Objectives: The aim of this meta-analysis is to investigate to what extent vacation has positive effects on health and well-being, how long such effects endure after work resumption, and how specific vacation activities and experiences affect these relationships. Methods: Based on a systematic literature search (PsycInfo, Medline) and methodological exclusion criteria, in a stepwise approach, 7 studies were selected and reviewed. Effect sizes (Cohen's d) were calculated i) for every outcome variable within every study, ii) for every study by averaging the effect sizes per study, and iii) for homogeneous categories of outcome variables (exhaustion, health complaints, life satisfaction). Results: The results suggest that vacation has positive effects on health and well-being (small effect, d=+0.43), but that these effects soon fade out after work resumption (small effect, d=-0.38). Our research further demonstrated that vacation activities and experiences have hardly been studied. Therefore, their contribution to vacation effect and fade out remains unclear. Discussion: Progress in future vacation research will depend on strong research designs that incorporate repeated measurements pre-, inter- and post-vacation.

Objectives: On December 1, 2015, the Japanese government launched the Stress Check Program, a new occupational health policy to screen employees for high psychosocial stress in the workplace. As only weak evidence exists for the effectiveness of the program, we sought to estimate the risk of stress-associated long-term sickness absence as defined in the program manual. Methods: Participants were 7356 male and 7362 female employees in a financial service company who completed the Brief Job Stress Questionnaire (BJSQ). We followed them for 1 year and used company records to identify employees with sickness absence of 1 month or longer. We defined high-risk employees using the BJSQ and criteria recommended by the program manual. We used the Cox proportional regression model to evaluate the prospective association between stress and long-term sickness absence. Results: During the follow-up period, we identified 34 male and 35 female employees who took long-term sickness absence. After adjustment for age, length of service, job type, position, and post-examination interview, hazard ratios (95% confidence intervals) for incident long-term sickness absence in high-stress employees were 6.59 (3.04-14.25) for men and 2.77 (1.32-5.83) for women. The corresponding population attributable risks for high stress were 23.8% (10.3-42.6) for men and 21.0% (4.6-42.1) for women. Conclusions: During the 1-year follow-up, employees identified as high stress (as defined by the Stress Check Program manual) had significantly elevated risks for long-term sickness absence.

Objectives: In the present study, we modified a night shift system for an ambulance service so that ambulance paramedics were assured of taking a nap, and examined the effects of this new system on the fatigue and physiological function of ambulance paramedics. Methods: Ten ambulance paramedics at a fire station in the center of a large city in Japan voluntarily enrolled as subjects in this field study. They worked a 24-h shift system. There were two teams of 5 ambulance paramedics in the fire station. Three ambulance paramedics per shift usually provided the emergency services. In the traditional system, the ambulance paramedics had to deal with all emergency calls throughout a 24-h shift (T-shift). In the modified system, 2 ambulance paramedics were allotted time for naps in the 21:00-3:00 (C-shift) or 3:00-8:30 (B-shift) shift by the addition of another a firefighter (D-shift). Results: There were fewer emergency dispatches and nap time was longer in the B- and C-shifts than in the T-shift. Parasympathetic nerve activities during naps in B- and C-shifts were higher than in the T-shift. The results of critical flicker fusion frequency and 3-choice reaction time in the B-shift at 7:30 tended to be higher and shorter than that in T-shift. Conclusions: The results of this study suggest that the modified night shift which ensured time for ambulance paramedics to take long, restful power naps alleviated subjective fatigue, and improved physiological function which are often adversely affected by night workload.

