Purpose Fiber-reinforced composites (FRCs) have received considerable attention, owing to their potential use in dental prostheses or bone fracture fixation applications. The aim of this systematic review was to analyze and report the biological properties of FRCs reported in the existing literature.Study selections A systematic search of four databases (PubMed/MEDLINE, Scopus, Web of Science, and Cochrane library) was performed to identify all relevant studies published between 1962 and 2019. The search was limited to laboratory-based studies published in English. Citation mining was also performed through cross-referencing of included studies and hand searching of relevant journals.Results A total of 1283 potentially relevant articles were initially identified, and thirty-three articles were full-text screened. In the final ten studies included for review, four investigated bacterial adhesion and growth abilities on FRCs, four investigated the fibroblastic cytotoxicity of different surface-treated FRCs, and two investigated the osseointegration between bone and FRCs. Owing to the heterogeneity of fiber types, FRC-coating, and lack of standardized testing protocols, a meta-analysis was not feasible. The included studies indicated that glass fibers, and in particular E-glass fibers, are superior to ceramics and other FRCs in terms of bacterial adherence, fibroblast cytotoxicity, and cell viability.Conclusions Glass-fiber-reinforced composites are cytocompatible materials that possess satisfactory biological properties and can be used in dental prosthesis and craniofacial implants. Further research is necessary to regulate the matrix ion release/degradation of FRCs to prolong the initially demonstrated properties.

Purpose: To review the current clinical studies regarding the accuracy of implant computer-guided surgery in partially edentulous patients and investigate potential influencing factors.Study selection: Electronic searches on the PubMed and Cochrane Central Register of Controlled Trials databases, and subsequent manual searches were performed. Two reviewers selected the studies following our inclusion and exclusion criteria. Qualitative review and meta-analysis of the implant placement accuracy were performed to analyze potential influencing factors. Angular deviation, coronal deviation, apical deviation, and depth deviation were evaluated as the accuracy outcomes.Results: Eighteen studies were included in this systematic review, including six randomized controlled trials, nine prospective studies, and three retrospective clinical studies. A total of 1317 implants placed in 642 partially edentulous patients were reviewed. Eight studies were evaluated using meta-analysis. Fully guided surgery showed statistically higher accuracy in angular (P <0.001), coronal (P <0.001), and apical deviation (P <0.05) compared with pilot-drill guided surgery. A statistically significant difference (P <0.001) was also observed in coronal deviation between the bounded edentulous (BES) and distal extension spaces (DES). A significantly lower angular deviation (P <0.001) was found in implants placed using computer-aided design/computer-aided manufacturing (CAD/CAM) compared to the conventional surgical guides.Conclusion: The edentulous space type, surgical guide manufacturing procedure, and guided surgery protocol can influence the accuracy of computer-guided surgery in partially edentulous patients. Higher accuracy was found when the implants were placed in BES, with CAD/CAM manufactured surgical guides, using a fully guided surgery protocol.

Purpose: Denture adhesives improve the stability of incompatible dentures; however, complete removal of adhesives after use is difficult. Only a few studies have focused on the removal of denture adhesives. Hence, this study aimed to assess the efficacy of surfactants in removing cream denture adhesives from acrylic resin materials.Methods: Solutions of twelve surfactants with various hydrophilic-lipophilic balance (HLB) values were prepared. Two cream denture adhesives, colored for visualization, were spread onto transparent acrylic resin plates. After immersion into surfactant solutions, the effects of the surfactants on residual adhesives were evaluated. We also investigated the effect of denture cleaners (with or without the surfactants) on the removability of adhesives and artificial oily dirt, and their effects on the surface properties of denture materials. The obtained data were analyzed using appropriate statistical methods.Results: Five surfactants [BT-5, BL-4.2, BT-7, BT-9, and Triton X-100 (TX)] with HLB values in the 10.5-13.5 range effectively removed adhesives. Addition of BT-9 and TX (HLB=13.5) to denture cleaners improved the adhesives’ removal. Furthermore, the addition of TX to the cleaners did not interfere with the removal of artificial oily dirt and did not damage the denture materials’ surface.Conclusion: Surfactants with HLB values in the 10.5-13.5 range are suitable for removal of cream denture adhesives from acrylic resin materials. In particular, TX (HLB=13.5) efficiently removes adhesives without damaging denture materials or impairing original detergency.

