Several reports indicate an important role for vitamin K in bone health as well as blood coagulation. However, the current Adequate Intakes (AI) might not be sufficient for the maintenance of bone health. To obtain a closer estimate of dietary intake of phylloquinone (PK) and menaquinones (MKs), PK, MK-4 and MK-7 contents in food samples (58 food items) were determined by an improved high-performance liquid chromatography method. Next, we assessed dietary vitamin K intake in young women living in eastern Japan using vitamin K contents measured here and the Standard Tables of Food Composition in Japan. PK was widely distributed in green vegetables and algae, and high amounts were found in spinach and broccoli (raw, 498 and 307 μg/100 g wet weight, respectively). Although MK-4 was widely distributed in animal products, overall MK-4 content was lower than PK. MK-7 was observed characteristically in fermented soybean products such as natto (939 μg/100 g). The mean total vitamin K intake of all subjects (using data from this study and Japanese food composition tables) was about 230 μg/d and 94% of participants met the AI of vitamin K for women aged 18-29 y in Japan, 60 μg/d. The contributions of PK, MK-4 and MK-7 to total vitamin K intake were 67.7, 7.3 and 24.9%, respectively. PK from vegetables and algae and MK-7 from pulses (including fermented soybean foods) were the major contributors to the total vitamin K intake of young women living in eastern Japan.

Background: The Standard Tables of Food Composition in Japan do not include information on trans fatty acids. Previous studies estimating trans fatty acid intake among Japanese have limitations regarding the databases utilized and diet assessment methodologies. We developed a comprehensive database of trans fatty acid food composition, and used this database to estimate intake among a Japanese population.Methods: The database was developed using analytic values from the literature and nutrient analysis software encompassing foods in the US, as well as values estimated from recipes or nutrient compositions. We collected 16-day diet records from 225 adults aged 30 to 69 years living in 4 areas of Japan. Trans fatty acid intake was estimated based on the database and the 16-day diet records.Results: Mean total fat and trans fatty acid intake was 56.9 g/day (27.7% total energy) and 1.7 g/day (0.8% total energy), respectively, for women and 66.8 g/day (25.5% total energy) and 1.7 g/day (0.7% total energy) for men. Trans fatty acid intake accounted for greater than 1% of total energy intake, which is the maximum recommended according to the World Health Organization, in 24.4% of women and 5.7% of men, and was particularly high among women living in urban areas and those aged 30–49 years. The largest contributors to trans fatty acid intake were confectionaries in women and fats and oils in men.Conclusions: Although mean trans fatty acid intake was below the maximum recommended intake of the World Health Organization, intake among subgroups was of concern. Further public health efforts to reduce trans fatty acid intake should be encouraged.

BACKGROUND: Few epidemiologic studies have examined the association between the rate of eating and obesity. In this study, we cross-sectionally examined the association of the self-reported rate of eating with current Body Mass Index (BMI), and BMI-change from 20 years of age to the current age.METHODS: Subjects were 3737 male (mean age ± standard deviation and mean BMI ± standard deviation: 48.2 ± 7.1 years and 23.3 ± 2.7 kg/m2) and 1005 female (46.3 ± 7.0 years and 21.8 ± 2.8 kg/m2) Japanese civil servants. We measured self-reported categorical rate of eating, current BMI, BMI at age 20, and BMI-change from age 20. Energy intake was assessed over a 1-month period with a brief-type diet history questionnaire.RESULTS: The multiple regression analysis in which the current BMI was regressed by categorical rate of eating, energy intake, age, and lifestyle factors showed that current BMI steadily increased by -0.99, -0.67, 0.81, and 1.47 kg/m2 along with the progress of categorical rate of eating from the 'medium' group to 'very slow', 'relatively slow', 'relatively fast', and 'very fast' groups, respectively, in men. In women, the corresponding values were -1.06, -0.35, 0.50, and 1.34 kg/m2. When the BMI increment from age 20 to current age was regressed in the same manner, the increment was -0.63, -0.34, 0.57, and 1.05 kg/m2 in men and -0.71, -0.32, 0.34, and 1.14 kg/m2 in women, respectively. Additionally, both BMI at age 20 and current height were positively associated with rate of eating.CONCLUSIONS: Our results among middle-aged men and women suggest that eating fast would lead to obesity.J Epidemiol 2006; 16: 117-124.

