Dietary factors were analyzed for the regional difference of GI tract cancer mortality rates in China. Sixty-five rural counties were selected among a total of 2, 392 counties to represent a range of rates for seven most prevalent cancers. The dietary data in the selected 65 counties were obtained by three-day dietary record of households in 1983. The four digestive cancer mortality rates (annual cases per 100, 000 standardized truncated rates for ages 35-64) and per capita food consumption were analyzed by the principal components factor analysis. Esophageal cancer associated with poor area, dietary pattern rich in starchy tubers, and salt, lack of consumption of meat, eggs, vegetables and rice. Stomach cancer seemed to be less associated with diet in this study because of its small model Kaiser-Meyer-Olkin (KMO) measure of sampling adequacy, suggesting some other carcinogenic factors would play more important role in the development of this cancer in China. The colon and rectal cancer showed close relation to diet; rich in sea vegetables, eggs, soy sauce, meat and fish, while lack in consumption of milk and dairy products. Rapeseed oil was more important risk factor for colon cancer than that of rectum. Rice, processed starch and sugar were closely associated with colon cancer, supporting the insulin/colon cancer hypothesis. J Epidemiol, 1999 ; 9 : 275-284.

The intake of flavonoids and isoflavonoids was estimated based upon a preliminary database of 40 food items, covering at least 80% of total food consumption. Fifty volunteer women in "I- City" recorded the weight of all dietary intake for 3 days in September 1996, and received a health check-up, as well as laboratory examination. The data was analyzed in relation to the various food factors. Average daily intake per capita of flavonoids was as follows: 4.9 mg kaempferol, 8.3 mg quercetin, 1.5 mg rutin, 0.6 mg myricetin, 0.3 mg luteolin, 0.01 mg myricitrin, 0.4 mg fisetin, and 0.3 mg eriodictyol. Total intake from vegetables and fruits was less than 10 mg. 16.2 mg (range: 3.18-35.61 mg) and 23.27 mg (4.62-52.12 mg) of isoflavones, such as daidzein and genistein, respectively, were taken per day, and total isoflavone intake was 39.46 mg (7.80-87.73 mg). Chief component analysis on ingested vitamins, flavonoids and isoflavonoids was carried out. Factor 1 was mainly composed of flavonoids and antioxidant vitamins. Factor 1 was positively associated with age and the level of HDL cholesterol and negatively related to the level of triglycerides. Factor 2, which was mainly composed of isoflavonoids, was positively associated with creatinine and uric acid levels. So far, these factors did not show a significant association with bone density and other health indices, such as BMI and blood pressure. J Epidemiol, 1998 ; 8 : 168-175.

Cerebrovascular disease was a leading cause of death from 1955 to 1980 in Japan. The mortality rate from this disease has decreased sharply in recent decades. This downward trend seems to correspond to the dietary habits of Japanese. Data from a large prospective cohort study were analyzed to examine the association between dietary habits and cerebrovascular disease mortality in Japan. The subjects for this analysis were 223, 170 men and women aged 40 to 69 at baseline in December 1965. There were 6, 168 deaths in men and 4, 862 deaths in women due to cerebrovascular disease (ICD7: 330-334) during the follow-up period from January 1966 to December 1981. Rate ratio (RR) and 95% confidence interval (95%Cl) adjusted for sex, attained age, follow-up period, prefecture, cigarette smoking, alcohol drinking and occupation was used for comparison. In this study, the risk of mortality from cerebrovascular disease was inversely associated with dairy milk, meat and fish consumption. Therefore the joint effect of dairy milk, meat and fish (DMF) as animal fat and protein was of interest. In the binary analysis, DMF (D, M, F) means the combination of dairy milk (1-3 times/week or more), meat (1-3 times/week or more) and fish (4 times/week or more). Thus DMF (d, m, f) was the reference group having dairy milk (less than 1 time/week), meat (less than 1 time/week) and fish (less than 4 times/week). For the disease, the RR of DMF (D, M, F) was 0.68 with 95%CI of 0.63 to 0.74, relative to the reference group. Furthermore the joint effect of DMF was more strongly associated with cerebral haemorrhage (ICD7: 331, DMF (D, M, F); RR: 0.63, 95%Cl: 0.55-0.70) than with cerebral embolism and thrombosis (ICD7: 332, DMF (D, M, F); RR: 0.79, 95%Cl: 0.70- 0.89). These findings suggest that the increasing intake of animal fat and/or protein may have played a key role in reducing cerebrovascular disease in Japan. J Epidemiol, 1999 ; 9 : 268-274.

