Objectives: Fitness is essential to specialist police forces, who have higher occupational demands than general police, and vital to performance and mission success. However, little research has been done profiling the metabolic fitness of these units and how they compare to other populations. The objective of this study was to profile the aerobic fitness of a specialist police unit. Methods: Body weight was measured to account for any impact on metabolic fitness, while VO2 max was estimated via number of shuttles completed on the 20 m Progressive Shuttle Run Test (PSRT) (n=47) on two dates one calendar year apart. Results: There were no significant (p=.116) differences (mean difference 0.40±1.70 kg) in body weight between the initial measures (mean=88.84±8.25 kg) and the final measure (mean=89.24±8.77 kg) 13 months later. PSRT results increased significantly (p<.005) between the initial (mean=72.62±11.76 shuttles) and final assessments (77.51±11.46 shuttles), with a mean increase of 4.89 (± 2.94) shuttles and a small effect size (d=0.42). The mean VO2 max of the specialist police unit was 51.06±3.61 ml/min/kg following the first assessment, and 52.56±3.46 ml/min/kg following the second assessment. This was a significant finding (p<.001), with a mean difference of 1.19±1.27 ml/min/kg and a small effect size (d=0.23). Conclusions: Elite police forces have a higher metabolic fitness than the general population and general duties police officers. Having and maintaining this fitness level is imperative for their operational success and preventing injuries. This research suggests that despite the challenges posed by operational requirements, high fitness standards can not only be maintained, but also improved.

Objectives: To evaluate the effects of a 2-month lifestyle intervention for dry eye disease in office workers.Methods: Prospective interventional study (randomized controlled study). Forty-one middle-aged Japanese office workers (men, 22; women, 19; 39.2 ± 8.0 years) with definite and probable dry eye disease were enrolled and randomized to an intervention group (n = 22) and a control group (n = 19). The intervention aimed at modifying diet, increasing physical activity, and encouraging positive thinking. The primary outcome was change in dry eye disease diagnoses. Secondary outcome was change in disease parameters, including dry eye symptoms, as assessed using the Dry Eye-Related Quality of Life Score, corneal and conjunctival staining scores, tear break-up time, and Schirmer test results.Results: A total of 36 participants (intervention group, 17; control group, 19) completed the study. The number of definite dry eye disease diagnoses decreased from four to none (p =.05), and the dry eye symptom score showed a significant decrease in the intervention group (p =.03). In contrast, the corneal and conjunctival staining scores, tear break-up time, and Schirmer test results did not differ significantly between groups.Conclusions: The 2-month lifestyle intervention employed in this study improved dry eye disease status among office workers, with a considerable decrease in subjective symptoms. Lifestyle intervention may be a promising management option for dry eye disease, although further investigation of long-term effects are required.

Objectives: After presbycusis, noise exposure is considered the second cause of sensorineural hearing loss. Due to exposure to high-intensity sounds, musicians may be at risk of noise-induced hearing loss (NIHL). Given the importance of good hearing in music career, this study aimed to investigate the frequency of hearing loss and use of protective measures among Iranian musicians. Methods: In this cross-sectional study, 125 musicians, including 21 women (16.8%) and 104 men (83.2%), with at least five years of work experience were recruited. All participants underwent clinical and audiometric examinations. Demographic data, complaints about hearing loss, and information about the use of protective devices were collected through interviews. Results: Audiometric notch in either one or both ears and bilateral hearing loss were present in respectively 42.4% and 19.2% of the participants. The history of tinnitus after performance and ear pain during performance was reported by 64 (51%) and 35 (28%) individuals, respectively. Less than 2% of the participants used hearing protection devices. Conclusions: Long-term exposure to loud sounds puts musicians at risk of hearing loss. However, due to their inadequate knowledge, most musicians never use protective devices to prevent damage to their auditory system.

