[Purpose] This study aimed to investigate the effect of phase proprioceptive training on balance in patients with chronic stroke. [Subjects and Methods] Participants included 30 patients with stroke who were randomly assigned to the proprioceptive training group (n=15) or control group (n=15). Participants in the proprioceptive training group underwent proprioceptive training and received general physical therapy each for a total of 20 thirty-minute sessions, five times per week, during a period of four weeks; the control group received general physical therapy for a total of 20 sixty-minute sessions, five times per week, during a period of four weeks. [Results] All participants were evaluated with the Berg Balance Scale, Timed Up and Go (TUG) test, and Activities-specific Balance Confidence (ABC) Scale instrument before and after intervention. After training, the differences in BBS, TUG, and ABC scores in the proprioceptive group were significantly greater than those in the control group. [Conclusion] In conclusion, proprioceptive training was effective on balance ability. Therefore, proprioceptive training may be efficient when combining general physical therapy with phase proprioceptive training for patients with impairments of balance. Further research is needed to investigate proprioceptive training methods.

[Purpose] The purpose of this study was to analyze the architectural properties of muscles on ultrasonographic images of chronic stroke patients taken during different muscle activities. [Methods] Thirty chronic stroke patients were equally divided into three groups according to their Modified Ashworth Scale grade (1 to 3). Ultrasonographic equipment was used to measure structural properties of their skeletal muscles (muscle thickness, muscle pennation angle, and length of muscle fascicles). [Results] Muscle thickness, pennation angle, and fascicle length significantly decreased both at rest and during MVIC (Maximum Voluntary Isometric Contraction) as muscle spasticity increased. Each group’s muscle pennation angle markedly increased during MVIC compared to at rest. Each group’s muscle thickness and fascicle length decreased during MVIC compared to at rest. [Conclusion] Changes in structural properties of the skeletal muscles influenced architectural properties of the muscles on ultrasonographic images. Our results indicate that understanding the structural properties of the skeletal muscles of chronic stroke patients needs to take into consideration the ultrasonographic architectural properties of the muscles.

[Purpose] The purpose of this study was to investigate the effects of a virtual reality (VR) program on cognitive function and balance in the elderly with mild cognitive impairment (MCI) attending G welfare center in Gurye. [Subjects and Methods] Twenty-four patients with MCI were studied. The patients were exposed to the VR program for 30 min per experiment, which was conducted 20 times for four weeks. [Results] The cognitive function and balancing ability of the experimental group, when compared to the control group, showed a statistically significant increase in Visual Span Test (VST), Word or Test (WCT), and Limit of Stability (LOS), which are the sub-categories of CNT 4.0, after the exposure to the program. In all test categories, the experimental group exhibited statistically significant differences compared to the control group. [Conclusion] Thus, the VR program is an effective intervention for the elderly with MCI.

[Purpose] This study aimed to analyze the effects of complex training on carbon monoxide, cardiorespiratory function, and body mass among lege students with the highest smoking rate among all age group. [Subjects and Methods] A total of 40 lege students voluntarily participated in this study. All subjects smoked and were randomly divided into two groups: the experimental group (N=20) and the control group (N=20). The experimental group underwent complex training (30 min of training five times a week for 12 weeks) while the control group did not participate in such training. The complex training consisted of two parts: aerobic exercise (walking and running) and resistance exercise (weight training). [Results] Two-way ANOVA with repeated measures revealed significant interactions among CO, VO2max, HRmax, VEmax, body fat, and skeletal muscle mass, indicating that the changes were significantly different among groups. [Conclusion] A 12 week of complex physical exercise program would be an effective way to support a stop-smoking campaign as it quickly eliminates CO from the body and improves cardiorespiratory function and body condition.

[Purpose] This research aimed to examine the effects of back massage and foot bath with blended essential oil on psychophysiological response in stroke patients. [Subjects and Methods] The subjects were 14 adult stroke patients randomly divided into the experimental group (7 patients) and the control group (7 patients). Physical and psychological stress, mood state and sleep satisfaction was measured using evaluation instruments and body temperature was measured with infrared thermography (T-1000). [Results] Measurements included physical and psychological stress, and mood state of the experiment group became significantly lower than that of the control group. The body temperature and sleeping satisfaction of the experimental group became significantly higher than that of the control group. [Conclusion] The present study suggested that aroma therapy and foot bath that can be used as alternative physical therapy that offers an overall beneficial effect on psychophysiological response such as reduced stress, mood state and increased body temperature, sleeping satisfaction of stroke patients.