Background: Depression in elderly people is a major global concern around the world. Epidemiological evidence of the association of beverages with depressive symptoms has received research attention; however, epidemiological studies on the association of coffee and green tea with depressive symptoms for the elderly population are limited. The objective of this study is to cross-sectionally examine the association of depressive symptoms with the intake of coffee, green tea and caffeine and to verify the antidepressant effect of caffeine.Methods: The subjects were 1992 women aged 65–94 years. Intakes of coffee, green tea and caffeine as well as depressive symptoms were assessed with a validated brief dietary history questionnaire (BDHQ) and the Center for Epidemiologic Studies Depression Scale (CES-D), respectively. Multiple logistic regression analysis was used to calculate odds ratios (OR) and 95% confidence intervals (CI) for depressive symptoms with adjustments for potential confounders.Results: Coffee intake was associated with a lower prevalence of depressive symptoms, the ORs of which for the 4th vs. the 1st quartiles of intake was 0.64 (95% CI 0.46-0.88, P for trend = 0.01) in a fully adjusted model. Caffeine intake was marginally associated with depressive symptoms, but the association was not statistically significant (OR = 0.75; 95% CI 0.55–1.02, P for trend = 0.058).Conclusion: The result suggests that the inverse association on depressive symptoms might be expected not only caffeine intake but also some other substances in coffee or factors related to coffee intake. Because of its cross-sectional design, longitudinal studies are required to confirm the present finding.

Background: Diet is a modifiable factor that may affect sleep, but the associations of macronutrient intakes with insomnia are inconsistent. We investigated the associations of protein, fat, and carbohydrate intakes with insomnia symptoms.Methods: In this cross-sectional analysis of 4435 non-shift workers, macronutrient intakes were assessed by the brief-type self-administered diet history questionnaire, which requires the recall of usual intakes of 58 foods during the preceding month. Presence of insomnia symptoms, including difficulty initiating sleep (DIS), difficulty maintaining sleep (DMS), and poor quality of sleep (PQS) were self-reported. Logistic regression analysis was used to estimate odds ratios (ORs) and 95% CIs adjusted for demographic, psychological, and behavioral factors, as well as medical histories.Results: Low protein intake (<16% vs ≥16% of total energy) was associated with DIS (OR 1.24, 95% CI 0.99–1.56) and PQS (OR 1.24, 95% CI 1.04–1.48), while high protein intake (≥19% vs <19% of total energy) was associated with DMS (OR 1.40, 95% CI 1.12–1.76). Low carbohydrate intake (<50% vs ≥50% of total energy) was associated with DMS (OR 1.19, 95% CI 0.97–1.45).Conclusions: Protein and carbohydrate intakes in the daily diet were associated with insomnia symptoms. The causality of these associations remains to be explained.

We compared fatty acid intake estimated from our 138-item food frequency questionnaire (FFQ) with 28-day weighed dietary records among a subgroup of JPHC Study Cohort 1 (102 men and 113 women), and with the corresponding two serum phospholipid levels (88 men). Spearman rank correlation coeffi- cients between fatty acid intakes estimated from FFQ and intakes estimated from DR were as follows: saturated fatty acid, r=0.61 and r=0.60; monounsaturated fatty acid, r=0.50 and r=0.44; for energy adjusted value and Eicosapentaenoic acid (EPA), r=0.62 and r=0.55; docosahexaenoic acid (DHA), r=0.61 and r=0.50; for percentage of total fatty acid intake in men and women, respectively. Spearman rank correlation coefficients between fatty acid intakes estimated from FFQ and the corresponding serum phospholipid levels (% of total fatty acid) were as follows: EPA, r=0.43 and r=0.59; DHA, r=0.35 and r=0.49; for crude value (g/day) and percentage of total fatty acid intake, respectively. In conclusion, relatively high correlations were observed for SFA, MUFA and marine-origin n-3 polyunsaturated fatty acid, whereas we must take into account the indicator of each fatty acid intake when using the data of fatty acid intake assessed with FFQ for JPHC study.J Epidemiol 2003;13(Suppl):S64-S81.

Background: The disparity of overall diet quality by personal educational attainment has been a public issue. However, it remains unknown which food groups contribute to the disparity. This cross-sectional study assesses which food groups explain associations between education and overall diet quality in Japanese women.Methods: A total of 3788 middle-aged (mean: 47.7 years) and 2188 older women (mean: 74.4 years), who lived in 47 prefectures in Japan, provided data on their education (low, middle, and high) and dietary intakes from a diet history questionnaire. A diet quality score (possible score 0-70) was calculated based on seven food components. Mean diet quality scores with adjustment for lifestyle and neighborhood variables were estimated by education, and Dunnett’s multiple comparison was conducted. Additionally, mean scores of each food component were estimated by education and compared using the same manner.Results: After adjustment for lifestyle and neighborhood variables, mean diet quality score of high or middle education was higher than low education for both generations. Middle-aged women with high and middle education had higher scores of ‘milk’, ‘snacks, confection, and beverages’, ‘fruits’, and ‘vegetable dishes’ than those with low education. Older women with high and middle education had higher scores of ‘sodium from seasonings’ and ‘fruits’ than those with low education.Conclusions: This study suggests that positive associations between education and diet quality are explained by different food groups in middle-aged and older Japanese women, which are independent of lifestyle and neighborhood variables.