Objectives: Patients with underlying conditions are at a higher risk of developing sepsis, a systematic response to infection, which has a high mortality rate. After the March 2011 Fukushima Daiichi nuclear power plant accident, there has been an influx of migrant decontamination workers; however, little is known about their health status.Case: A Japanese 55-year-old male decontamination worker, who had several underlying diseases, was transferred to our hospital in cardiopulmonary arrest. He had a history of diabetes mellitus and hypertension and a past history of tuberculosis. Control of underlying conditions was poor, with HbA1c of 13.8% at presentation. He was diagnosed with pneumonia-induced bacteremia and sepsis due to Klebsiella pneumoniae. Although spontaneous circulation returned in emergency room, he died a day after admission.Conclusion: The poor control of underlying diseases seen in this patient could have been influenced by his recent job transfer and engagement in decontamination work and additionally related to his socioeconomic status (SES). This case highlights the need for further research to elucidate the underlying diseases, working conditions, and SES of this population.

Objective: To evaluate the participatory ergonomic method on the development of upper extremity musculoskeletal disorders and disability in office employees. Methods: This study is a randomized controlled intervention study. It comprised 116 office workers using computers. Those in the intervention group were taught office ergonomics and the risk assessment method. Cox proportional hazards model and generalized estimating equations (GEEs) were used. Results: In the 10-month postintervention follow-up, the possibility of developing symptoms was 50.9%. According to multivariate analysis results, the possibility of developing symptoms on the right side of the neck and in the right wrist and hand was significantly less in the intervention group than in the control group (p<0.05). Neck disability/symptom scores over time were significantly lower in the intervention group compared with the control group (p<0.05). Conclusion: The participatory ergonomic intervention decreases the possibility of musculoskeletal complaints and disability/symptom level in office workers.

Introduction Occupational allergy to rodents among laboratory animal workers is common. Most patients generally experience allergic symptoms after the first few years of work. Associated symptoms are usually mild, such as rhinoconjunctivits, urticaria, and asthma. Anaphylaxis, although rare, could be severe and life threatening. Methods We have described in this study two cases of laboratory workers that developed skin and respiratory reactions following laboratory rat and mouse bites, consistent with anaphylaxis. Results Skin testing was found positive for rat epithelium in the patient with anaphylaxis due to rat bite. Elevated levels of specific IgE antibodies against rat and mouse epitheliums were also detected in both the patients. Conclusion These cases illustrate a severe hypersensitivity reaction that could potentially occur in occupational workers that are in close contact with rodents. Reduction of allergen exposure, regular screening, and job modification could be beneficial for affected individuals. Health care workers should be made aware that anaphylaxis could be a serious consequence of laboratory animal bites, particularly in those already sensitized.

Objectives: The aim of this study was to identify the determinants of presenteeism, taking health and individual factors into account. Methods: A quantitative analysis applying structural equation modelling analysis was conducted on the basis of secondary data from the Health and Retirement Survey (2008 wave), which measured presenteeism and its determinants. Results: Stress-related factors at work (β = -0.35, p< 0.001), individual factors (β = -0.27, p< 0.001), and health (β = 0.24, p< 0.001) were significantly related to presenteeism. Individual factors were found to be directly correlated with stress-related factors at work (β =0.22, p< 0.001). Significant indirect effects between stress-related factors at work and presenteeism (Sobel z = -6.61; p< 0.001) and between individual factors and presenteeism (Sobel z = -4.42; p< 0.001), which were mediated by health, were also found. Overall, the final model accounted for 37% (R2 = 0.37) of the variance in presenteeism. Conclusions: Our study indicates some important and practical guidelines for employers to avoid the burdens of stress-related presenteeism among their employees. These findings could help select target factors in the design and implementation of effective presenteeism interventions in the aging working population.

