The human life-span is increasing worldwide. During the 1970s, senior citizens comprised 7.1% of the population in Japan, and by 1994, they constituted 14% of the population. Life expectancy has risen to 79.19 years for men and 85.99 years for women. This extended lifespan of seniors brings with it an increased need for improvement in quality of life. One intervention that can be used to meet this need is long-term music therapy. The combination of singing and listening to "familiar music"or"reminiscent music"while utilizing social activity was explored in the context of an evidence- based medicine framework. A deep understanding of the elderly is important in order to determine the effects of the music therapy and their relationship with the life history of the patient.

Here we present the case of a child who visited the university with the chief complaint of dysarthria. He initially suffered from dysarthria and mild mental retardation, but later developed serious problems with reading and writing. We considered his phonological processing ability and a possible connection with developmental dyslexia. The child was a 7-year-old boy who was in second grade at a regular elementary school. At 5 years of age, he was diagnosed with a submucous cleft palate and underwent surgery to correct this. After surgery, he was evaluated and started receiving training. He was diagnosed with language delay due to mild mental retardation and dysgraphia accompanying underdeveloped dysarthria, and received 7 months of ST training. In the re-evaluation that followed his training, he was not seen to have general mental retardation, but his problems with reading and writing became apparent. Cross-examining his problems reading and writing with a triangle model and visual cognition processes suggested a problem with bilateral processing of writing and phonology. This was a decoding problem, which is the basic condition of a patient with developmental dyslexia. The boy's core problem thus appeared to be a phonological type of developmental dyslexia.

The purpose of this report is to clarify the roles and tasks of medical social workers in terminally ill patient care. An analysis was performed based on tasks of medical welfare, which were gathered through focus group interviews carried out on the medical staff at "O" Acute Hospital, "A" city. As a result of the survey analysis, it became clear that the target and viewpoint of traditional medical social work, such as response to problems of poverty and the overall relationship of the person and the situation, are needed and as such, new roles, such as construction of a regional cooperative system, the relationship to the viewpoint of living assurance, and the provision of resource ill patient care, by providing a differentiation of functions at medical institutions through a change in medical policy.