[Purpose] The purpose of this study was to examine the effects of galvanic vestibular stimulation on event-related potentials. [Subjects and Methods] Forty normal female adult subjects were randomly distributed to a galvanic vestibular stimulation application group (20 subjects) and sham group (20 subjects). For galvanic vestibular stimulation application, a positive electrode was applied to the right mastoid process, and a negative electrode was applied to the left mastoid process; simulation was applied for 10 minutes. A test was conducted on the N100 and P300 components of the event-related potentials before and after galvanic vestibular stimulation. [Results] The N100 latency showed statistically significant differences in interaction effects between time and group in the F3, F4, Fz, and Pz areas. The P300 latency showed the same results in the Fp1 and Fp2 areas, the N100 amplitude showed the same results in the Fp2, Fz, and Pz areas; and the P300 amplitude showed the same results in the Pz area. [Conclusion] These results suggest that galvanic vestibular stimulation may play a positive role in the N100 and P300 components of the event-related potentials of the cerebral cortex related to decision-making in matching words with images.

[Purpose] The objective of this study was to investigate the effects of spinal support device (SSD) on pain and hamstring extensibility in patients with non-specific low back pain (NSLBP). [Subjects and Methods] 20 patients with NSLBP were recruited and randomly assigned to either the SSD group or the control group. In the SSD group, SSD was applied; in the control group, bed rest in supine position was performed. Both groups underwent treatment 20 min/day, 3 times a week, for a duration of 4 weeks. To assess the hamstring extensibility, sit and reach test (SRT) was performed. To assess pain pressure threshold (PPT) of the sacroiliac joint, a pressure algometer was used. Visual analog scale (VAS) was used to quantify pain. [Results] The SSD group showed a significant improvement in sacroiliac joint pain with increased VAS, and the control group showed a significantly increased VAS after intervention. In the SSD group, VAS was significantly increased, but SRT was not changed compared with the control group. [Conclusion] These results demonstrated that an application of SSD effectively attenuates low back pain. Therefore, SSD may be a suitable intervention for pain control in patients with NSLBP.

[Purpose] To clarify factors contributing to habituation of pelvic floor muscle training (PFMT) for urinary incontinence. [Subjects and Methods] We included 13 healthy females and examined diurnal and nocturnal urination frequency at initial program participation and at 3 months. The survey used the International Consultation on Incontinence Questionnaire-Short Form (ICIQ-SF), a 10-level self-assessment of anxiety associated with urinary incontinence, and a 10-level self-evaluation of PFMT understanding and skill acquisition. We evaluated PFMT practice at home and postures that facilitated PFMT. The practice of PFMT at home was surveyed during a 3-month period. [Results] Compared to baseline, the level of skill acquisition assessed by the ICIQ-SF and PFMT according to the 10-level self-evaluation improved significantly at 3 months. The rate of PFMT sessions performed at home per week was high. The number of times PFMT was performed per day was positively correlated with level of understanding and acquisition of skills pertaining to PFMT, according to the 10-level self-assessment. [Conclusion] By incorporating behavior modification techniques appropriate for urinary incontinence and by increasing the level of understanding regarding incontinence and PFMT, as well as the level of skill acquisition, self-efficacy increased. This may have motivated habituation of PFMT.

[Purpose] The purpose of this study was to compare the effects of muscle strength training and muscle endurance training on muscle deoxygenation level and endurance performance. [Subjects and Methods] Nineteen healthy young men were randomly assigned to a muscle strength training (STR: n = 6) group, muscle endurance training (END: n = 6) group, or a control (CON: n = 7) group. The training intensity for STR was 60°/sec × 10 repetitions × 5 sets/day and that for END was 240°/sec × 50% fatigue repetitions × 2 sets/day, 3 days/week, for 6 weeks. All subjects performed cardiopulmonary exercise testing (CPX) to measure maximum oxygen uptake, exercise time and muscle deoxygenation level of vastus lateralis, and underwent muscle strength and muscle endurance measurements pre- and post-training. [Results] In the STR group, muscle strength tended to increase, while muscle endurance significantly increased in the END group. Muscle deoxygenation level was significantly increased in both training groups. Maximum oxygen uptake did not change; however, in the END group alone, exercise time was significantly prolonged. [Conclusion] These results suggest that muscle endurance training is more effective at increasing endurance performance than muscle strength training.

[Purpose] To present legislation comparing direct and referred access—or other measures—to physical therapy. The focus is on the management of the most burdensome musculoskeletal disorders in terms of regulations, costs, effectiveness, safety and cost-effectiveness. [Methods] Main biomedical databases and gray literature were searched ranging from a global scenario to the analysis of targeted geographical areas and specifically Italy and the Region Piedmont. [Results] legislation on Direct Access highlights inconsistencies among the countries belonging to World Confederation for Physical Therapy. Direct Access could be an effective, safe and efficient organization model for the management of patients with musculoskeletal diseases and seems to be more effective safer and cost effective. [Conclusion] Direct Access is a virtuous model which can help improve the global quality of physical therapy services. Further studies are required to confirm this approach and determine whether the findings of the present overview can be replicated in different countries and healthcare systems.

