Patulous eustachian tube (PET) is not an uncommon disease. Drugs are seldom used for PET treatment. The purpose of this study was not to evaluate the usefulness of the Chinese medicine “Kami-Kihi-To” for PET, but to call attention to the possibility of drug therapy for PET.The diagnosis of PET is made from its symptoms and abnormal tympanic membrane movement during hyperventilation detected with impedance audiometry.“Kami-Kihi-To” was administered to 88 patients, and 66 of whom questioned subjective symptoms, and 24 of whom were evaluated objectively by impedance audiometry. Complete relief was achieved in 36 patients (54.5%) treated with “Kami-Kihi-To”, and 14 patients (21.2%) obtained partial relief. There was objective evidence of complete improvement of tympanic membrane movement in eight patients (33.3%), and partial improvement in nine patients (37.5%). Side effect were noted in three patients (4.5%).These results suggest that drug therapy for PET is possible.

The present study was made to investigate in detail the mechanism of the bileflow into the duodenum in rabbit. The bile flow through the choledochoduodenaljunction was measured with a constant-rate infusion apparatus or with a constant-pressure perfusion apparatus. Pressures in gall-bladder, common bile duct and duodenum were recorded with pressure transducer. The electrical activities were recorded with bipolar electrodes which were implanted in the choledochoduodenal junction (muscle of Oddi) and the anterior wall of duodenum. The results obtained are as follows:1. The activity of choledochoduodenal junction appears to be affected by the change in duodenal wall muscle tone.2. The important factors controlling the bile flow are the rhtyhmic movements of both the choledochoduodenal junction and the duodenal wall.3. The common bile duct plays a role to regulate the pressure of biliary truct.4. Electrical and mechanical stimulations to gall-bladder were ineffective to cause the relaxing reflex of the muscle of Oddi.5. The function of gall-bladder is to accumulate the bile and control the pressure of common bile duct.From these results, it seems unlikely that the choledochoduodenal junction (muscle of Oddi) would be independent of duodenum in rabbit.

モルモットの摘出胆のう,総胆管,Oddi筋にはα収縮性受容体が存し,消化管とは異なった生理機能臓器である.胆のうは不可逆的α遮断薬であるdibenamineの比較的低用量において,α作用が消失逆転することから,α受容体量は少ない.また,経壁刺激反応からはcholine作働性収縮が顕著に優位で,このcholine作働性収縮に対してadrenaline作働性α作用が神経末端に抑制支配を行なうことが重要と思われるが,adrenaline作働性の直接支配も無視出来ない.胆のう壁はtyramine作用において,tyramine遊離型の内因性catecholamineをほとんど含有していない.しかし5-HTによって遊離されるcatecholamineを含有する.総胆管は自動運動を持ち,α収縮性受容体とβ弛緩性受容体があり,その収縮,弛緩力によって胆道内圧調節と胆汁排出上に積極的に関与すると考えられた.モルモットOddi筋のα受容体は収縮性で,十二指腸のそれと異なることから,Oddi筋は十二指腸より独立していると言える.一方,ウサギの場合,摘出胆のうは反応性に貧しく,神経支配機能を充分検討できなかったが,β弛緩性受容体の存在が推論された.総胆管標本は,α収縮性受容体,β弛緩性受容体が存し,また,choline作働性収縮支配,adrenaline作働性収縮,弛緩支配があり,自動運動も有することから,胆道内圧調節に積極的に関与すると思われる.しかしOddi筋はα,β両受容体とも弛緩性であり,摘出,生体位とも十二指腸類似であった.神経支配もcholine作働性収縮とatropine抵抗性収縮支配,非adrenaline作働性弛緩支配がみられ,十二指腸と質的に同じであった.それ故,ウサギOddi筋はadrenaline受容体,自律神経支配様式からは,十二指腸よりの独立性を支持出来ない.またウサギにおけるこれら神経支配機構は,胆汁排出上の重要因子ではなく,主にcholecystokininなどのホルモン性調節が重要なものであろう.