The purpose of this study was to propose a hypothesis that there is a potential association between the incidence of selective IgA deficiency in various countries and COVID-19 cases. The number of deaths due to COVID-19 increased in clear proportion to the number of infected patients, and the difference in the number of deaths by country was due to the difference in the number of infected patients. The frequency of selective IgA deficiency has a strong positive correlation with the prevalence of COVID-19 per population. The low infection rate contributed to the low death rate from COVID-19 in Japan, suggesting that the extremely low frequency of selective IgA deficiency may be a contributing factor.

Background: The personality of healthy individuals has not been well studied in relation to health consciousness, dietary habits and actual food intake, simultaneously. Objective: Our objective was to study the association between personality and dietary habits. Design: Information on dietary habits, including taste preferences and the frequency of food consumption, was collected through a questionnaire from 76 male and 394 female students. The personality of students was determined by a modified NEO-FFI test. Health status, height, body weight, body fat percentage and blood pressure were measured by physical examination. Main outcome measures were personality scores as indicators of a healthy dietary pattern. Results: Food intake was influenced by neuroticism (N), extraversion (E), openness (0) and agreeableness (A) of personality. Taste preferences and receptivity to dietary advice were also influenced by personality: the odds ratios (ORs) between the high and low tertiary points of the NEO-FFI scores for salty and sweet taste preferences were significantly higher in the group that scored high for neuroticism (N) (salty taste preference: OR=2.25, NS in males and OR=2.39, 95%CI=1.16-4.93 in females; sweet taste preference: OR=21.00, 95%CI=2.40-183.99 in males and OR=3.33, 95%CI=1.61-6.91 in females). On the other hand, the groups with high scorer for 0 and A did not like salty tastes. The groups with high scores for A and C were receptive to dietary advice. High scores of each N, E, 0, A, and C factor were characterized by distinguishable, dietary habits and lifestyle. For nutritional or health education, group classes are sufficient for high A and O. High C scorer displayed discrepancies between health consciousness and dietary habits, so intervention or a close follow-up by medical professionals would be necessary to improve the health of individuals in this group. High E scorer possessed a confident attitude towards their health, but they were not interested in developing healthy habits. High N scorer was adverse to receiving health information and learning healthy dietary habits. Conclusion: Personality determined by NEO-FFI test was related to dietary habits and health attitude. We report here a historical cohort study on cancer mortality among Minamata disease (MD) patients (n=1, 351) in Kagoshima and Kumamoto Prefectures of Japan. Taking into account their living area, sex, age and fish eating habits, the residents (n=5, 667; 40 years of age or over at 1966) living in coastal areas of Kagoshima, who consumed fish daily, were selected as a reference group from the six-prefecture cohort study conducted by Hirayama et al. The observation periods of the MD patients and of the reference group were from 1973 to 1984 and from 1970 to 1981, respectively. Survival analysis using the Poisson regression model was applied for comparison of mortality between the MD patients and the reference group. No excess of relative risk (RR) adjusted for attained age, sex and follow-up period was observed for mortality from all causes, all cancers, and non-cancers combined. Analysis of site-specific cancers showed a statistically significant decrease in mortality from stomach cancer among MD patients (RR, 0.49; 95 % confidence interval, 0.26-0.94). In addition, a statistically significant eight-fold excess risk, based on 5 observed deaths, was noted for mortality from leukemia (RR, 8.35; 95 % confidence interval, 1.61-43.3). It is, however, unlikely for these observed risks to be derived from methylmercury exposure only. Further studies are needed to understand the mechanisms involved in the observed risks among MD patients. J Epidemiol, 1996 ; 6 : 134-138.

The Japan Public Health Center-based Prospective Study on Cancer and Cardiovascular Diseases (JPHC Study; formerly called “Koseisho Multipurpose Prospective Cohort Study”) began in 1990(Cohort I) and 1993(Cohort II). The JPHC Study covers 11 public health center areas throughout Japan and includes a total of 140, 420 residents. The study's design includes a baseline survey with a self-administered questionnaire on lifestyle and collection of blood and health checkup data; a follow-up system for mortality, migration, and incidence of cancer and cardiovascular diseases; an additional follow-up survey after 5 and 10 years; and distribution of a newsletter. The JPHC study is expected to provide evidence for the prevention and control of cancer and cardiovascular diseases in the 21st century. J Epidemiol, 2001 ; 11 (Suppl) : S3-S7.
