Removal of mandibular third molars is one of the most common operations in oral surgery. Hypoesthesia of the lower lip occurring after the extraction of third molars is a serious complication, requiring a prolonged time for recovery. In this study, we investigated factors potentially affecting the incidence of hypoesthesia.A total of 781 mandibular third molars extracted at our clinic in 2002 were examined. There were 17 cases (2.2%) of hypoesthesia of the lower lip. There was no relation of gender, difficulty of extraction, or the skill of the operator to the incidence of hypoesthesia. In contrast, the incidence of hypoesthesia strongly correlated with age and the root position of mandibular third molars with respect to the mandibular canal.Eleven of the 17 cases of hypoesthesia (64.7%) resolved within 1 month, and 16 of the 17 cases (94.1%) resolved within 6 months. The patients who had hypoesthesia for more than 1 monthwere significantly older than those who recovered from hypoesthesia within 1 month and were all female.These results might be useful when informing patients about potential complications of tooth extraction.

We describe three patients (1 man and 2 women; age, 28 to 83 years) with herpes zoster of the trigeminal nerve that developed after dental treatment. The patients presented with toothache or periodontal disease as initial symptoms. Ten cases of herpes zoster occurring after dental treatment, have been reported previously. The initial symptom was toothache or periodontal tissue discomfort in most cases, and the age of the patients ranged from 7 to 73 years. Symptoms of herpes zoster appeared within 2 days after dental treatment in half of the patients. Although the relation between dental treatment and the occurrence of herpes zoster is unclear, we should include herpes zoster in the differential diagnosis when a patient presents with toothache or periodontal tissue discomfort.

Fracture of the mental spine (genial tubercles, spina mandibularis) is comparatively rare entity of mandibular fracture. It can be assertained only by occlusal radiogram. This type of fracture is considered that normal occlusal stress is a main role, in the severely atrophied edentulous mandible and hypertrophied spines in the denture bearing elders.The pathogenesis of this fracture is proposed as one of the stress or spontaneous fractures, like a so-called pathologic fracture. In this sequence, the dental practitioners should keep in mind the possibility of this fracture, especially in denture construction.The authors reported three further cases treated conservatively and discussed the mechanism and treatment of this injury.

A case of pyostomatitis vegetans associated with ulcerative colitis is reported. A 57-year-old man with a history of ulcerative colitis was referred to our hospital because of pain in both sides of the buccal mucosa and malnutrition. A snail-track-shaped ulcer and miliary pustules with a cobblestone appearance were found in both sides of the buccal mucosa and soft palate. Treatment with topical corticosteroids had no effect on this lesion. The presence of abdominal pain and hematochezia led us to suspect a recurrence of ulcerative colitis. Immediately after the systemic administration of corticosteroids was started, the oral mucosal lesions resolved. This case suggests that treatment of ulcerative colitis with systemic corticosteroids is required to cure pyostomatitis vegetans.