Objectives: The aim of this review was to summarize the lessons learned from the experience in protecting the health of workers engaged in operations following the accident at the Fukushima Daiichi Nuclear Power Plant (NPP). Methods: We reviewed all types of scientific papers examining workers’ health found in Medline and Web of Sciences as well as some official reports published by the Ministry of Health, Labour and Welfare of Japan and other governmental institutes. Results: The papers and reports were classified into those investigating workers at the Fukushima Daiichi and Daini NPPs workers engaged in decontamination operations in designated areas, and other workers. Regarding workers at the NPPs, many efforts were made to establish an emergency-care and occupational health system. Risk management efforts were undertaken for radiation exposure, heat stress, psychological stress, outbreak of infectious diseases, and fitness for work. Only a few reports dealt with decontamination workers and others; however, the health management of these workers was clearly weaker than that for workers at the NPPs. Conclusions: Many lessons can be learned from what occurred. That knowledge can be applied to ongoing decommissioning work and to future disasters. In addition, it is necessary to study the long-term health effects of radiation exposure and to accumulate data about the health of workers engaged in decontamination work and other areas.

Backgrounds: Two-component acrylic resin used in nail art and dentistry can cause allergic contact dermatitis (ACD), but only a few reports from Japan have been published. Cases: A 35-year-old female manicurist (case 1) and 30-year-old female dental hygienist (case 2) were diagnosed with ACD caused by ethyl methacrylate and methyl methacrylate in a liquid monomer of two-component acrylic resin respectively. Case 1 was sensitized from direct skin contact with nail art acrylic products, which could have been avoided, and gave up both being a manicurist and a user. Onset in case 2 occurred when she started to work as an apprentice. She had believed that the cause of her symptoms was use of disposable natural rubber gloves, not a liquid monomer penetrating through her gloves, until she was patch tested positive to (meth)acrylates and liquid monomer. Conclusions: To prevent sensitization, it should be emphasized in occupational training in both specialties that there should be no direct contact with acrylic products and that disposable gloves do not have sufficient protecting properties.(J Occup Health 2014; 56: 229-234)

Objectives: The aim of this study was to identify whether arsenic (As) exposure could induce hippocampal neural apoptosis in vivo. Methods: Sixty-four mice were randomly divided into 4 groups of 16 each. Group 1 orally received drinking water alone as a control. Groups 2-4 were given arsenic trioxide (As2O3) orally at the doses of 1 ppm, 2 ppm and 4 ppm, respectively. All the treatments continued for 60?days. Morphological changes in the hippocampus were observed by HE staining. Apoptosis in the hippocampus was examined by TUNEL assay and transmission electron microscopy. The expression levels of Bcl-2 and Bax genes and their proteins in the hippocampus were determined by real-time PCR and Western blotting. The activity of caspase-3 was determined by spectrophotometry. Results: Abnormal histopathological changes and apoptosis were found in the hippocampus of As-exposed mice. The expressions of the Bcl-2 gene and its protein in the hippocampus of As-exposed mice were significantly lower than those in the control group (p<0.05). However, the expressions of the Bax gene and its protein, and the expression ratio of Bax/Bcl-2 in the hippocampus were significantly higher in the groups exposed to As than in the control group (p<0.05). Moreover, the activity of caspase-3 in the hippocampus of mice exposed to As was higher than that in the control (p<0.05). Conclusions: These results indicate that subchronic exposure to As induces apoptosis in the hippocampus of mice by disturbing normal Bax/Bcl-2 regulatory pathways. Meanwhile, it is suggested that the induced apoptosis in the hippocampus may be at least partly responsible for As-induced neurotoxicity.

Objectives: To elucidate the factors that influence occupational physicians’ decision to issue an employer warning.Methods: The interview was conducted with 10 Japan Society for Occupational Health certified occupational physicians (COPs) and certified senior occupational physicians (CSOPs) to create nine fictive scenarios in which an occupational physician may need to consider issuing a warning. Sixteen CSOPs assessed the seriousness of the problem in each of nine scenarios where they may need to consider issuing an employer warning. Next, using a survey questionnaire, 597 COPs and CSOPs were asked to rate how likely they were to issue a warning in each of the nine scenarios, and answer items on their characteristics and number of previously issued warnings. A multilevel logistic regression analysis nested for various scenarios was used to assess the odds ratio (OR) of being likely to issue a warning.Results: Valid questionnaires were obtained from 117 participants (19.6%). The ORs and 95% confidence intervals (CIs) were as follows: mean score of seriousness of the problem, 5.90 (4.50-7.75); years of experience as occupational physician, 1.04 (1.02-1.06); women, 1.75 (1.20-2.54); being a part-time occupational physician without in-house experience, 2.08 (1.31-3.29); and having previously issued two or more times warnings, 1.99 (1.29-3.06), compared with those who had never issued a warning.Conclusions: Occupational physicians’ likelihood to issue a warning was associated with the seriousness of the problem as assessed in various scenarios, years of experience as occupational physician, gender, employment type, experience as in-house occupational physician, and number of past warnings.

