2 0 0 0 OA 右上肢ガス壊疽後に敗血症をきたし死亡した一例

症例は70歳男性.草取りをした後に右前腕の疼痛が出現したため,クリニックを受診し,筋肉痛の診断でステロイドの局注を受けた.翌日,疼痛の増悪と前腕の腫脹を来たし,近医へ入院となった.抗生剤投与するも,腫脹は上腕まで広がり,Vitalの悪化と意識レベルの低下をきたしたため,受傷後2日目に当院紹介,救急搬送された.入院時,既に意識レベルの低下を認め,右前腕から上腕まで著明な腫脹と暗赤色調の紫斑と水泡形成を認めた.単純レントゲン,CTにて皮下から筋層内まで及ぶガス像を認め,ガス壊疽の診断にて緊急手術(上腕及び前腕切開排膿,デブリードマン)を施行した.培養にてKlebsiella pneumoniaeを認めた.全身管理の甲斐なく術翌朝永眠された.非クロストリジウム性ガス壊疽の死亡率は25.1% と生命予後不良の疾患である.いかに早期診断,早期治療,迅速な救命処置が必要か考えさせられる症例であった.

We studied 130 knees in 91 patients to determine the problems in fibula osteotomy sites following high tibial osteotomy. Fibula nonunion was seen radiologically in 20% of knees. The occurrence of nonunion was related to the length of the osteotomy site interval. Nonunion especially occurred with high frequency when the length was greater than 9mm. Nonunion could be divided into the hypertrophic and atrophic type. Gait pain around the fibula osteotomy site was frequently observed in patients who had hypertrophic nonunion.We have introduced a new fibula interlocking osteotomy which provides wide contact between the osteotomy sites to prevent occurrence of nonunion and fibula osteotomy site pain.
