症例は76歳の男性. IgA腎症による末期腎不全で64歳時に透析導入した. 維持血液透析中に腰痛と発熱が出現し, 炎症反応高値であることから抗生物質投与のうえ入院となった. 熱源精査目的の腰椎MRIで大動脈腎動脈分岐部から総腸骨動脈分岐レベルでT2延長域を認め, 造影CTでは大動脈周囲に境界不明瞭な組織濃度上昇が認められた. 画像上血管自体の変化に乏しいため大動脈周囲炎を第一に考えた. 一方, 感染性動脈炎も否定できないためステロイド投与は直ちに行わず抗生物質投与を続行したところ, 血液検査や画像上炎症所見の改善が得られたが, 左腎動脈分岐部に動脈瘤が出現し急速に増大した. 破裂の危険に対しステント留置術を施行した. 経過から感染性大動脈炎に伴う腹部大動脈瘤すなわち感染性動脈瘤であると診断した. 今回, 治療法の異なる大動脈周囲炎と感染性大動脈炎の鑑別に難渋し, 感染性大動脈炎から瘤への形成過程を観察し得た貴重な症例を経験したので報告する.

Background: Remote monitoring of Cardiac implantable electronic devices (CIEDs) improves patient safety. However, operating such remote monitoring systems (RMS) has increased workload of medical staffs. Biotronik EHR DataSync® software supports medical staffs by integrating CIEDs information and data using international standards. The software has a potential to enhance clinical usefulness, reduce errors and lighten the workload. The aim of this study was to develop the automatic electronic medical record (EMR) system for CIEDs and to assess its effectiveness.Methods: We self-built the EMR system that automatically stores varied numericals and data from the RMS and its programmer. We examined accuracy of the automatic EMR system and reduction of the workload.Results: A total of 3,011 data was successfully collected and integrated, and the success rates of data collection were 99.8%. Moreover, the automatic EMR reduced 3 hours and 55 minutes per year of the workload for dealing CIEDs.Conclusion: The self-built automatic EMR system could integrate CIEDs data successfully. This could help to reduce the workload of medical staffs..
