母乳栄養児における難治性湿疹で,食物アレルギーの疑いで紹介されるケースの中に亜鉛欠乏症が存在する.今回我々は亜鉛欠乏症の3兄弟例を経験した.第1子は生後4カ月時に低亜鉛母乳による亜鉛欠乏症と診断治療し,第2子は早期に対応できた.しかし,第3子は家族歴の申告についての指導不足により食物アレルギーの疑いで紹介となった.亜鉛欠乏による皮疹の存在を周知する必要性と兄弟例に対する指導の大切さを痛感させられた.

In the past5 to 10years, several American and European global corporations have obtained dynamic global structure management capabilities. They look like well managing the current big world tide, Borderless, Deregulation and Information Technology Revolution. On the contrary, Japanese corporations are still keepig the old Japanese style to manage overseas subsidiaries. Central contorol by Japanese managers, and heads of overseas subsidiaries are Japanese and their mission is work for the boss in Japan, and not for the merit to the overseas subsidiaries. In this report, a concrete proposal is shown to make a revolutional first step to become a competitive transnational company.

急激な身長の増加と陰茎肥大を主訴に受診し, Leydig細胞腫と診断された5歳男児例を報告する. 初診時, 身長 128.0 cm (+4.95 SD), 体重 26.7 kg (+3.60 SD), 骨年齢9歳7ヵ月, 精巣容量右6 ml, 左4 ml, 陰茎長 9 cm, 血中testosterone 3.40 ng/mL, LH<0.10 IU/mL, FSH<0.20 IU/mLであり, ゴナドトロピン非依存性思春期早発症と診断した. エコーおよびMRIにて右精巣に腫瘤を認めた. 右高位精巣摘出術を施行し, 組織学的にLeydig 細胞腫と診断した. 摘出術後, 成長率は正常化し, testosterone値は測定感度以下に低下した.
