入院,入所している難聴高齢者の難聴に由来する思いとそれに影響を与えている要因を明らかにし,看護の課題を導くことを目的とし,様々なタイプの施設計8箇所から計42名の65歳以上の難聴者に面接と参加観察を行なった。対象者が表現した難聴に由来する思いについて繰り返し述べているものを「主題」とし,これに主に関連している「環境要因」「個人要因」を比較検討した。その結果,6つのグループが形成され,各グループの「主題」は,【寂しい,つらい】【聞き取れないのは相手や環境が悪い】【難聴の為に不当な対応を受けている】【相手に申し訳ない】【大事な話の時に困る】【難聴でも困らない】であった。「主題」に関連している「個人要因」は会話への価値観であった。さらに,その価値観からくる会話へのニードに対して「環境要因」がどのように満たしているか/いないのかという点で「主題」に影響を与えていた。入院,入所している難聴高齢者に対して,『会話に楽しみを感じているか』『必要な情報は得られているか』『人間関係はうまくいっているか』を把握する看護援助の必要性が示唆された。This study, the purpose of which is the clarification of the state of mind of hearing-impaired elderly patients in hospitals or long-term care settings and related factors in order to direct relevant nursing issues, was carried out by interview and participatory observation at 8 facilities of different types with 42 individuals over the age of 65. Reoccurrent comments by patients regarding the hearing-impaired state of mind were considered as "subjects", while directly related comments were comparatively reviewed as "environmental factors" and "personal factors". As a result, 6 "subject" groups were formed; [lonely, painful], [impaired hearing caused by averse conversation partners or environments], [unreasonable treatment received due to impaired hearing], [remorse for their treatment of conversation partners], [trouble in important conversations] and [no impaired hearing-related trouble]. "Personal factors" related to "subjects" consisted of values related to conversations. Additionally, satisfaction - or the lack thereof -of the need arising from the said value by "environmental factors" affected the original "subjects". The study indicates the need for nursing support extended to hearing-impaired elderly patients in facilities to be fully aware of "enjoyment of the conversation", "provision of sufficient information", and "adequate progress with human relationships".
