Objective: Early mobilization and rehabilitation has become common and expectations for physical therapists working in intensive care units have increased in Japan. The objective of this study was to establish consensus-based minimum clinical practice standards for physical therapists working in intensive care units in Japan. It also aimed to make an international comparison of minimum clinical practice standards in this area. Methods: In total, 54 experienced physical therapists gave informed consent and participated in this study. A modified Delphi method with questionnaires was used over three rounds. Participants rated 272 items as "essential/unknown/non-essential". Consensus was considered to be reached on items that over 70% of physical therapists rated as "essential" to clinical practice in the intensive care unit. Results: Of the 272 items in the first round, 188 were deemed essential. In round 2, 11 of the 62 items that failed to reach consensus in round 1 were additionally deemed essential. No item was added to the "essential" consensus in round 3. In total, 199 items were therefore deemed essential as a minimum standard of clinical practice. Participants agreed that 42 items were not essential and failed to reach agreement on 31 others. Identified 199 items were different from those in the UK and Australia due to national laws, cultural and historical backgrounds. Conclusions: This is the first study to develop a consensus-based minimum clinical practice standard for physical therapists working in intensive care units in Japan.

We investigated the microbicidal effect of weak acid hypochlorous solutions of pH 5.0 - 6.0, produced by mixing NaClO and HCl in water, against various bacteria, fungi, and virus in vitro. The weak acid hypochlorous solution had excellent microbicidal effect against a broad microbicidal spectrum of standard strains and clinical isolates in a short time. The microbicidal effects of hypochlorous solutions did not depend on the available chlorine concentration but on the HClO concentration. These results show that the weak acid hypochlorous solution has practical applicability in such places as hospitals and establishments related to the food industry.

A working group is studying the feasibility of a future Japanese space-borne coherent Doppler wind lidar (CDWL) for global wind profile observation. This study is composed of two companion papers: an instrumental overview of the space-borne CDWL for global wind profile observation (Part 1), and the wind measurement performance (error and bias) investigated using a full-fledged space-borne CDWL simulator (Part 2). This paper aims to describe the future space-borne CDWL in terms of technical points and observation user requirements. The future mission concept is designed to have two looks for vector wind measurement with vertical resolutions of 0.5 (lower troposphere: 0-3 km), 1 (middle troposphere: 3-8 km), and 2 km (upper troposphere: 8-20 km) and horizontal resolution of < 100 km along a satellite. The altitude and orbit of the satellite are discussed from a scientific viewpoint. The candidate altitude and orbit of the satellite are 220 km and an inclination angle of 96.4° (polar orbit) or 35.1° (low-inclination-angle orbit). The technical requirements of the space-borne CDWL are a single-frequency 2-μm pulse laser with an average laser power of 3.75 W, two effective 40-cm-diameter afocal telescopes, a wide-bandwidth (> 3.4 GHz) detector, a high-speed analog-to-digital converter, and a systematic lidar efficiency of 0.08. The space-borne CDWL looks at two locations at a nadir angle of 35° at two azimuth angles of 45° and 135° (225° and 315°) along the satellite track. The future space-borne CDWL wind profile observation will fill the gap of the current global wind observing systems and contribute to the improvement of the initial conditions for numerical weather prediction (NWP), the prediction of typhoons and heavy rain, and various meteorological studies.

Purpose: Pulsed arterial spin labeling (PASL) is a magnetic resonance (MR) method for measuring cerebral blood flow. Although several validation studies for PASL in animals and humans have been reported, no reports have detailed the fundamental study of PASL using a flow phantom. We compared the true and theoretical flow rates in a flow phantom to confirm the analytical validity of quantitative perfusion imaging with Q2TIPS sequence. Methods: We built a flow phantom consisting of a 40-mm diameter plastic syringe filled with plastic beads and small plastic tubes 4 mm in diameter. Gd-DTPA-doped 8L water solution (0.1 mM) was circulated between the syringe and a tank through a plastic tube by a constant flow pump while the flow rate was adjusted between 0 and 2.61 cm/s. Q2TIPS sequence parameters were TI1=50 ms and TI2=1400 ms. Five imaging slices of 50 subtraction images were acquired sequentially in a distal-to-proximal direction using a single-shot echo planar imaging (EPI) technique. The theoretical flow rate calculated based upon the previously reported kinetic model for Q2TIPS was compared with the true flow rate. Results: A good linear relationship was observed between the theoretical, F′, and true flow rates, F, in a flow rate range of 1.43 to 1.95 cm/s (F′=1.024•F−1.915, R2=0.902). The ratio of theoretical to true flow rate was 92 (+/−) 4%. Conclusion: Flow rate was quantified with reasonable accuracy when the entire amount of labeled bolus within the phantom could be recovered. Our experiment confirmed the analytical validity of Q2TIPS and suggested that blood flow measurement may be feasible using the Q2TIPS pulse sequence and kinetic model of the PASL equation.

CX3CL1-/fractalkine is a chemokine with a unique CX3C motif. Hypoxia mediates the expression of various genes, such as vascular endothelial growth factor (VEGF), cyclooxygenase-2, and plasminogen-activator inhibitor-1, in vascular endothelial cells. We studied the eect of hypoxia on the expression of fractalkine induced by interferon-γ (IFN-γ) in endothelial cells. Human umbilical vein endothelial cells were cultured, and the stimulation of the cells with IFN-γ was found to induce the expression of fractalkine. Hypoxia inhibited the expression of fractalkine mRNA and protein by IFN-γ, and this eect was observed with concomitant increase in VEGF expression. Desferrioxamine, an iron chelator that mimics hypoxia in vitro, also inhibited the fractalkine production induced by IFN-γ. Hypoxia did not aect the degradation of fractalkine mRNA. The inhibition of fractalkine expression by hypoxia was reversed on returning the cultures to reoxygenation condition. Inhibition of IFN-induced fractalkine expression by hypoxia was not aected by the presence of a radical scavenger, N-acetyl-L-cysteine, and the involvement of reactive oxygen species may be excluded. Inhibition of fractalkine expression by hypoxia may be involved in the pathophysiology of ischemic diseases.