Objectives: Although workers with poor sleep quality are reported to have problems with work performance, few studies have assessed the association between dietary factors and sleep quality using validated indexes. Here, we examined this association using information acquired from validated questionnaires. Methods: A total of 3,129 female workers aged 34 to 65 years were analyzed. Dietary intake was assessed using a self-administered diet history questionnaire (DHQ), and subjective sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI). The relationship between the intake of several food groups and nutrients and sleep quality was examined using multivariable logistic regression models. The effect of eating habits on sleep quality was also examined. Results: Poor sleep quality was associated with low intake of vegetables (p for trend 0.002) and fish (p for trend 0.04) and high intake of confectionary (p for trend 0.004) and noodles (p for trend 0.03) after adjustment for potential confounding factors (age, body mass index, physical activity, depression score, employment status, alcohol intake and smoking status). Poor sleep quality was also significantly and positively associated with consumption of energy drinks and sugar-sweetened beverages, skipping breakfast, and eating irregularly. In addition, poor sleep quality was significantly associated with high carbohydrate intake (p for trend 0.03). Conclusion: A low intake of vegetables and fish, high intake of confectionary and noodles and unhealthy eating habits were independently associated with poor sleep quality. Poor sleep quality was also associated with high carbohydrate intake in free-living Japanese middle-aged female workers.

Recent studies have shown that the urinary excretion levels of water-soluble vitamins can be used as biomarkers for the nutritional status of these vitamins. To determine changes in the urinary excretion levels of water-soluble vitamins during pregnant and lactating stages, we surveyed and compared levels of nine water-soluble vitamins in control (non-pregnant and non-lactating women), pregnant and lactating women. Control women (n=37), women in the 2nd (16-27 wk, n=24) and 3rd trimester of pregnancy (over 28 wk, n=32), and early- (0-5 mo, n=54) and late-stage lactating (6-11 mo, n=49) women took part in the survey. The mean age of subjects was ~30 y, and mean height was ~160 cm. A single 24-h urine sample was collected 1 d after the completion of a validated, self-administered comprehensive diet history questionnaire to measure water-soluble vitamins or metabolites. The average intake of each water-soluble vitamin was ≈ the estimated average requirement value and adequate intake for the Japanese Dietary Reference Intakes in all life stages, except for vitamin B6 and folate intakes during pregnancy. No change was observed in the urinary excretion levels of vitamin B2, vitamin B6, vitamin B12, biotin or vitamin C among stages. Urine nicotinamide and folate levels were higher in pregnant women than in control women. Urine excretion level of vitamin B1 decreased during lactation and that of pantothenic acid decreased during pregnancy and lactation. These results provide valuable information for setting the Dietary Reference Intakes of water-soluble vitamins for pregnant and lactating women.

Most Japanese women do not consume the estimated average requirement of vitamin B6 (1.7 mg/d) during pregnancy. Nevertheless, these deficiencies are not reported. We investigated a nutritional biomarker of vitamin B6 in pregnant Japanese women as well as their vitamin B6 intakes. Vitamin B6 intakes in the first, second, and third trimesters of pregnancy, and 1 mo after delivery were 0.79±0.61 (n=56), 0.81±0.29 (n=71), 0.90±0.35 (n=92), and 1.00±0.31 (n=44) mg/d, respectively. Plasma pyridoxal 5'-phosphate (PLP) concentrations in the first, second, and third trimesters of pregnancy, and 1 mo after delivery were 57.1±27.6 (n=56), 23.3±16.7 (n=71), 18.3±12.5 (n=92), and 43.9±33.4 (n=44) nmol/L, respectively. The plasma concentrations significantly decreased in the second and third trimesters of pregnancy compared to values from the first trimester (p<0.05), and these concentrations returned to the values of the first trimester of pregnancy 1 mo after birth.

No study has documented Japanese children’s food portion sizes. Since this information is essential to establish valid measurement tools and effective education methods for dietary intake, we measured them using one-day, semi-weighed diet records (DRs) for 489 Japanese children aged 3–6 y. Each food’s frequency of appearance on the DRs was counted. If a child consumed a certain food more than once per day, an average weight for that food was calculated and used in the portion size calculation as that child’s representative value. In total, 67 food items were consumed by twenty or more children. We calculated the mean, standard deviation, median, minimum, and maximum portion size values for these food items. In addition to these 67 items, Chinese noodles and water were included in the analysis. The most frequently consumed food was well-milled rice (mean portion size for rice=79 g), which was consumed by 350 children, followed by soy sauce (4 g), eggs (26 g), and carrots (9 g). Among the five most frequently consumed foods, portion sizes of rice and pork but not milk, eggs, or carrots significantly increased with age, height, and weight. In multivariate linear regression analysis, however, the significant relationships between rice portion size and age were not observed. Regarding pork, only the relationship with height was significant. A sex difference was detected in the rice and milk portion sizes. Most of portion sizes observed here were apparently smaller than those reported among United Kingdom children. This study provides important basic information for the implementation of quantitative nutritional research and educational efforts for Japanese preschool children.

