This study was designed to investigate the changes of low back pain and saggital pelvic inclination over time after total hip arthroplasty (THA) in osteoarthrosis (OA) of the hip. Fifty OA cases were classified into three groups according to age: Adult; 40 to 64 years of age, 19 hips; Young-Old; 65 to 74 years of age, 16 hips; Old-Old; 75 years of age ∼16 hips. The incidence of low back pain before THA was about 60% in all groups, but the transition of low back pain after THA was that the low back pain tended to improve more in the younger group. No improvement was seen in the “Old-Old” group. The degree of saggital pelvic inclination was calculated from the transverse axis length (b/a) ratio of the radiological shape of the pelvic cavity in antero-posterior radiographs. As a result, we found that the pelvis tended to incline posteriorly with increasing age. Therefore, pelvic anterior inclination was found in all “Adult patients” and pelvic posterior inclination in most “Old-Old patients”. After THA, the pelvis in most patients tended to slightly incline posteriorly over time.

日常生活における前屈動作は股関節と脊椎の協調動作であり互いに密接に関わりあっている.変形性股関節症患者における股関節と脊椎の前屈時の動作関係を明らかにするために,立位中間位および座位前屈位の単純X線側面像を用いて股関節可動域と腰椎可動域を測定した.対象は初回人工股関節全置換術を受けた40~88歳(平均62歳)の変形性股関節症の女性患者164例であり,術前の単純X線で評価した.中間位からの前屈動作で股関節と腰椎と合計して80~120度屈曲していた.腰椎変性の強い高齢者ほど前屈時の腰椎可動域が小さく,変形性股関節症があるのにもかかわらず主に股関節で前屈していた.中高年者は腰椎変性が進行していないため,主に腰椎で前屈しており股関節可動域は小さかった.そのため変形性股関節症の罹病期間が長くなると腰椎へ過度の負荷がかかることとなり,腰痛や腰椎変性の原因となることが予想される.

In the present study, changing in pelvic inclination before total hip arthroplasty (THA) were investigated using 59 patients with osteoarthritis of the hip. The subjects were divided into following four groups by their age: Group A consisted of 21 patients between 45 and 54 years, Group B 10 patients between 55 and 64, Group C 18 patients between 65 and 74, and Group D 10 patients at 75 years or older. Pelvic inclination became retroverted with age, further retroverting at the standing position especially in elderly people. When performing THA on patients with severe pelvic retroversion, alignments of the lumber spine, pelvis, and hip should be carefully considered. Insertion of acetabular socket should be considered in some cases taking into account standing position or pelvic inclination in future. In elderly people with lumbar degenerative kyphosis and severe pelvic retroversion, it was important to insert the acetabular socket in prospect of future changes of the pelvis inclination since the pelvis retroverts according to posture and age.
