A retrospective study of 16 cases with visual disturbances resulting from head injury, seen in our clinic between the years 1969-1972, is described. A proposed endnasal-trans-ethomoidal operative technique was performed in all cases, and the following results were obtained.1) The patients consisted of 14 males and 2 females.2) The most prominent cause of visual disorders of the present series was the accident of motor cycle crashes.3) Once blindness occurred completely, their visual powers have never recovered, even if the operation procedures were performed safely in these 6 cases except one case.4) In 10 cases of having imcomplete loss of visual power, some remarkable improvements were gained after the operation.5) Having compared the time of operation with their results, they have been no difference in the series. 6) An otologist should be consulted early in all cases of visual loss resulted from head injury.

片眼の詐病が疑われる症例を経験した。初回検査時、受傷眼の矯正視力は0.05であった。しかし、詐病を疑い視力検査法を工夫したことにより、再検時には0.5まで確認することができた。この方法は、受傷眼にピンホールを装用し、非受傷眼に凸レンズによる不完全遮閉を行うもので、両眼開放視力検査の応用である。この視力検査を行うにあたっては、不完全遮閉の目的で装用する凸レンズの度数が非常に重要である。そのため今回は、どのような度数で遮閉するのが適当かを検討する目的で、正常者7名(14眼)を対象に、各眼の完全矯正値に-1.0Dから+5.0Dを加えた状態で、負荷度数による視力の変化を測定した。視力測定はノートパソコンを利用したランドルト環単独視標を用い、恒常法で行った。得られた視力値は各負荷度数とも個人差が見られ、+1.0D付近で最も個人差が大きかった。負荷度数の増加に伴い、視力の低下は緩やかになった。+3.0Dの負荷では、0.15以上の視力を得られたものはいなかった。十分かつ安定した遮閉効果がある凸レンズをもって健眼の遮閉を行うには、+3.0D以上を用いるのが適当である事が示唆された。
