To assess the effectiveness of response strategies of avoiding large gatherings or crowded areas and to predict the spread of COVID-19 infections in Japan, we developed a stochastic transmission model by extending the Susceptible-Infected-Removed (SIR) epidemiological model with an additional modeling of the individual action on whether to stay away from the crowded areas. The population were divided into three compartments: Susceptible, Infected, Removed. Susceptible transitions to Infected every hour with a probability determined by the ratio of Infected and the congestion of area. The total area consists of three zones crowded zone, mid zone and uncrowded zone, with different infection probabilities characterized by the number of people gathered there. The time for each people to spend in the crowded zone is curtailed by 0, 2, 4, 6, 7, and 8 hours, and the time spent in mid zone is extended accordingly. This simulation showed that the number of Infected and Removed will increase rapidly if there is no reduction of the time spent in crowded zone. On the other hand, the stagnant growth of Infected can be observed when the time spent in the crowded zone is reduced to 4 hours, and the growth number of Infected will decrease and the spread of the infection will subside gradually if the time spent in the crowded zone is further cut to 2 hours. In conclusions The infection spread in Japan will be gradually contained by reducing the time spent in the crowded zone to less than 4 hours.

Web application second-order vulnerabilities first inject malicious code into the persistent data stores of the web server and then execute it at later sensitive operations, causing severe impact. Nevertheless, the dynamic features, the complex data propagation, and the inter-state dependencies bring many challenges in discovering such vulnerabilities. To address these challenges, we propose DISOV, a web application property graph (WAPG) based method to discover second-order vulnerabilities. Specifically, DISOV first constructs WAPG to represent data propagation and inter-state dependencies of the web application, which can be further leveraged to find the potential second-order vulnerabilities paths. Then, it leverages fuzz testing to verify the potential vulnerabilities paths. To verify the effectiveness of DISOV, we tested it in 13 popular web applications in real-world and compared with Black Widow, the state-of-the-art web vulnerability scanner. DISOV discovered 43 second-order vulnerabilities, including 23 second-order XSS vulnerabilities, 3 second-order SQL injection vulnerabilities, and 17 second-order RCE vulnerabilities. While Black Widow only discovered 18 second-order XSS vulnerabilities, with none second-order SQL injection vulnerability and second-order RCE vulnerability. In addition, DISOV has found 12 0-day second-order vulnerabilities, demonstrating its effectiveness in practice.

Background: Administering premixed drugs in commodity packets was first reported in Asia in 2015, but there continues to be a dearth of related population-based data. This study aimed at examining (1) the prevalence of drug packet use in the population and (2) the sociodemographic profiles, particularly gender distribution, of drug packet users.Methods: Data were derived from a survey of 18,626 Taiwanese civilians, aged 12-64 years, using stratified, multi-stage, random sampling in 2018. Participants anonymously completed a computer-assisted self-interview on tablet computers which covered the use and problematic use of illicit drugs/inhalants, prescription drugs and other psychoactive substances, among others.Results: Approximately 1.46% of respondents had a lifetime use of illicit drugs, with drugs in commodity packets (0.18%) being ranked the fifth-most commonly used illicit drugs, higher than nitrous oxide (0.14%) and heroin (0.09%). Ten formats of drug packets were endorsed by users. Approximately 81.6% of persons with drug packet use had a lifetime use of other illicit drugs. The correlates of the use of drugs in commodity packets were similar to those of the exclusive use of other drugs except that there was a lack of gender differences in the use of drugs in commodity packets but not in the exclusive use of other drugs.Conclusion: Drugs in commodity packets have become a common way of administering illicit drugs in the population in Taiwan, and there were no gender differences among users. Our findings have implications for more efficient drug testing and culturally appropriate intervention for drug packet use.

During a six-week period from July 20 to August 31, 2022, Japan experienced its highest level of COVID-19 infection ever, with an average of nearly 200,000 new infections per day nationwide. Cases requiring inpatient care peaked at 1,993,062. Twenty-seven prefectures (out of 47 prefectures) had an average hospital bed occupancy of 50% or higher, and bed occupancy in Kanagawa in particular reached 98% in mid-August. In Tokyo, bed occupancy by patients with severe COVID-19 reached 57% and peaked at 64% in mid-August. Although the number of new infections per day has decreased since September, hospital bed occupancy, the number of severe cases, and deaths remain high nationwide. Efforts including vaccination campaigns, domestic surveillance, and routine infection control measures based on the varied knowledge that the Japanese public already has should be thoroughly implemented to reduce the number of the infected in order to avoid an increase the number of serious cases and deaths.

As the number of people with COVID-19 increases daily around the world, point-of-care testing (POCT) is gaining attention as a tool that can provide immediate test results and greatly help to deter infection and determine what to do next. POCT has several drawbacks such as a low sensitivity and specificity, but according to studies POCT has increased sensitivity on par with that of polymerase chain reaction testing. The advantage of POCT is that the results can be obtained quickly, regardless of the location. To further enhance its benefits, POCT is being developed and researched in conjunction with the Internet of medical things (IoMT), which allows POCT results to be collected, recorded, and managed over a network. IoMT will be beneficial not only for the use of POCT simply as a testing tool but also for its integration into diagnostic and health management systems. IoMT will enable people to regularly receive their test results in their daily lives and to provide personalized diagnosis and treatment of individual conditions, which will be beneficial in terms of disease prevention and maintenance of health.