Objectives: Rotating shift work has been reported to increase the risk of cardiovascular diseases. Vascular endothelial dysfunction and platelet activation are among the leading causes of thrombus formation in patients with myocardial infarction or stroke. Endothelial function has been shown to be impaired immediately after night-shift work; however, it is not known whether platelets are also activated. The aim of this study was to investigate the acute impact of night-shift work on platelet function. Methods: This observational study included 11 healthy medical staff members (seven women, median age 32 years). We examined each subject's platelet aggregation rates and the serum concentrations of eicosanoid mediators after night-shift work and on day-shift work without preceding night-shift work (baseline). Results: Platelet aggregation did not differ from baseline levels after night-shift work. However, serum cyclooxygenase (COX)-metabolized eicosanoid mediators, particularly thromboxane (Tx) B2 (a stable metabolite of TxA2 and the most important marker of platelet activation), were significantly higher after the night-shift than at baseline (median 65.3 vs 180.4 ng/ml). Conclusions: Although platelet aggregation did not increase, there was an increase in serum COX-metabolized eicosanoid mediators such as TxB2 in healthy medical staff after night-shift work. This platelet hypersensitivity may be one of the mechanisms underlying the significant association between night-shift work and adverse cardiovascular outcomes.

Workstyle reform for Japanese doctors is a priority of Japanese labor policy. Factors influencing this include the low birthrate and longevity of the population and the long working hours experienced by Japanese workers. Long working hours imperil the health of doctors and create discord between their work and other life roles. The Japanese government enacted measures to promote workstyle reform for doctors, including a provision that capped legal overtime work. The limit was set to sustain healthcare services in the community. However, the allowed level of overtime is substantially higher than the so-called “line of karoshi” at which point worker compensation for work-related medical disorders is approved. Despite limited research, several measures can be considered to reduce health risks of overworked doctors, such as recommending psychological detachment from clinical work and maintaining work-life balance. Doctors should realize that self-care is a professional imperative. Organizational efforts from public administration entities or hospitals are required to secure the health of doctors. The number of surgeons and obstetricians has not recently increased. Gender role stereotyping has encouraged male Japanese doctors to work long hours and makes it difficult for female doctors to continue their professional life after childbirth. Without reforming the workstyle and protecting the health of doctors, younger workers, particularly women, are liable to avoid entering medical professions. To ensure the provision of good healthcare, it is necessary to change traditional values and improve the work environment. Academics in occupational fields should produce evidence that will help improve Japanese labor policies.

Objectives: To examine whether disaster-related variables, in addition to known work-related risk factors, influence burnout and its subscales (exhaustion, cynicism, and lack of professional efficacy) among public servants who experienced a major disaster. Methods: Cross-sectional studies were conducted among public servants of Miyagi prefecture at 2 and 16 months after the Great East Japan Earthquake (n=3,533, response rate 66.8%); burnout was assessed at 16 months using the Japanese version of the Maslach Burnout Inventory-General Survey. We examined the relationships between burnout and its subscales with disaster-related variables at 2 months after the disaster, while controlling for age, gender, and work-related variables at 16 months after the disaster. Results: After controlling for age, gender, and work-related variables, a significant risk factor of burnout was having severe house damage. For the each subscale of burnout, living someplace other than their own house increased the risk of both exhaustion and cynicism, while handling residents' complaints did so only for exhaustion. Notably, workers from health and welfare departments showed an increased risk of burnout, exhaustion, and cynicism, but not lack of professional efficacy. Conclusions: The findings suggest that special attention is needed for workers with severe house damage to prevent burnout, as well as those who lived someplace other than their own house to prevent exhaustion and cynicism after a major disaster. Interventions directed at workers of the health and welfare department should focus more on limiting exhaustion and cynicism, rather than promoting professional efficacy.

