Objective: The success of a client-centred practice depends on the relationship between the client and therapist and on their ability to make constructive decisions together, particularly in the field of occupational therapy. The aim of this study was to develop a Occupational Therapy Collaborative Relationship Scale (OTCRS) to measure the quality of such interaction. Methods: This work included constructing a draft questionnaire and testing its validity and reliability. A Rasch analysis was applied to determine its validity, and several tests were used to confirm its internal consistency. Results: After reviewing more than 130 scientific papers and books, we built explicit selection criteria for issues to be addressed in this instrument, and we developed 40 questions to be included. These were analysed using a standard content validation process and a Rasch analysis to examine confirmation validity. A nine-item scale was finalised for testing (OTCRS-9). This review process revealed the validity, high internal consistency, and item/person separation reliability of OTCRS-9. Conclusion: This study presents only the initial phase of scale development. As suggested by the Consensus-based Standards for the Selection of Health Measurement Instruments (COSMIN), the OTCRS-9 score should be tested further for validity and reliability and should also be conducted in subjects of other ethnicities to improve its generalizability.

Objective/Background: In this study, we classified the psychological adjustments of family members of terminal cancer patients using a conceptual structure based on recurring cases. We hypothesised that it is possible to determine the need for intervention on the basis of the occupational performance patterns of the family members.Methods: The subjects were bereaved family members of cancer patients who died more than a year ago. For our classification system, we adopted code matrix analysis using MAXQDA10 analysis software.Results: We categorised the 9 cases into 3 types on the basis of functionality with regard to occupation: i) type of continued occupation; ii) type of resumed occupation; iii) type of interrupted occupation. Conclusions: The categorisation of occupational types assumes that occupation has an effect on the family members' ability to progress through or alter mental states according to occupational performance patterns.

The primary goal of occupational therapy is to enable people to participate in normal everyday activities. To understand and support a client's desired activities, it is necessary to share the meaning of occupation between the therapist and client. One concept that has been used to capture the psychological state for an activity is flow. Therefore, we devised a process of adjusting the challenge-skill balance for occupational therapy based on flow theory. In this pilot study, we aimed to verify whether adjusting the challenge-skill balance for occupational therapy improved subjective quality of life in a recovery rehabilitation unit. To inform the future design of randomized controlled trials, we therefore compared standard occupational therapy with this new approach. Both interventions took place from recovery rehabilitation unit entry to discharge, and outcomes (continuous variables) were analyzed using a Bayesian approach explored with generalized linear mixed modeling. Among a total of 22 patients, those receiving the new approach showed a significant improvement in Ikigai-9 (quality of life) compared with those receiving standard occupational therapy. The mean improvement was 4.44 ± 2.17 with a 95% credible interval of 0.104-8.713. The dependence factor, effective sample size, and autocorrelation time were 3.769, 8093, and 6.18, respectively. We conclude that adjusting the challenge-skill balance during occupational therapy could improve a client's subjective quality of life.