Over 700 bacterial species have been detected in the oral cavity. Several studies have suggested that periodontitis is associated with systemic disorders such as diabetes mellitus, indicating a key role for oral microbiota in human health. However, the relationship between oral microbiota and diabetes has not been well clarified. Therefore, we conducted microbiome analysis of saliva samples obtained from 15 elderly residents (3 with type 2 diabetes mellitus [DM] and 12 without diabetes [non-DM]) at three different nursing homes, as well as 9 young healthy controls (HC). Genomic DNA was extracted from each sample, and then the V4 region of the 16S rRNA gene was amplified and sequenced. Alpha diversity, in terms of operational taxonomic unit richness, was significantly higher in samples from the non-DM group than in those from the HC group. Weighted UniFrac distance analysis showed that salivary microbial communities in the DM group were separately clustered. Furthermore, in the DM group, Actinomyces and Selenomonas showed significantly higher abundance, whereas Alloprevotella showed significantly lower abundance, relative to the non-DM group. Although our findings were limited by the small sample size, oral bacterial diversity in the DM group was clearly different from that in the non-DM group.

It has been reported that treatment with a pharmacological dose (45 mg/d) of menaquinone-4 (MK-4) prevents bone loss in postmenopausal women. However, it is not known whether supplementation with low dose MK-4 has beneficial effects on bone metabolism in healthy women. The aim of this study is to examine the effects of the supplementation of 1.5 mg/d MK-4 for 4 wk on bone and lipid metabolism in healthy postmenopausal Japanese women. The study was performed as a randomized double blind placebo-controlled trial. The participants aged 53-65 y were randomly assigned to 2 groups and supplemented with 1.5 mg/d of MK-4 or a placebo for 4 wk (n=20 for each group). The most marked effects of MK-4 intake were observed on serum osteocalcin (OC) concentrations. Serum undercarboxylated OC (ucOC) concentration decreased, and the γ-carboxylated OC (GlaOC) and GlaOC/GlaOC+ucOC ratio that indicates the degree of OC γ-carboxylation increased significantly at 2 and 4 wk compared with that at baseline in the MK-4 group. The serum ucOC and GlaOC concentrations in the MK-4 group were significantly different from those in the placebo group at 2 wk. These results suggest that supplementation with 1.5 mg/d MK-4 accelerated the degree of OC γ-carboxylation. The concentrations of serum lipids and other indices were not different between the groups at either intervention period. Thus, the additional intake of MK-4 might be beneficial in the maintenance of bone health in postmenopausal Japanese women.

The possible role of eating rate in promoting obesity has long been of interest. We examined the association between rate of eating and overweight status in Japanese children and adolescents. This Japanese cross-sectional study included 15,974 children (6-11 y; 7,956 boys and 8,018 girls) and 8,202 adolescents (12-15 y; 3,944 boys and 4,258 girls). Rate of eating was self-reported according to five categories. Body mass index (BMI) was calculated from self-reported body weight and height. Overweight was defined according to the International Obesity Task Force age- and sex-specific BMI cutoffs. The overall prevalence of overweight was 13.2%. Rate of eating was positively associated with the risk of overweight, independent of protein, fat, and dietary fiber intakes. Multivariate odds ratios (95% confidence intervals) for overweight in the very slow, relatively slow, medium, relatively fast, and very fast categories were 0.31 (0.20, 0.49), 0.49 (0.40, 0.60), 1 (reference), 2.81 (2.42, 3.26), and 4.49 (3.47, 5.81) in male children; 0.42 (0.31, 0.58), 0.49 (0.41, 0.59), 1 (reference), 2.74 (2.27, 3.31), and 5.69 (3.75, 8.63) in female children; 0.13 (0.03, 0.54), 0.43 (0.28, 0.65), 1 (reference), 2.31 (1.88, 2.84), and 3.84 (2.77, 5.31) in male adolescents; and 0.55 (0.30, 1.01), 0.46 (0.33, 0.65), 1 (reference), 1.30 (0.99, 1.71), and 1.49 (0.84, 2.65) in female adolescents, respectively (all p for trend<0.0001). In conclusion, this large cross-sectional study in Japan showed that a higher rate of eating was independently positively associated with an increasing risk of overweight in children and adolescents.