Objectives: This 2-month cohort study aimed to investigate the changing prevalence and factors associated with COVID-19-related workplace bullying among the general workers in Japan. Methods: A baseline survey was conducted of 4,120 full-time workers at Time 1 (March 2020) and they were invited to a follow-up survey at Time 2 (May 2020) after the outbreak of COVID-19 in Japan. The prevalence of COVID-19-related workplace bullying was compared between Time 1 and Time 2 by using McNemar's test. Multiple logistic regression analysis was conducted to investigate the associations between occupation (health care and non-health care workers), socioeconomic status, living in areas under the national emergency announcement, workplace measures against COVID-19, occupational class, chronic physical comorbidities, chronic mental comorbidities, and COVID-19-related workplace bullying at Time 2, adjusting for that at Time 1. Results: A total of 1,421 responded to the survey at Time 2. Data from 996 respondents after excluding 36 who retired during the follow-up were analyzed. The prevalence of COVID-19-related workplace bullying increased more than double from Time 1 (2.8%) to Time 2 (6.5%). Being a manual worker (OR=3.80), having higher education (OR=2.37), and having chronic physical comorbidity (OR=2.11) was significantly associated with the COVID-19-related workplace bullying at Time 2. Conclusions: COVID-19-related workplace bullying increased during the outbreak of COVID-19 in Japan. A lower-class occupation (manual workers) and having chronic physical comorbidity may be associated with greater victimization of COVID-19-related workplace bullying, while those with high educational attainment may be more sensitive to it.

Objectives: We investigated the effects of fatigue on NK cell function and lymphocyte subpopulations in nurses performing shift work using a longitudinal design. Methods: Fifty-seven female nurses engaged in shift work at a hospital in Japan were selected for our study cohort. The hospital used a counterclockwise rotating three-shift system. Night shifts followed day shifts after a seven-hour interval. Immune parameters measured at the beginning of the day shift through to the end of the night shift were compared between two groups stratified by their level of fatigue. Statistical differences were evaluated after adjusting for baseline immune values and other demographic features. Results: Subjective feelings of fatigue increased progressively from the beginning of day shifts to the end of night shifts. From the beginning of day shifts to the end of night shifts, NK cell activity and CD16+CD56+ lymphocytes decreased, while CD3+ and CD4+ lymphocytes increased. The group with the greater increase in fatigue showed a larger decrease in NK cell activity and a larger increase in CD4+lymphocytes when compared with the group reporting less fatigue. These findings did not change after adjusting for demographic factors and sleep hours. Conclusion: Our data suggest that shift work has deleterious effects on NK cell function and that the effects depend on the degree of fatigue. Proper management of shift work may lessen fatigue in workers and also ameliorate many health problems experienced by shift workers.

Objectives: The aim of the study was to investigate the number of workplace measures implemented to respond to the COVID-19 epidemic reported by employees and their association with company size and industry among employees in Japan. Methods: A cross-sectional online questionnaire survey was conducted of full-time employees in Japan from March 19–22, 2020. Questions were about announcements by the company of measures taken and 23 items of workplace measures. Associations of company size and industry with the announcements and the number of workplace measures were analyzed. Results: The final sample consisted of 1,379 respondents. The mean number of implemented preventive measures for COVID-19 among 23 items was 11.2 (standard deviation, 5.9). Proportions of respondents who reported receiving announcement of measures taken was high (79.9%), as were proportions of respondents reporting implementation of some of the workplace measures, while the less than half reported implementation of most of the workplace measures. The announcement of measures and the number of workplace measures that were implemented in the workplace were generally lower/smaller among respondents in smaller companies and in retail, wholesale, and transportation industries. Conclusions: While most respondents reported receiving information about measures by their companies/organizations, it may be a further challenge to implement workplace measures in smaller companies and companies in retail, wholesale, and transportation industries.

Objectives: Declining physical capacity caused by aging increases the risk of occupational falls on the same level and to lower levels. In emerging countries in Asia, the development of a program for older farmers to assess their risk of occupational falls is valuable. The current study aimed to evaluate the relationship between physical capacity and experience of occupational falls among middle-aged and older Thai farmers.Methods: We conducted a cross-sectional survey of 419 Thai farmers aged 40 years and over during March and April, 2021. For the assessment of physical capacity, we used the Self-Check Risk Assessment of Falls and Other Accidents in the Workplace tool developed in Japan, consisting of five physical test components. Multiple logistic regression and receiver operating characteristic curves were used to analyze the data.Results: The results revealed that 25.5% of participants had experienced occupational falls in the past 12 months. For each of the five physical test components, there was no significant association between physical capacity and experience of occupational falls. The area under the receiver operating characteristic curve was less than 0.60 for each of the five physical test components. A similar trend was observed when the analysis was limited to participants aged 50 years and over.Conclusions: The current study did not reveal any associations between physical capacity in each test and experience of occupational falls among middle-aged and older Thai farmers. Because the mechanisms underlying occupational falls are complex, multiple intervention approaches may be important for preventing accidents.