The toxic heavy metal cadmium has been proven to cause pancreatic dysfunction and lead to the development of DM. However, the underlying mechanisms have not been completely elucidated. Here, we investigated the effects of cadmium on the pancreatic β cell line MIN6 and explored the underlying mechanisms. The Cell Counting Kit-8 (CCK8) assay and flow cytometry were used to determine cell viability and apoptosis in MIN6 cells. The expression levels of signal transducer and activator of transcription 6 (STAT6) were assessed by western blotting. We further assessed the effects of cadmium on the function of pancreatic β cells under high glucose levels using enzyme-linked immunosorbent assay (ELISA) and western blotting. Insulin secretion and expression were decreased by cadmium in MIN6 cells. In addition, cadmium suppressed cell viability and promoted apoptosis of MIN6 cells, downregulated insulin secretion and genesis of MIN6 cells under high glucose conditions, while inhibiting STAT6. Furthermore, after treatment with IL-4, the activator of STAT6, the MIN6 cell viability suppression and apoptosis promotion effect caused by cadmium were blocked. In conclusion, cadmium inhibits pancreatic β cell MIN6 growth by regulating the activation of STAT6. Our findings reveal a new mechanism of cadmium toxicity in pancreatic β cells.

The first case of COVID-19 in Japan was reported on 16 January 2020. The total number of the infected has reached 313,844 and the number of deaths has reached 4,379 as of 16 January 2021. This article reviews the characteristics of and responses to the three waves of COVID-19 in Japan during 2020-2021 in order to provide a reference for the next step in epidemic prevention and control. The Japanese Government declared a state of emergency on 7 April 2020, which suppressed the increase in the number of the infected by curtailing economic activity. The first wave peaked at 701 new cases a day and it decreased to 21 new cases on May 25 when the state of emergency was lifted. However, the number of the infected increased again due to the resumption of economic activity, with a peak of 1,762 new cases a day during the second wave. Although the situation was worse than that during the first wave, the government succeeded in limiting the increase without declaring a state of emergency again, and that may be attributed to a decrease in crowd activities and an increase in the number of inspections. During the third wave, the number of the infected continued to exceed the peak during previous waves for two months. Major factors for this rise include the government’s implementation of further policies to encourage certain activities, relaxed immigration restrictions, and people not reducing their level of activity. An even more serious problem is the bed usage for patients with COVID-19; bed usage exceeds 50% not only in major cities but also in various areas. On 7 January 2021, 5,953 new cases were reported a day; this greatly exceeded the previous peak, and the state of emergency was declared again. Although Japan has been preparing its medical system since the first wave, maintaining that system has imposed a large economic burden on medical facilities, hence stronger measures and additional support are urgently needed to combat COVID-19 in the coming few months.

We investigated the accuracy of various bleeding risk scores to estimate the bleeding risk in patients with acute myocardial infarction (AMI) managed with percutaneous coronary intervention (PCI) access via the radial artery.We retrospectively enrolled 1,651 patients who were definitively diagnosed with ST-elevation myocardial infarction (STEMI) or non-STEMI (NSTEMI). We assessed the predictive validities of 30-day bleeding events in various scoring systems using receiver operating characteristic curves.Overall, ACUITY-HORIZONS exhibited the highest area under the curve to predict 30-day bleeding, followed by ACTION and CRUSADE; HAS-BLED displayed the lowest score. With a cut-off of 17, ACUITY-HORIZONS demonstrated the best discrimination for the Thrombolysis in Myocardial Infarction (TIMI) 30-day serious bleeding rate. We observed significant differences among all-cause death, cardiovascular death, and major adverse cardiac events between the ACUITY-HORIZONS groups with a score of ≤ 17 and > 17. ACUITY-HORIZONS score > 17, initial systolic blood pressure (SBP) < 90 mmHg, and Killip III and IV upon admission positively predicted the 30-day bleeding risk, whereas myocardial infarction (MI) and TIMI major bleeding within 30 days, heart failure at admission, and initial SBP < 90 mmHg positively predicted the 30-day mortality.Comparatively, ACUITY-HORIZON is the most reliable system in predicting 30-day bleeding for patients with AMI via transradial PCI. In the transradial scenario, bleeding and MI within 30 days are substantially related to 30-day mortality.

Neural networks have garnered attention over the past few years. A neural network is a typical model of machine learning that is used to identify visual patterns. Neural networks are used to solve a wide variety of problems, including image recognition problems and time series prediction problems. In addition, neural networks have been applied to medicine over the past few years. This paper classifies the ways in which neural networks have been applied to medicine based on the type of data used to train those networks. Applications of neural networks to medicine can be categorized two types: automated diagnosis and physician aids. Considering the number of patients per physician, neural networks could be used to diagnose diseases related to the vascular system, heart, brain, spinal column, head, neck, and tumors/cancer in three fields: vascular and interventional radiology, interventional cardiology, and neuroradiology. Lastly, this paper also considers areas of medicine where neural networks can be effectively applied in the future.

A combination of horizontal expansion and vertical stacking of optical Banyan (HVOB) is the general architecture for building Banyan-based optical cross-connects (OXCs), and the intrinsic crosstalk problem of optical signals is a major constraint in designing OXCs. In this paper, we analyze the blocking behavior of HVOB networks and develop the lower bound on blocking probability of a HVOB network that is free of first-order crosstalk in switching elements. The proposed lower-bound is significant because it provides network designers an effective tool to estimate the minimum blocking probability they can expect from a HVOB architecture regardless what kind of routing strategy to be adopted. Our lower bound can accurately depict the overall blocking behavior in terms of the minimum blocking probability in a HVOB network, as verified by extensive simulation based on a network simulator with both random routing and packing routing strategies. Surprisingly, the simulated and theoretical results show that our lower bound can be used to efficiently estimate the blocking probability of HVOB networks applying packing strategy. Thus, our analytical model can guide network designers to find the tradeoff among the number of planes (stacked copies), the number of SEs, the number of stages and blocking probability in a HVOB network applying packing strategy.
