Flight attendants experience circadian misalignment and disrupted sleep and eating patterns. This survey study examined working time, sleep, and eating frequency in a sample (n=21, 4 males, 17 females) of Australian flight attendants (mean age=41.8 y, SD=12.0 y, mean BMI=23.8 kg/m2, SD=4.1 kg/m2). Respondents indicated frequencies of snack, meal, and caffeine consumption during their last shift. Reported sleep duration on workdays (mean=4.6 h, SD=1.9 h) was significantly lower than on days off (M=7.2 h, SD=1.2 h, p<0.001), and significantly lower than perceived sleep need (M=8.1 h, SD=0.8 h, p<0.001). Food intake was distributed throughout shifts and across the 24 h period, with eating patterns incongruent with biological eating periods. Time available, food available, and work breaks were the most endorsed reasons for food consumption. Caffeine use and reports of gastrointestinal disturbance were common. Working time disrupts sleep and temporal eating patterns in flight attendants and further research into nutritional and dietary-related countermeasures may be beneficial to improving worker health and reducing circadian disruption.

Cardiovascular disease (CVD) risk in train drivers is associated with health conditions that can result in sudden incapacity. Drivers are at high risk on several CVD risk factors with research suggesting that sleep may predict CVD risk, however this relationship has not yet been explored. This study investigated the link between sleep and CVD risk, in relation to hours of work day and days off sleep. N=309 Australian drivers completed cross-sectional survey. A CVD risk score was calculated by summing scores from behavioural and biomedical risk factors. Sleep was most frequently cited as the main reason for decline in perceived health status. Main analyses showed that shorter work day sleep (M=5.79 h) was a significant predictor of increased CVD risk (p=0.013). This relationship was moderated by days off sleep, such that when days off sleep (M=8.17 h) was higher, the effect of work day sleep on CVD risk was weaker (p=0.047). Findings indicate the amount of sleep a driver obtains on non-work days may compensate for adverse health outcomes. Successful management of fatigue in safety critical occupations appears essential not only for the prevention of safety hazards, but also for the long-term health of shift workers. Further investigation is warranted.

This study examined the impact of eating during simulated night shift on performance and subjective complaints. Subjects were randomized to eating at night (n=5; 23.2±5.5y) or not eating at night (n=5; 26.2±6.4y). All participants were given one sleep opportunity of 8h (22:00h-06:00h) before transitioning to the night shift protocol. During the four days of simulated night shift participants were awake from 16:00h-10:00h with a daytime sleep of 6h (10:00h-16:00h). In the simulated night shift protocol, meals were provided at ≈0700h, 1900h and 0130h (eating at night); or ≈0700h, 0930h, 1410h and 1900h (not eating at night). Subjects completed sleepiness, hunger and gastric complaint scales, a Digit Symbol Substitution Task and a 10-minute Psychomotor Vigilance Task. Increased sleepiness and performance impairment was evident in both conditions at 0400h (p<0.05). Performance impairment at 0400h was exacerbated when eating at night. Not eating at night was associated with elevated hunger and a small but significant elevation in stomach upset across the night (p<0.026). Eating at night was associated with elevated bloating on night one, which decreased across the protocol. Restricting food intake may limit performance impairments at night. Dietary recommendations to improve night-shift performance must also consider worker comfort.

This period of impairment is of concern to workers who are on-call, or nap during work hours, and need to perform safety-critical tasks soon after waking. While several studies have investigated the best sleep timing and length to minimise sleep inertia effects, few have focused on countermeasures -especially those that can be implemented after waking (i.e. reactive countermeasures). This structured review summarises current literature on reactive countermeasures to sleep inertia such as caffeine, light, and temperature and discusses evidence for the effectiveness and operational viability of each approach. Current literature does not provide a convincing evidence-base for a reactive countermeasure. Caffeine is perhaps the best option, although it is most effective when administered prior to sleep and is therefore not strictly reactive. Investigations into light and temperature have found promising results for improving subjective alertness; further research is needed to determine whether these countermeasures can also attenuate performance impairment. Future research in this area would benefit from study design features highlighted in this review. In the meantime, it is recommended that proactive sleep inertia countermeasures are used, and that safety-critical tasks are avoided immediately after waking.

Fatigue is an increasingly recognised risk factor for transportation accidents. In light of this, there is the question of whether driving whilst fatigued should be a criminal offence. This paper discusses the current legal position, including the problems of voluntary conduct and self awareness. Three models for reform are proposed. The manner in which scientific research can inform legal consideration and future directions for research are discussed.