<b>東京圏在住サラリーマンの通勤時身体運動量:中野治美ほか.大妻女子大学家政学部公衆衛生研究室―目的:</b>東京圏在住サラリーマンの中強度以上身体活動の量を測定し,電車通勤者とクルマ通勤者とで比較する. <b>対象と方法:</b>歩数および身体活動の測定には,身体活動強度METs(=安静時の何倍かを表す単位)を1分ごとに記録する身体活動量計(オムロンHJA-350 IT)を使った.データをパソコンに移して,通勤時間帯および全日の運動量「エクササイズEx」(=METs(≥3)×時間)を計算した. <b>結果:</b>電車通勤男性群(74人)は,朝夕の通勤にそれぞれ70±30,103±43分を使い,朝+夕通勤時のExは3.4±1.7で,これは全日のEx 5.3±2.4の64%を占めた.この全日Exは,クルマ通勤男性群(78人)の全日Ex 1.8±0.8の2.9倍であった.1日の歩数は,電車通勤男性群9,305±2,651歩で,クルマ通勤男性群3,490±1,406歩の2.7倍であった. <b>考察:</b>厚生労働省「健康づくりのための運動指針2006」は,週23 Ex以上の身体運動を推奨している.東京圏在住の電車通勤サラリーマンの運動量は大きく,週日5日間では男性で平均26.5 Exとなり,電車通勤は生活習慣病予防に貢献しているように見える.<br> (産衛誌2010; 52: 133-139)<br>

Objectives: The aim of this study was to validate the Japanese version of the job crafting scale (JCS-J). JCS measures four independent job crafting dimensions, namely increasing structural job resources, decreasing hindering job demands, increasing social job resources, and increasing challenging job demands. Methods: The translated and back-translated JCS-J questionnaires were administered online to 972 employees of a Japanese manufacturing company. The data were then divided into independent explorative and confirmative samples. Exploratory and confirmatory factor analyses were performed to evaluate the factorial validity of JCS-J. The relationship with potential consequences of job crafting (e.g., job demands, job resources, and psychological well-being) was investigated to evaluate construct validity. Internal consistency was examined to evaluate the reliability of the four JCSs. Results: An exploratory factor analysis extracted a five-factor solution. Decreasing hindering job demands was further split into two separate dimensions supporting a five- rather than four-factor structure. A series of confirmatory factor analyses revealed that the modified five-factor model that allows covariance between items fits the data best. Construct validity was generally supported by the expected correlations of each job crafting dimension with each corresponding job resource (+), job demand (+), and psychological well-being (+). Cronbach's α coefficient was sufficient for each of the four dimensions of job crafting (α ranged between 0.76 and 0.90). Conclusions: This study confirmed that JCS-J is an adequate measure of job crafting that can be used in the Japanese context.

Objectives: Fitness is essential to specialist police forces, who have higher occupational demands than general police, and vital to performance and mission success. However, little research has been done profiling the metabolic fitness of these units and how they compare to other populations. The objective of this study was to profile the aerobic fitness of a specialist police unit. Methods: Body weight was measured to account for any impact on metabolic fitness, while VO2 max was estimated via number of shuttles completed on the 20 m Progressive Shuttle Run Test (PSRT) (n=47) on two dates one calendar year apart. Results: There were no significant (p=.116) differences (mean difference 0.40±1.70 kg) in body weight between the initial measures (mean=88.84±8.25 kg) and the final measure (mean=89.24±8.77 kg) 13 months later. PSRT results increased significantly (p<.005) between the initial (mean=72.62±11.76 shuttles) and final assessments (77.51±11.46 shuttles), with a mean increase of 4.89 (± 2.94) shuttles and a small effect size (d=0.42). The mean VO2 max of the specialist police unit was 51.06±3.61 ml/min/kg following the first assessment, and 52.56±3.46 ml/min/kg following the second assessment. This was a significant finding (p<.001), with a mean difference of 1.19±1.27 ml/min/kg and a small effect size (d=0.23). Conclusions: Elite police forces have a higher metabolic fitness than the general population and general duties police officers. Having and maintaining this fitness level is imperative for their operational success and preventing injuries. This research suggests that despite the challenges posed by operational requirements, high fitness standards can not only be maintained, but also improved.

