Three cases of intrascrotal rupture of the testis were presented. Case 1: A 21-year-old student got a kick at his left scrotum during Kwarate training. On the third posttraumatic day, the ruptured tunica albuginea was surgically sutured. Case 2: A 25-year-old man fell from the height and got the right scrotum injured. On the third posttraumatic day, the ruptured tunica albuginea was sutured. Case 3: A 58-year-old man got an injury at his right scrotum due to traffic accident while on a motorcycle. It was on the tenth posttraumatic day that the operation was performed. Because the tunica albuginea was so extensively injured, orchiectomy was necessitated. Histological findings of the ruptured testes were described. The rat testes were experimentally ruptured and histological study was made several times after trauma in order to investigate the changes with time.

1) VAB-6(Vinblastine, actinomycin D, Bleomycin, cyclophosphamide, cisplatinum)療法を行なった症例で,GOT, GPT, LDH, γ-GPTなどの上昇と平行したAFPの一過性の上昇を経験した.2) 1979年より1984年まで当教室で治療した進行性睾丸腫瘍11例につき,PVB療法,salvage療法,VAB-6療法のそれぞれによる肝障害の有無につき検討した.VAB-6療法症例ではGOT, GPT, LDHは化療後有意に上昇しており,治療前正常であったAFPも治療後有意に上昇していた.3) VAB-6療法後のAFPの一時的軽度の上昇は一過性の肝機能障害と強く相関していた.4)一部の症例ではAFPのconcanavalin Aへの吸着性の検討が可能であり,治療前正常であったAFPの一過性の上昇は肝臓由来と思われた

1983年10月~1995年6月迄に福井医科大学において治療した腎盂尿管癌40例に関して臨床的検討を行った.平均年齢は65歳,男女比は1.5であった. 1)組織型は全例移行上皮癌で,深達度では壁内浸潤(pT1とpT2)を12例,壁外浸潤(pT3とpT4)を19例に認めた.リンパ節転移は12例に,遠隔転移は4例に認められた. 2)腎盂・尿管壁内脈管侵襲としてリンパ管侵襲を63%に,静脈侵襲を46%に認め,その頻度は深達度,異型度と関連した. 3)膀胱癌の併発を21例に認めた. 4)全症例での5年生存率は57.1%で,深達度,脈管侵襲が予後と有意に関連していたWe reviewed 40 patients with renal pelvic and/or ureteral transitional cell carcinomas, consisting of 24 males and 16 females with a mean age of 65 years. The histopathological stage of surgically removed specimen was pTa in 6 patients, pT1 in 7, pT2 in 5, pT3 in 11 and pT4 in 6. Three patients with Tis and 2 with T3 did not undergo surgery. Of 35 patients pathologically examined, lymphatic and venous invasions were detected in 22 (63%) and 16 (46%), respectively, and were associated with pathological stage and grade. Overall the 5-year actuarial survival rate was 57.1%. Tumor staging and vascular invasion had a prognostic significance on the treatment outcome, but not metachronous or synchronous bladder cancer, identified in 55% of the patients. Adjuvant chemotherapy appeared to improve the survival of the patients with tumors pT2 or higher, grade 3 or vascular invasion without metastases.

精巣の血流遮断と解除,放射線照射,およびホルモン投与の3つの条件下で31P MRSを測定した。1)血流遮断後,ATPはすみやかに減少し60分以内に完全に消失した。2) 3時間までの血流遮断では遮断解除後にATPの回復が認められたが,4時間以上の遮断では解除後もATPの回復は認められなかった。3)放射線照射2週間後ではPME/β-ATP比およびPME/PDE比の減少が認められ,PMEの減少は造精機能障害の指標として有用と考えられた。4)ホルモン投与によるgonadotropin抑制モデルでは,PME/PDE比の減少とPDE/β-ATPの増加を認め,PDEの増加も造精機能障害の第二の指標となりうるTo evaluate the efficacy of 31P magnetic resonance spectroscopy (31P MRS) as a diagnostic method for abnormal testicular function, we examined three conditions using rat testes, ischemia, irradiation, and hormone manipulation. 1) Ischemia: Immediately after clamping of the feeding vessels, ATP signals began to fall and disappeared within 60 minutes. With the release of blood supply after 3 hours of ischemia, ATP appeared within 2 hours. However, after more than 4 hours of ischemia, ATP did not recover within 2 hours and the testis became necrotic after 1 week. 2) Irradiation: 31P MRS of the testis 2 weeks after irradiation with 10 Gy., 9 MeV showed a significant decrease (P < 0.05) in the PME/beta-ATP ratio from 1.30 +/- 0.11 (control level) to 1.11 +/- 0.12 and a decrease in PME/PDE ratio from 1.43 +/- 0.17 to 1.13 +/- 0.20. However 3 weeks later, the PME/beta-ATP ratio recovered to a control level. 3) Hormone manipulation: In the testes 5 weeks after weekly intramuscular injections of estradiol benzoate and testosterone enanthate, PDE/beta-ATP ratio significantly increased (P < 0.01) from 0.83 +/- 0.09 (control level) to 1.03 +/- 0.19. 31P MRS is a non-invasive method for evaluation of various testicular abnormalities, and ATP signals may be useful to evaluate an acute ischemic change for example testicular torstion and the changes of PME, PDE signals may be useful parameters in the assessment of the status of spermatogenesis.