Objectives: This retrospective cohort study evaluated the impact of the Stress Check Program, a recently introduced national policy and program aimed at reducing psychological distress among Japanese workers. Methods: A baseline survey was conducted from November 2015 to February 2016, the period when Japan began enforcing the Stress Check Program. A one-year follow-up survey was conducted in December 2016. In the follow-up survey, two exposure variables were collected: having taken the annual stress survey, and experiencing an improvement in the psychosocial work environment. Psychological distress was assessed using the Brief Job Stress Questionnaire (BJSQ) at baseline and 1-year follow-up. The two exposure variables were used to define four groups: "Neither", "Stress survey (SS) only", "Psychosocial work environment improvement (WI) only", and "Both". BJSQ results were analyzed using repeated measures general linear modeling (GLM). Results: The study included 2,492 participants: 1,342 in the "Neither" group, 1,009 in the "SS only" group, 76 in the "WI only" group, and 65 in the "Both" group. Overall time-group interaction effects were not significant. The "Both" group showed significantly greater improvements in psychological distress than the "Neither" group (p = 0.02) at the 1-year follow-up, although the effect size was small (d = -0.14). Conclusions: Combination of the annual stress survey and improvement in psychosocial work environment may have been effective in reducing psychological distress in workers, although the effect size was small.

Objectives: This study aimed to examine the potential effects of perceived organizational support (POS) and psychological capital (PsyCap) on combating depressive and anxious symptoms among Chinese underground coal miners. Methods: A cross-sectional survey was conducted in a coal-mining population in northeast China. The Center for Epidemiologic Studies Depression (CES-D) Scale, the Zung Self-Rating Anxiety Scale (SAS), the Survey of Perceived Organizational Support (SPOS) scale and the Psychological Capital Questionnaire (PCQ), which measure depressive and anxious symptoms, POS and PsyCap were distributed to 2,500 underground coal miners (1,925 effective respondents). Hierarchical linear regression was performed to examine the associations of POS and PsyCap (self-efficacy, hope, resilience and optimism) with depressive and anxious symptoms and the moderating roles of PsyCap and its components. The mediating roles of PsyCap and its components were examined using asymptotic and resampling strategies. Results: The mean levels of depressive and anxious symptoms were 19.91 and 49.69, respectively. POS, PsyCap, hope, resilience, optimism and POS × PsyCap were negatively associated with depressive symptoms. POS, PsyCap, resilience, POS × PsyCap and POS × resilience were negatively associated with anxious symptoms. PsyCap, hope, resilience and optimism partially mediated the association between POS and depressive symptoms. PsyCap and resilience partially mediated the association between POS and anxious symptoms. Conclusions: POS, PsyCap, hope, resilience and optimism could be effective resources for reducing depressive and anxious symptoms. PsyCap, hope, resilience and optimism act as moderators and mediators in the associations of POS with depressive and anxious symptoms. Managers should promote supportive settings and investment in PsyCap to improve workers' mental health.(J Occup Health 2015; 57: 58–68)

