The aim of this study was to show the reaction varieties of human tooth enamel and dentin with a carbonated soft drink, Sprite. After one week exposing into the soft drink, the sliced tooth specimens were examined by macroscopic and microscopic and then carried out the micro X-ray diffraction analysis using a 100 μm diameter X-ray beam. The macroscopic and microscopic results showed the wide varieties from the one showing a very slight increase white-spot areas in the enamel to the another showing almost complete decay of the enamel. The crystallites in the examined enamel and dentin changed drastically as follows; the one enamel sample showed the decrease of crystal amount and some ionic substitutions, and the another dentin sample showed the increase in crystallinity and ionic substitutions. These results clearly showed that the original tooth enamel and dentin had their own crystallographic properties differing from each others. This study provided a crystallographic basic data which will be applied to a tayler-made individual preventive dentistry in near future.

We herein report the first case of foveolar-type gastric adenocarcinoma that developed after the initiation of vonoprazan (VPZ). A 51-year-old man had heartburn at the first visit and reflux esophagitis endoscopically, so he started taking VPZ. An approximately 5-mm-sized reddish polyp with a raspberry-like morphology was detected at the anterior wall of the upper body of the stomach 156 weeks after starting maintenance therapy with VPZ 10 mg/day. It was diagnosed as foveolar-type gastric adenocarcinoma based on a biopsy. Another approximately 4-mm-sized foveolar-type gastric adenocarcinoma was also detected at the posterior wall of the middle body of the stomach.

To elucidate the crystallo-chemical reaction between the enamel and carbonated soft drink, a micro-X-ray diffraction analysis (micro-XRD) was conducted using the human tooth sections soaked in the drink (Sprite®) for 1 and 7 days. In this study, 4 typically and heavily dissolved teeth were selected from 20 teeth, and the divergence of the crystallographic properties of the enamel was analyzed.All of the untreated human tooth enamels exhibited only the apatitic XRD patterns. We confirmed that the carbonated soft drink changed the macro- and microscopic morphological features of individual tooth enamel. The following 4 results were obtained from our study: i) the unit cell dimensions and crystallinity of the untreated enamel apatite varied between the layers and the individual teeth; ii) after the soaking experiment, the crystallographic properties of the enamels changed remarkably and displayed non-uniformity; iii) no relationship was discerned between the enamel’s crystallographic properties and the sensitivity of the carious attack; and iv) the intermediate reaction product of soaking was uncovered; however, it was not identified at this time. The presence or absence of occurrence and variations in the amount of intermediate product reflected the complex chemical and crystallo-chemical reactions between the decalcification solution and the human tooth enamel crystallites.We clearly established that the untreated human tooth enamels differed in their crystallographic properties and did not react in the same manner, which resulted in varied apatitic structures after soaking. These results necessitate reconsideration of the generally accepted caries protection methods that are applied as common standards for all individuals and tooth enamels.

Coronavirus disease 2019 (COVID-19) causes a variety of pain symptoms in the acute phase. Severe chest pain suddenly occurs even without abnormalities on examination and is sometimes refractory to analgesics. Such pain is a clinical concern in care facilities with limited resources, and this is the first report on the use of saikanto for its treatment. In Miyagi Prefecture, Japan, COVID-19 patients with mild symptoms were admitted to a hotel that operated as an isolation facility, and their symptoms were observed. In this article, we report four cases in which chest pain comorbid with mild to moderate COVID-19 was successfully treated with saikanto, a traditional Japanese (Kampo) medicine. The patients presented with chest pain and underwent medical examination at the facility. Two patients had severe chest pain refractory to acetaminophen. Critical cardiopulmonary diseases were ruled out in all the patients, and three patients had features of pneumonia on chest radiograph. Medications, including saikanto, were administered to the patients. The patients’ chest pain and other symptoms improved 1-4 days after the administration of saikanto, and they left the care facility without hospitalization. The cause of the chest pain experienced by these patients is unclear, but we speculate that it could be minimal pleural inflammation or neuropathy. Previous pharmacological studies have suggested anti-inflammatory and analgesic properties of the crude drugs that constitute saikanto. This case report suggests that saikanto could be a treatment option for chest pain refractory to analgesics in patients with mild to moderate COVID-19.

The present study investigated cis-unsaturated free fatty acid (FFA)-regulated glucose uptake. In the cell-free assay of protein tyrosine phosphatase 1B (PTP1B), cis-unsaturated FFAs such as linoleic, linolenic, and oleic acid significantly suppressed PTP1B activity in a concentration (1 – 100 μM)-dependent manner, with the highest potential for oleic acid. Oleic acid (1 μM) stimulated insulin (0.1 nM)-induced phosphorylation of the insulin receptor at Tyr1185 and increased insulin (0.1 nM)-induced phosphorylation of Akt at Thr308 and Ser473 in differentiated 3T3-L1-GLUT4myc adipocytes. In the föerster resonance energy transfer analysis, oleic acid activated Rac1 in PC-12 cells, which is inhibited by the phosphatidylinositol 3-kinase (PI3K) inhibitor wortmannin, the 3-phosphoinositide-dependent protein kinase-1 (PDK1) inhibitor BX912, or the Akt inhibitor MK2206. Oleic acid (1 μM) significantly increased insulin (0.1 nM)-stimulated glucose uptake in 3T3-L1-GLUT4myc adipocytes, although oleic acid by itself had no effect on the glucose uptake. Taken together, the results of the present study show that oleic acid enhances insulin receptor signaling through a pathway along an insulin receptor/PI3K/PDK1/Akt/Rac1 axis in association with PTP1B inhibition and facilitates insulin-induced glucose uptake into adipocytes.