Objectives: To evaluate the effects of a 2-month lifestyle intervention for dry eye disease in office workers.Methods: Prospective interventional study (randomized controlled study). Forty-one middle-aged Japanese office workers (men, 22; women, 19; 39.2 ± 8.0 years) with definite and probable dry eye disease were enrolled and randomized to an intervention group (n = 22) and a control group (n = 19). The intervention aimed at modifying diet, increasing physical activity, and encouraging positive thinking. The primary outcome was change in dry eye disease diagnoses. Secondary outcome was change in disease parameters, including dry eye symptoms, as assessed using the Dry Eye-Related Quality of Life Score, corneal and conjunctival staining scores, tear break-up time, and Schirmer test results.Results: A total of 36 participants (intervention group, 17; control group, 19) completed the study. The number of definite dry eye disease diagnoses decreased from four to none (p =.05), and the dry eye symptom score showed a significant decrease in the intervention group (p =.03). In contrast, the corneal and conjunctival staining scores, tear break-up time, and Schirmer test results did not differ significantly between groups.Conclusions: The 2-month lifestyle intervention employed in this study improved dry eye disease status among office workers, with a considerable decrease in subjective symptoms. Lifestyle intervention may be a promising management option for dry eye disease, although further investigation of long-term effects are required.

<b>目的:</b> 睡眠は単に睡眠時間のみで良好か不良かを判断できるものではなく,睡眠の導入や維持といった睡眠の質,就寝時刻や起床時刻といった規則性まで考慮しなければならない.しかしながら,それらすべてを一度に評価できる指標は現在のところ存在しない.本研究は睡眠の規則性・質・量の3要素を評価するための質問票を独自に開発し,その信頼性と妥当性の検証を行うことを目的とした. <b>対象と方法:</b> 対象は製造業およびサービス業に従事する日勤労働者563名(男性370名,女性193名)で,平均年齢は40.4歳であった.先行研究および専門家との討議を参考に,規則性・質・量それぞれ7項目,計21項目からなる質問紙を作成・編集した.まず項目分析を行い,その後因子分析にかけて構成概念妥当性を検証した.信頼性はクロンバックα信頼性係数を算出して求めた.また,主成分分析およびクラスター分析にて標準化・分類を行い,生活習慣や日中の眠気,ストレス,持病の有無などを比較することにより,判別的妥当性の検証を行った. <b>結果:</b> 項目分析および因子分析にて,21項目中6項目が除外対象となったが,予測通り規則性・質・量の3因子構造が得られた.α信頼性係数はそれぞれ0.744,0.757,0.548であった.量因子として作成した2項目が規則性因子として抽出されていたが,それ以外は予測通りの因子として抽出された.入眠困難,熟眠障害,中途覚醒,早朝覚醒はすべて質因子として一定の負荷量を示していた.判別的妥当性については,最も点数の高いグループで健康意識が高くストレスや日中の眠気を感じていない者の割合が有意に高かった.一方で,最も点数の低いグループではストレスや持病などの睡眠障害リスクファクターを有している者の割合が有意に高かった. <b>考察:</b> 今回我々が開発した質問票にて,睡眠の規則性・質・量における構成概念妥当性が示された.しかしながら,分析過程にて不適切と判断され除外された項目や,予測していた因子とは異なる因子として抽出された項目が存在し,信頼性および内容的妥当性については課題が残った.今後これらの質問項目について再度編集・改訂し,より信頼性・妥当性を高めていく必要がある.また,年齢や性等による影響を除いたより詳細な判別的妥当性の検討も要する.