Purpose: To clarify the effect of a multi-purpose primer combining several functional monomers on two prosthodontic materials (cast titanium and a gold alloy) after airborne-particle abrasion.Methods: Disk-shaped adherends were prepared from cast titanium (CP Titanium JIS2) and a gold alloy (Casting Gold M.C. Type IV). A silane-containing two-liquid primer (M&C primer (MC)) and two silane-free single-liquid primers (Alloy Primer (AP) and V-Primer (VP)) were used as surface-treatment agents. The shear bond strengths were determined before and after thermocycling to evaluate the adhesive durability, and the results were compared using a non-parametric statistical analysis. The effect of airborne-particle abrasion with alumina on the titanium surface was analyzed by X-ray photoelectron spectroscopy (XPS).Results: There was no significant difference in bond strength between the MC and AP before and after thermocycling, whereas VP showed significantly lower values. XPS revealed that the titanium acquired hydrophilic properties after the airborne-particle abrasion.Conclusions: The novelty of this study is that it shows that the presence/absence of the silane had no effect on the bonding of cast titanium with an acrylic resin. The study also showed that the multi-purpose primer can be used without any problems with both cast titanium and gold alloy, in combination with airborne-particle abrasion with alumina.

Purpose: The purpose of this study was to evaluate the influence of water immersion on the mechanical properties of three kinds of glass fiber posts and the fracture resistance of structures using resin composites with glass fiber posts.Methods: Each post was divided into three groups; a control group and two water immersion groups (30 and 90 days). Flexural strength was determined by three-point bending test. Each structure was divided into two groups; a control group and a water immersion group for 30 days. The fracture strength of structures was determined by a static loading test.Results: In the flexural strength, two kinds of post in water immersion groups showed lower values than control groups. In the fracture strength, two kinds of structures in water immersion group showed lower values than control groups.Conclusion: The prefabricated glass fiber posts and structures using resin composites with glass fiber posts were affected by water immersion.

Purpose: To determine the effectiveness of palatal brushing in the treatment of denture-related erythematous stomatitis (DES) in complete denture wearers.Methods: This two-parallel-arm RCT was conducted in three university clinics in Brazil, Canada, and Chile. Participants (n=77) were randomly allocated to receive (i) instructions for palatal brushing and standard oral/denture hygiene (“intervention”); or (ii) standard oral/denture hygiene instructions only (“control”). Data collection was carried out at the baseline and at 3 and 6 months after intervention. Outcomes included the magnitude of oral Candida carriage and the degree of inflammation of denture-bearing tissues. Groups were compared using generalized estimating equations and chi-square test (α=0.05).Results: Palatal inflammation levels were reduced significantly in the “intervention” compared to “control” group at 6 months (intervention: 70%, control: 40%; chi-square, p=0.04). There was no between-group significant difference in the Candida count from denture and palatal biofilms; however, a subgroup analysis restricted to baseline Candida carriers showed further reduction with the intervention at 6 months. No adversity was observed by trialist or reported by participants.Conclusion: Including palatal brushing in oral instructions for denture wearers has positive impact on DES-related mucosal inflammation. Thus, our findings endorse the inclusion of palatal brushing in standard oral hygiene instructions to treat DES.

Purpose: Evidence regarding the performance of digital dentures (DDs) fabricated using 3D printing is insufficient. This study aimed to evaluate patient satisfaction with conventional dentures (CDs) and DDs fabricated using 3D printing.Methods: This randomized controlled trial was conducted between November 2017 and May 2020. In the CD-DD group, CDs were fabricated before DDs, while the protocol was reversed in the DD-CD group. The primary outcome was patient satisfaction, quantified using a 100-mm visual analog scale, which assessed chewing efficiency, pain, stability, retention, comfort, esthetics, ease of cleaning, phonetics, and general satisfaction. Secondary outcomes were quality of life (QOL), number of visits, time required for definitive denture fabrication, number of adjustment appointments, and time required for denture stabilization after denture delivery.Results: Patient satisfaction with CDs was superior in terms of phonetics, ease of cleaning, stability, comfort, and general satisfaction. Secondary outcomes such as social disability and the number of clinic visits were significantly lower in patients with DDs. However, the two groups showed no significant differences in the other outcomes. Although patient satisfaction with DDs was inferior to that with CDs, 20% of patients preferred and used DDs (12 patients preferred CDs, and three patients opted for DDs).Conclusion: Although patient satisfaction or QOL with DDs may be somewhat inferior to that with CDs, 20% of patients preferred and used DDs daily. Thus, DDs fabricated using 3D printing may have comparable practicality and efficacy to CDs.