[Purpose] The purpose of this review was to investigate the types of exercises that can improve the tropism of the multifidus muscles, based on clinical evidence. [Methods] Following to the PICO (Problem, Intervention, Comparison, Outcome) model, we considered studies of subjects with specific or non-specific LBP that used exercises aimed at activating the lumbar multifidus muscle and measured its cross-sectional area or thickness with ultrasound, computed tomography or magnetic resonance imaging. [Results] This review found that most studies compared different types of exercises for lumbar muscles, but without specifically investigating the multifidus muscle. However, a few studies showed that the cross-sectional area and thickness of the multifidus muscle can be increased by activating this muscle, and they progressed from motor control to increased static and dynamic loads. [Conclusion] A review of the literature revealed that specific supervised and home exercises may improve the symmetry of the multifidus muscle.

[Purpose] This study systematically reviewed the antalgic effects of non-invasive physical modalities (NIPMs) on central post-stroke pain (CPSP). [Subjects and Methods] Clinical studies were sought on September 2015 in 10 electronic databases, including Medline and Scopus. The searching strings were “central pain and stroke” and “treatment, and physical or non-pharmacological”. The inclusion and exclusion criteria were set for screening the clinical articles by two reviewers. Pain scores on visual analog scale in an article were used as the outcome measure for resulting judgment. The NIPMs intervention summarized from the eligible articles was rated from Levels A to C according to Evidence Classification Scheme for Therapeutic Interventions. [Results] Over 1200 articles were identified in the initial searches and 85 studies were retrieved. Sixteen studies were eligible and judged. Caloric vestibular stimulation (n=3), heterotopic noxious conditioning stimulation (n=1), and transcutaneous electrical stimulation (n=1) were rated below Level C. Transcranial direct current stimulation (TDCS; n=2) and transcranial magnetic stimulation (TMS; n=9) were rated as Level B. [Conclusion] The findings suggest that TMS and TDCS were better than other treatments for CPSP relief but the studies were of insufficient quality.

[Purpose] The purpose of this study was to examine the degrees of changes in the gait and feet when walking on a treadmill using Nordic poles and without Nordic poles for 30 minutes. [Subjects] The participants of this study were 30 young adult males. The subjects were divided into a Nordic pole walking group of 15 subjects and a normal walking group of 15 subjects. [Methods] The subjects in both groups were encouraged to face forward and walk in comfortable postures on the treadmill. They did not wear any shoes during walking, and the speed of the treadmill was set to the average of the speeds at which the subjects could comfortably walk in both groups. We measured the gait and feet. [Results] In the Nordic pole group, step length was significantly increased. The rate of each segment from heel contact to foot flat and foot flat to heel off during the stance phase after gait showed significant differences between before and after the experiment. [Conclusion] The results of this study indicate that Nordic pole walking increases the stride and can be considered as helping patients with a disease affecting their gait. Since the use of Nordic poles shortens the time from foot flat to heel off, it is considered that the use of Nordic poles can shorten the midstance during the stance phase.

[Purpose] To review and assess the effectiveness of whole body vibration therapy on the physical function of patients with type II diabetes mellitus. [Subjects and Methods] A computerized database search was performed through PubMed, Medline, EMBASE, the Cochrane Central Register of Controlled Trials, the Physiotherapy Evidence Database, and the reference lists of all relevant articles. The methodological quality was evaluated using the Physiotherapy Evidence Database scale. [Results] Five articles (four studies) with a combined study population of 154 patients with type II diabetes qualified for the inclusion criteria. Our review shows that whole body vibration therapy may have a positive impact on the muscle strength and balance of people with type 2 diabetes mellitus, whereas the effect on their mobility is still under discussion. [Conclusion] There was no sufficient evidence to support the premise that whole body vibration therapy is beneficial for the physical function of people with type II diabetes. Larger and higher-quality trials are needed.