Objectives: In this study, we applied novel nasal filters to assess animal allergen exposure of veterinary staff during their normal daily routine. Methods: Rhinix nasal filters were worn during work by 94 employees at different veterinary practices and 18 employees at a research institute, who acted as controls representing an animal-free environment. Contact with animals and the activities performed were documented by the study participants using a short questionnaire. Major allergens of cats (Fel d 1), dogs (Can f 1), and domestic mites (DM) were measured using fluorescence enzyme immunoassays. Results: At the practices, Can f 1 was detected in 98%, Fel d 1 in 82%, and DM allergens in 39% of the samples. Allergens were also detected in some control samples (6% for Can f 1, 39% for Fel d 1, and 17% for DM) but in very low concentrations. There was a highly significant difference between allergen levels in veterinary workers who treated at least one cat or dog during the sampling period and those who did not (2.66 vs. 0.70 ng/filter for Can f 1 and 1.01 vs. 0.20 ng/filter for Fel d 1). The amount of sampled Fel d 1 increased significantly with increasing duration of contact with cats. This effect was not observed for Can f 1. Conclusions: The majority of veterinary workers are exposed to dog and cat allergens, even without direct contact with these animals. Rhinix nasal filters may be considered a simple and easily applicable method for monitoring personal allergen exposure.

Objective: While the workplace wellness program (WWP) has been widely provided around the world, there was little discussion of ethical issues around WWP. This article describes the overview of how ethical issues arise in WWP, and how practitioners, as well as employers, can mitigate these problems. Methods: The author reviewed the literature addressing the ethical issues around WWP and ethical analytic tools, potentially useful for practitioners to improve WWP from ethical perspectives. Results: The ethical issues in WWP were categorized based on the three principles in bioethics: the principle of autonomy, the principle of beneficence/non-maleficence, and the principle of justice. For each category, various ethical concerns have been discussed in previous research, such as autonomous decision-making under financial incentives, the importance of evidence-based intervention, unexpected consequences of health promotion programs, and disparities among workers under WWP. The author also identified several useful tools and approaches, which practitioners can utilize for improving WWP from ethical perspectives. Conclusion: The employers and practitioners are recommended to conduct ethical analysis with multiple stakeholders to improve their WWPs from ethical perspective during the process of program development and implementation.

Objectives: The aim of this study was to identify whether arsenic (As) exposure could induce hippocampal neural apoptosis in vivo. Methods: Sixty-four mice were randomly divided into 4 groups of 16 each. Group 1 orally received drinking water alone as a control. Groups 2-4 were given arsenic trioxide (As2O3) orally at the doses of 1 ppm, 2 ppm and 4 ppm, respectively. All the treatments continued for 60?days. Morphological changes in the hippocampus were observed by HE staining. Apoptosis in the hippocampus was examined by TUNEL assay and transmission electron microscopy. The expression levels of Bcl-2 and Bax genes and their proteins in the hippocampus were determined by real-time PCR and Western blotting. The activity of caspase-3 was determined by spectrophotometry. Results: Abnormal histopathological changes and apoptosis were found in the hippocampus of As-exposed mice. The expressions of the Bcl-2 gene and its protein in the hippocampus of As-exposed mice were significantly lower than those in the control group (p<0.05). However, the expressions of the Bax gene and its protein, and the expression ratio of Bax/Bcl-2 in the hippocampus were significantly higher in the groups exposed to As than in the control group (p<0.05). Moreover, the activity of caspase-3 in the hippocampus of mice exposed to As was higher than that in the control (p<0.05). Conclusions: These results indicate that subchronic exposure to As induces apoptosis in the hippocampus of mice by disturbing normal Bax/Bcl-2 regulatory pathways. Meanwhile, it is suggested that the induced apoptosis in the hippocampus may be at least partly responsible for As-induced neurotoxicity.