Objective: We reviewed studies on pulmonary, reproductive, and developmental toxicity caused by carbon nanotubes (CNTs). In paricular, we analyzed how CNT exposure affects the several processes of pulmonary toxicity, including inflammation, injury, fibrosis, and pulmonary tumors. Methods: In pulmonary toxicity, there are various processes, including inflammation, injury, fibrosis, respiratory tumor in the lungs, and biopersistence of CNTs and genotoxicity as tumor-related factors, to develop the respiratory tumor. We evaluated the evidence for the carcinogenicity of CNTs in each process. In the fields of reproductive and developmental toxicity, studies of CNTs have been conducted mainly with mice. We summarized the findings of reproductive and developmental toxicity studies of CNTs. Results: In animal studies, exposure to CNTs induced sustained inflammation, fibrosis, lung cancer following long-term inhalation, and gene damage in the lung. CNTs also showed high biopersistence in animal studies. Fetal malformations after intravenous and intraperitoneal injections and intratracheal instillation, fetal loss after intravenous injection, behavioral changes in offsprings after intraperitoneal injection, and a delay in the delivery of the first litter after intratracheal instillation were reported in mice-administered multi-walled carbon nanotubes (MWCNTs). Single-walled carbon nanotubes (SWCNTs) appeared to be embryolethal and teratogenic in mice when given by intravenous injection; moreover, the tubes induced death and growth retardation in chicken embryos. Conclusion: CNTs are considered to have carcinogenicity and can cause lung tumors. However, the carcinogenicity of CNTs may attenuate if the fiber length is shorter. The available data provide initial information on the potential reproductive and developmental toxicity of CNTs.

Objective: To assess the usefulness of the Laboratory of Physical Science (LOPS) protocol for detecting exaggerated blood pressure (BP) response as a risk factor for future hypertension when controlling for work and personal factors. Methods: Subjects were 815 healthy normotensive men (mean age, 43.1 ± 6.76 yr; range, 29-64 yr) who participated in the LOPS protocol, a graded 4-stage exercise test undertaken for the measurement of 40-70% of maximum oxygen consumption. A hypertensive response was defined as systolic blood pressure (SBP) of 250 mmHg or diastolic blood pressure (DBP) of 120 mmHg during the exercise test. Results: New-onset hypertension or the initiation of antihypertensive drug treatment had occurred in 108 men (13.3%) after 7 yr of follow-up. Cox proportional survival analysis revealed significantly increased risks of developing hypertension were associated with exaggerated BP response to exercise (hazard ratio, 2.3; 95% confidence interval, 1.4-3.7) and higher frequency of business trips (hazard ratio, 1.7; 95% confidence interval, 1.2-2.5) after multivariable adjustments for work and personal-related risk factors. Conclusions: These results suggest that the LOPS protocol is effective for detecting exaggerated BP response as a risk factor for future hypertension when controlling for work and personal-related risk factors. Exaggerated BP response to exercise and higher frequency of business trips are risk factors for developing future hypertension.

Objectives: Work ability is a crucial occupational health issue in health care settings where a high physical and psychosocial work capacity is required and a high risk of disabling injuries and illnesses is predictable. This study aims to examine the association between the work ability index (WAI) and individual characterizations, workload, fatigue, and diseases among intensive care units' (ICUs') nurses. Methods: The study sample included 214 nurses selected by a random sampling method from a target population consisting of 321 registered nurses working in eight ICUs. Multiple linear regression analysis was used to test the association between WAI scores and each of the independent variables. Results: Results of multivariate analysis revealed a strong and negative association between WAI scores and diseases (B=-5.82, 95% CI=-7.16, -4.48, P<0.001). Among the studied individual characterizations, body mass index (BMI) was significantly and inversely associated with WAI scores. A significant and negative association was also found between WAI scores and dimensions of MFI-20, such as general fatigue (B=-0.31, 95% CI=-0.53, -0.09, P=0.005) and physical fatigue (B=-0.44, 95% CI=-0.65, -0.23, P<0.001). From dimensions of workload, frustration (B=-0.04, 95% CI=-0.07, -0.02, P<0.001) and temporary demand (B=-0.04, 95% CI=-0.08, -0.0001, P=0.04) showed a negative and significant association with WAI scores, while performance showed a positive and significant association (B=0.04, 95% CI=0.01, 0.07, P=0.005). Conclusions: Based on the study findings, development of health care programs with the aim of setting up a healthy work environment characterized by a well-structured preventive attitude toward controlling diseases, and a well-designed organizational framework toward increasing the level of performance and motivation, reducing the level of fatigue, as well as reducing the workload, is necessary to promote work ability among ICUs' nurses.