[Purpose] The purpose of this study was to investigate the effects of combined exercise training with aerobic and functional strengthening exercises on balance ability of hemiplegic stroke patients. [Subjects] The subjects were 21 patients with chronic stroke who were attending a rehabilitation center. [Methods] Participants were randomly allocated to two groups: a combined exercise training group (n=11) and a conventional exercise group (n=10). Both groups did 60 minutes exercise per day, 5 times a week for four weeks. [Results] Both groups showed significant improvements in static and dynamic balances, and the combined exercise showed greater improvement in dynamic balance. [Conclusion] The result of this study suggests that combined exercise training with functional strengthening exercise and aerobic exercise was effective at improve static and dynamic balance ability, and was more effective than conventional exercise at improving dynamic balance. This suggests that combined exercise training can be prescribed for stroke patients to reduce their risk of falls and lead to independent ADL.

[Purpose] This study examined the activation of the rhomboid muscle according to the angle of the arm. [Subjects and Methods] The current study was conducted on 15 healthy males. The participants performed the pulling exercise in 5 conditions. The surface electromyography system was used to measure the muscle activities of the rhomboid and upper trapezius. [Results] The activity of the upper trapezius in condition 5 was significantly increased compared to that in condition 4. The activity of the rhomboid in condition 4 was significantly increased compared to that in conditions 1 and 5. [Conclusion] This study showed that performing a pulling exercise with the arms raised above the head (shoulder flexion at 120°) is more effective for reducing upper trapezius tension, while also selectively strengthening the rhomboid muscle.

[Purpose] This study investigated the effect of sleep posture on neck muscle activity. [Subjects and Methods] The study recruited 20 healthy subjects, who were positioned in three supine sleeping positions: both hands at sides, both hands on the chest, and dominant hand on the forehead. The activities of the scalene and upper trapezius muscles bilaterally were measured by surface electromyography. [Results] The upper trapezius and scalene muscle activity on the right side was significantly greater in the supine with dominant hand on the forehead position than in the other positions. [Conclusion] Sleep posture is important and prevent neck and shoulder musculoskeletal pain.

[Purpose] The objective of this study was to investigate whether vibroacoustic therapy (VAT) could improve the psychological symptoms of 15 elderly nursing home (NH) residents with symptoms of depression. [Methods] Fifteen subjects received VAT for 30 minutes per day for 10 days. Depression was evaluated using the Dementia Mood Assessment Scale (DMAS). Tympanic temperature, pulse, blood pressure, and SpO2 were measured as physiological indexes of relaxation. In addition, sleep-wake rhythms of the 15 subjects were evaluated using actigraphy. [Results] Based on DMAS scores, mitigation of depression was observed in NH residents after receiving VAT. Moreover, significant decreases in tympanic temperature and pulse were observed after treatment. Total sleeping hours per day showed a significant decrease when mean sleeping hours in the first week were compared with the mean sleeping hours in the second week. [Conclusion] VAT provided relaxation effects for elderly NH residents, and improved depressive symptoms.

[Purpose] This study was conducted to investigate the effects of ankle functional rehabilitation exercise on ankle joint functional movement screen results and isokinetic muscular function in patients with chronic ankle sprain patients. [Subjects and Methods] In this study, 16 patients with chronic ankle sprain were randomized to an ankle functional rehabilitation exercise group (n=8) and a control group (n=8). The ankle functional rehabilitation exercise centered on a proprioceptive sense exercise program, which was applied 12 times for 2 weeks. To verify changes after the application, ankle joint functional movement screen scores and isokinetic muscular function were measured and analyzed. [Results] The ankle functional rehabilitation exercise group showed significant improvements in all items of the ankle joint functional movement screen and in isokinetic muscular function after the exercise, whereas the control group showed no difference after the application. [Conclusion] The ankle functional rehabilitation exercise program can be effectively applied in patients with chronic ankle sprain for the improvement of ankle joint functional movement screen score and isokinetic muscular function.

[Purpose] To analyze the effects of neuromuscular electrical stimulation (NMES) applied to the abdominal muscles of patients with COPD. [Subjects] A total of 22 patients in group A, mean age of 68.2 ± 6.4 years and in group B, 71.2 ± 10.1 years. [Methods] Randomized clinical trial. Patients with COPD were randomly an evenly divided into two groups: group A (NMES + conventional therapy) and group B (conventional physiotherapy) and respiratory muscle strength was measured by pressure manometry before and after the end of the experimental protocols. [Results] Maximum inspiratory pressure before and after the intervention of group A was -64 (44–74) cm H2O, respectively, and -68 (56–96) cmH2O, and in group B, -52 (46–92) cm H2O and -60 (50–72) cmH2O. Maximum expiratory pressure of group A was 84 (72–92) cmH2O and 112 (94–120), respectively, and in group B, 76 (60–100) cmH2O and 84 (60–108) cmH2O. [Conclusion] The results indicate that expiratory muscle training with Russian current together with conventional physical therapy during hospitalization improves the respiratory muscle strength of patients with COPD.