Purpose: This study aimed to develop a simple screening test for mastication, “the Sakiika (squid jerky) transport test (STT), which evaluates the vertical jaw movement coordinated with the lateral tongue movement during stage I transport, and investigate the possibility of its clinical application. Methods: The study included 73 people with dysphagia (mean age, 78.5 ± 7.8 years; median age, 79.0 years; interquartile range, 75.0–84.0). The STT evaluated the ability of a participant to transport a piece of squid jerky placed on the midline of the tongue to the molar region. The STT score was defined as the number of vertical jaw movements occurring as the tongue transported food to the molars. A cutoff value was set by comparing the STT scores with masticatory function evaluated via a videofluoroscopic swallowing study and with food texture evaluated using the Food Intake LEVEL Scale (FILS). Results: The STT scores counted by the two examiners had a κ coefficient of 0.79, indicating good reliability. The STT score was significantly associated with both the presence of masticatory movement ( p = 0.019) and food texture classified by FILS ( p = 0.032) at cutoff value of “3” (3 vertical movements). The STT showed 62% sensitivity and 75% specificity for masticatory movements. Conclusion: The STT could be a useful screening test to assess the presence or absence of food transportation to the molars for mastication in older patients with dysphagia. In addition, the STT could be useful in identifying the need to modify food texture to meet functions.

Purpose: This study evaluated the influence of firing procedures and layering thickness of porcelain on internal adaptation of maxillary anterior cantilever zirconia resin-bonded fixed dental prostheses (RBFDPs). Methods: The maxillary right central incisor on a model was prepared for a single-retainer zirconia RBFDP. A total of 36 frameworks of cantilever zirconia RBFDPs were classified into three groups (CB0.5, CB1.0, and CB2.0 groups) based on the thickness of the cut-back on the facial surface (0.5 mm, 1.0 mm, and 2.0 mm, respectively). Feldspathic porcelain was layered onto the facial surface of the zirconia RBFDP frameworks. Internal space widths between the abutment tooth and the framework were measured before and after porcelain firing by a replica technique. The internal space values before and after porcelain firings were compared using the paired t-test within the same group (α = 0.05). Differences in internal space values (distortion) between before and after porcelain firing were compared using one-way analysis of varianc e and Tukey multiple comparison tests. Results: For all framework designs, internal space values after porcelain firing were significantly higher than those before firing. Among all groups, the CB0.5 group showed the lowest internal distortion values for the whole measured area. The CB2.0 group showed higher distortion values than the CB1.0 group. Conclusions: Porcelain firing procedures negatively impacted the internal space in cantilever zirconia RBFDPs. In addition, the increased volume of layering porcelain had a negative effect on the internal distortion of ma xillary anterior cantilever zirconia RBFDPs.

The comparison condition used in this study was the diagnostic accuracy achieved by advanced specialty residents on these same vignettes using a virtual patient system. Methods: 27 individual disease vignettes were utilized. These vignettes had a variety of orofacial pain and oral medicine diseases. Post graduate orofacial pain and oral medicine residents at our University of Southern California interacted with their randomly assigned virtual patients were analyzed [n=574]. Virtual patient accuracy was based on whether the user selected the primary diagnosis as one of their top four choices after interviewing. Eleven English-language symptom checkers accuracy was based on whether the vignettes produced the primary diagnosis as one of their top four choices. Using these data, symptom checker and virtual patient accuracy rates were calculated. Results: The primary diagnosis on virtual patient encounters was found within the top four choices a mean of 67.2% of the time. The primary diagnosis for the same vignettes entered into the 11 symptom checkers was found within the top four choices a mean of 5.9% of the time. Conclusions: The accuracy of currently available symptom checkers that patient might use for self-diagnosis of common orofacial pain and oral medicine diseases was low, this result suggest that the improved diagnostic algorithms are needed.

Purpose: This study investigated the impact of the renewal of a removable prosthesis on the masticatory function by subjective and objective measures and its variation among the types of occlusal support. Methods: Seventy-eight patients who received newly fabricated removable denture patients participated in this study. For the objective assessment, masticatory performance was measured using test gummy jelly. For the subjective assessment, standardized questionnaires about food acceptability and the oral health-related quality of life (OHRQoL) were used. Pre- and post-insertion assessments were performed for each subject. Subjects were divided into three groups according to their posterior occlusion: with posterior occlusion (w/PO), without posterior occlusion (w/o PO) and edentulous. Wilcoxon’s signed rank test was used to compare the pre- and post-treatment measurements of each assessment. The analysis of covariance and a multiple comparison were used to assess the effect of new dentures and differences due to occlusal support. Results: The masticatory performance, OHRQoL and food acceptability following prosthodontic treatment were significantly improved by new denture insertion. The masticatory performance among groups varied to a relative degree. The rate of masticatory performance improvement for edentulous subjects was twice that in w/PO subjects. The OHRQoL was significantly lower in the w/o PO and edentulous groups with old denture than patients w/PO. The food acceptability improved most markedly in the edentulous group. Conclusions: The improvement in the masticatory performance by new denture insertion varied among types of occlusal support. Re-establishing the occlusal support of edentulous patients may help restore their OHRQoL and improve food acceptability.
