BABYSCAN, a whole-body counter (WBC) for small children was developed in 2013, and units have been installed at three hospitals in Fukushima Prefecture. Between December, 2013 and March, 2015, 2707 children between the ages of 0 and 11 have been scanned, and none had detectable levels of radioactive cesium. The minimum detectable activities (MDAs) for 137Cs were ≤3.5 Bq kg−1 for ages 0–1, decreasing to ≤2 Bq kg−1 for ages 10–11. Including the 134Cs contribution, these translate to a maximum committed effective dose of ∼16 µSv y−1 even for newborn babies, and therefore the internal exposure risks can be considered negligibly small.Analysis of the questionnaire filled out by the parents of the scanned children regarding their families’ food and water consumption revealed that the majority of children residing in the town of Miharu regularly consume local or home-grown rice and vegetables, while in Minamisoma, a majority avoid tap water and produce from Fukushima. The data show, however, no correlation between consumption of locally produced food and water and the children’s body burdens.

Background: Before the COVID-19 vaccine became available, many Japanese people were undecided about whether or not to receive them. Their decisions were keys to achieving herd immunity. The impact of the type of information source on the COVID-19 vaccine uptake decision-making process remains unclear. We aimed to investigate the association between information source usage on COVID-19 and subsequent vaccine uptake status among those who have yet to decide whether to receive vaccines from non-prioritized people for vaccination.Methods: Prospective cohort online self-administered surveys were conducted in February 2021 (T1), before the start of the mass vaccination program, and September–October 2021 (T2), when the vaccines were available to all citizens. The survey’s target population was registered monitors of an Internet research company. Participants who answered “I want to get vaccinated after waiting to see how it goes.” at T1 were eligible for analysis. The outcome variable was the COVID-19 vaccine uptake status in T2, and the predictors were 20 types of information sources, categorized based on people (family members, etc.), institutions (governments, etc.), or media (TV news, etc.). Adjusted odds ratio and 95% confidence intervals were estimated using logistic regression adjusted for possible confounders.Results: The 5,139 respondents, mean age and standard deviation was 42.8 ± 12.5, 55.7% female, were eligible for analysis. 85.7% completed vaccination (including reserved/intended people) in T2. In the multivariate logistic analysis, odds ratios (95% confidence interval) for vaccine uptake were 1.49 (1.18–1.89) for workplaces/schools, 1.81 (1.33–2.47) for LINE, 0.69 (0.55–0.86) for Internet news and 0.62 (0.48–0.82) for video sharing sites.Conclusions: The type of information source usage played an important role in the decision to vaccinate against COVID-19. Although caution is needed in interpreting the results, obtaining information from workplaces/schools and LINE was influential in promoting immunization.

Responding to the radiation-related concerns of parents/guardians with infants/small children is an important public health issue for regional recovery after radioactive contamination. This study summarizes the results of a systematic internal contamination screening of infants/small children, aged 0–6 years, using BABYSCAN and individual counselling sessions with physicians about radiation concerns from 2014 to 2018 in Minamisoma City. Of 3,114 participants, no one was found to have internal contamination with radioactive caesium with a detection limit of 50 Bq/body. The questionnaire survey showed a decreasing trend of concerns about food contamination and playing outside as possible causes of internal contamination over time. Because people’s concerns were diverse in counselling sessions, individual responses are required. This study showed that examinations using BABYSCAN provide an opportunity for direct dialogue between the parents/guardians of infants/small children and experts. This can be considered a model case for risk communication conducted by the local government after a radioactive contamination incident.

Objectives: Patients with underlying conditions are at a higher risk of developing sepsis, a systematic response to infection, which has a high mortality rate. After the March 2011 Fukushima Daiichi nuclear power plant accident, there has been an influx of migrant decontamination workers; however, little is known about their health status.Case: A Japanese 55-year-old male decontamination worker, who had several underlying diseases, was transferred to our hospital in cardiopulmonary arrest. He had a history of diabetes mellitus and hypertension and a past history of tuberculosis. Control of underlying conditions was poor, with HbA1c of 13.8% at presentation. He was diagnosed with pneumonia-induced bacteremia and sepsis due to Klebsiella pneumoniae. Although spontaneous circulation returned in emergency room, he died a day after admission.Conclusion: The poor control of underlying diseases seen in this patient could have been influenced by his recent job transfer and engagement in decontamination work and additionally related to his socioeconomic status (SES). This case highlights the need for further research to elucidate the underlying diseases, working conditions, and SES of this population.

