症例は26歳, 男性. 夕食にコンニャクを食べ, 翌日の正午から腹痛が出現. 翌々日の早朝から嘔気, 嘔吐を繰り返したため当科を受診した. 腹部所見では腸雑音は減弱しており, 腹部単純レントゲン検査で小腸ガス像を認め, 腹部CT所見でも腸液が充満, 拡張した小腸を認めたがイレウスの原因は確定できなかった. 内ヘルニアによる絞扼性イレウスも否定できないため, 同日開腹術を施行した. 回腸末端より70cm口側に Meckel 憩室を認め, 約4cm大のコンニャクが憩室内に1個とその口側の回腸内に長径2.5cmから4cm大のコンニャク片が10個充満していた. Meckel 憩室を楔状切除し, この切開部より全てのコンニャク片を除去した. イレウス状態の患者の診察にあたっては, 手術歴を確認する以外に食餌性イレウスの可能性も考慮し, 食事内容やそしゃく状況に関する丁寧な問診が重要であると考えられた.

The breast is one of the most common site of cancer originated primarily. Metastatic neoplasma to the breast, however, has been reported as rare. According to the Annul of the Pathological Autopsy Cases in Japan, seventy six cases who had metastatic neoplasma to the breast out of 219,41 cases were reported in 1981,which included 35 cases of the opposite side breast cancer, 9 of gastric cancer, 9 of malignant lymphoma, 5 of cervical or corpus cancer of the uterus etc. The incidence of the metastatic cancer to the breast is 0.35% in all the malignant neoplasma. In this paper, the authors reported 3 cases with the metastatic neoplasma to the breast experienced during a period of these 10 years. The first case was intrahepatic bile ductal cancer, the second was pancreatic cancer, and the third was descending colon cancer. The intrahepatic bile ductal cancer was the first case of metastatic cancer to the breast in Japan in the literature.
