判明した1例を報告した. 患者は57歳の男性で, 1987年6月右突発難聴, 耳閉塞感が発現し, 近くの病院で突発性難聴と診断され, 副腎皮質ホルモン療法を受けたが症状は改善されなかつた. 耳X線検査で右内耳道近辺の不整像が観察され, 聴神経腫瘍などの脳腫瘍が疑われた. 同年8月精査を希望し当科を受診, CTおよびMRI検査により右側頭骨底から後面にかけて大きな腫瘍塊が認められた. 右骨部外耳道底に出現した小さな腫瘍の生検によりこれらの腫瘍塊は肝細胞癌の転移癌であることがわかつた. この症例ではほかに同側の顔面神経麻痺, 反回神経麻痺が認められたが, 突発難聴とともに多発性脳神経症状が発現した場合には側頭骨転移癌の可能性も考慮すべきである.

Cryosurgery is gaining more and more importance as a therapeutic modality within the field of head and neck surgery.<br>Cryosurgery was performed in two patients with hemangioma arising from the entrance of the esophagus.<br>Case 1: A 41-year-old woman was referred for the diagnosis of hemangioma in the entrance of the esophagus which had been present for approximately five years. The patient felt slight pain on deglutition and sometimes noticed hemoptysis. The lesion was treated with cryoapplication of -60°C on its surface through the direct laryngoscope under general anesthesia. Cryoapplications were repeated by overlapping frozen areas during treatment. The duration of the individual freezing cycles ranged from one and half to three minutes. Over a period of 3 weeks the patient received 2 cryosurgical treatments. Postoperative healing was uneventful. After 3 weeks following 2nd cryosurgery the lesion disappeared without leaving a remarkable scar. No recurrence was noted after one-year follow-up.<br>Case 2: A 54-year-old man was admitted to the hospital with a history of foreign body sensation in the throat. Endoscopic examination revealed a dark, blue swelling in the entrance of the esophagus, suggesting a hemangioma. The lesion was frozen in three applications of -60°C each time. The frozen region was overlapped to compass the entire tumor. After thawing the frozen areas began to swell. In 24 hours it was almost twice of its original size. Then the necrotic tissue was well demarcated from vital tissue and on the seventh postoperative day the brownish lump exfoliated and showed a persistent smaller tumor. Total regression of the hemangioma and overgrowth of normal mucosa followed with a satisfactory functional result. No further growth was observed after six-month follow-up.<br>The general treatment of the hemangioma in the larynx, hypopharynx and esophagus was briefly discussed.

Acute sensori-neural hearing loss after exposure to loud sounds in rock music was experimently studied. With guinea pigs exposed to rock music for tour hours a day during one week, parmanent threshold shift (PTSs) were not recorded. Next, temporal threshold shifts (TTSs) were examied after exposing guinea pigs to rock music for 1-4 hours. There was a increased temporal threshold shifts (TTSs) at over two hours.<BR>Finally, with guinea pigs exposed to rock music for three hours and restricted in the bottle filled with water, temporal threshold shifts (TTSs) were greatest, but parmanent threshold shifts (PTSs) were not recorded at seven days later.