Objective: To evaluate the participatory ergonomic method on the development of upper extremity musculoskeletal disorders and disability in office employees. Methods: This study is a randomized controlled intervention study. It comprised 116 office workers using computers. Those in the intervention group were taught office ergonomics and the risk assessment method. Cox proportional hazards model and generalized estimating equations (GEEs) were used. Results: In the 10-month postintervention follow-up, the possibility of developing symptoms was 50.9%. According to multivariate analysis results, the possibility of developing symptoms on the right side of the neck and in the right wrist and hand was significantly less in the intervention group than in the control group (p < 0.05). Neck disability/symptom scores over time were significantly lower in the intervention group compared with the control group (p < 0.05). Conclusion: The participatory ergonomic intervention decreases the possibility of musculoskeletal complaints and disability/symptom level in office workers.

Objectives: “Intention to leave” (ITL) has been used interchangeably with the more positive construct “intention to stay” (ITS). The implicit assumption appears to be that both constructs represent different sides of the same coin. This study challenges this assumption. The objectives were (i) to test whether these constructs were similar measures of the same construct, and (ii) to assess the strength of the relationships between ITL and ITS with work-related outcomes. Methods: The Workforce Dynamics Questionnaire (WDQ) was administered to 298 staff. The WDQ included two items on ITL and was supplemented with three items on ITS. Structural equation modelling (SEM) was used. Results: The response rate was 43%. The correlation between the two constructs was negative and quite high (r=−0.84), indicating potential issues with discriminant validity. However, the constructs behaved differently in relation to job satisfaction and job integration. ITS was a strong predictor (0.95, p<0.001), whereas ITL was not significantly related (0.34, p=0.195) to JS. The direct effects of JI on ITS was 0.30 and on ITL was −0.42. The indirect effects of JI were more contrasting, being 0.56 for ITS and −0.30 for ITL, via job satisfaction. Conclusions: This is the first study amongst British health and social care workers that has demonstrated that ITS and ITL are not measuring the same construct. While there is overlap, care should be taken when using these constructs interchangeably, particularly when measuring these concepts in organizations and when developing retention programs, policies, or activities to modify ITS and ITL.(J Occup Health 2014; 56: 292-300)

Objectives: The aim of this estudy was to investigate the influence of allergen exposure levels and other risk factors for allergic sensitization, asthma, and bronchial hyperresponsiveness (BHR) in workers exposed to laboratory animals. Methods: This was a cross-sectional study performed at two universities, 123 workplaces with 737 subjects. Dust samples were collected from laboratories and animal facilities housing rats, mice, guinea pigs, rabbits, or hamsters and analyzed by enzyme-linked immunosorbent assay (ELISA) to measure allergen concentrations. We also sampled workplaces without animals. Asthma was defined by both symptoms and BHR to mannitol. The concentrations of allergens were tested for association with a skin prick test, respiratory symptoms, spirometry data, and BHR. This multivariate analysis was performed by using Poisson regression to estimate the relative risk (RR) for the exposed group. Results: Our sample comprised students and workers, with 336 subjects in the nonexposed group and 401 subjects in the exposed group. Sixty-nine subjects (17%) had positive results in the skin prick test for animal allergens in the exposed group; in the nonexposed group, 10 subjects had positive results (3%) (p<0.001). Exposure to laboratory animals over 2.8 years was associated with atopic sensitization (RR=1.85; 95% confidence interval: 1.09–3.15; p=0.02). Allergen concentration was not associated with sensitization, asthma, or BHR. Conclusion: Exposure to laboratory animals was associated with atopic sensitization. However, we did not find a cutoff allergen concentration that increased the risk for sensitization. Duration of exposure seems to be more relevant to sensitization than concentration of allergens in dust.(J Occup Health 2016; 58: 7–15)