The “Hiroshima Vision”, unveiled at the G7 Hiroshima Summit in 2023, leaves room for enhancement in the specific content of equitable global vaccine distribution plans. Despite the efforts of COVID-19 Vaccines Global Access (COVAX), vaccine supply faces severe disparities, with high-income countries receiving a disproportionately larger share. To mitigate future public health crises, mechanisms proven by past instances, such as establishing regional vaccine hubs, promoting technology transfers, and considering early patent rights relinquishment, need to be implemented. Correcting vaccine inequity necessitates learning from the COVID-19 pandemic and demands global cooperation and consensus from the G7.

BackgroundLittle information is available on the role of community-based rehabilitation after a nuclear disaster. Here, we report the case of an older couple living in an area repopulated after the Fukushima nuclear disaster of 2011 who received outpatient rehabilitation.Case presentationAn 84-year-old woman underwent total hip arthroplasty (THA) after she fell and sustained a trochanteric fracture while caring for her husband with Alzheimer's disease. The 85-year-old husband experienced worsening behavioral and psychological symptoms of dementia (BPSD) following his wife's hospitalization. The couple received rehabilitation at an outpatient facility in a nearby village using a shuttle service. The woman’s postoperative anxiety was relieved and her physical function improved. Moreover, the husband's BPSD symptoms decreased.ConclusionA wife and husband showed improvement in physical function after THA and alleviation of BPSD, respectively, following rehabilitation. In post-disaster, resource-scarce areas, older adults may benefit from utilizing the outpatient rehabilitation services available in the surrounding area.

Media coverage of disasters potentially damages mental health. Moreover, its effects may differ as recipients may have different emotional responses toward media. The present study examined whether social capital, known to be protective against mental problems, influences a recipient’s emotional response toward news media broadcasting of natural disasters via newspapers, television and internet in Japan. Three social capital components, social participation, social support and cognitive social capital, were considered in the present study as each component reportedly had different effect on mental health. This nationwide cross-sectional survey was undertaken in 2015 among 1,200 Japanese citizens aged 15 to 79 years who were selected using the multi-stage sampling procedure. Data were collected via the drop-off pick-up method using a printed structured questionnaire. Negative and positive emotions were classified based on recipients’ responses against news media. Among 1,190 participants who reported emotions toward news media, 30.9% (368) had experienced any natural disasters, 37.4% (445) belonged to at least one formal or informal organization (social participation), 40.2% (478) had high social support, and 68.8% (819) had high cognitive social capital. High social support was associated with both reduced negative emotional response (OR 0.66, 95% Confidence Interval (CI) 0.47-0.93) and increased positive emotional response (OR 1.48, 95% CI 1.04-2.12) in multivariate analyses, while high cognitive social capital was only associated with increased positive emotional response (OR 1.62, 95% CI 1.11-2.37). These results suggest protective effects of social support and cognitive social capital against news media coverage of natural disasters.

The purpose of the present study was to assess the effects of social capital on mental health among the Japanese population with or without natural disaster experience. A nationwide cross-sectional study was performed in the population aged 15 to 79 years old. We collected data on psychological status, social capital, disaster experience in ten years prior to the survey, and socio-demographic information. We assessed cognitive social capital (perceptions of support, reciprocity and trust), social support (support from individuals in the community), and social participation (participation in social activities) as components of social capital. The study outcome was mild mood or anxiety disorder (hereafter mood/anxiety disorder), defined as the score of 5 or higher in the Kessler Psychological Distress Scale (K6). Using logistic regression models, we tested whether each component of social capital was associated with mood/anxiety disorder with or without disaster experience. Out of 1,200 participants, 1,183 had available K6 score data and were considered. Among three components of social capital, only social support significantly interacted with disaster experience (p = 0.019). In the population without disaster experience, those with high social support were less likely to have mood/anxiety disorder (OR 0.45, 95% Cl 0.28-0.73); however, no such association was observed among those with disaster experience (OR 1.11, 95% CI 0.64-1.90). Thus, the protective effects of social support against mood/anxiety disorder vary in the Japanese population depending on disaster experience. The present study provides important insight into the role of social capital on mental health after natural disaster.
