George MacDonald (1824-1905), Scottish author, published his first three short fantasy works for children in 1864. The Shadows is one of them. The shadow represents a human soul. On his deathbed, Ralph, the main character, sees a vision, and he is chosen as the King of the fairyland by the Shadows, then he is carried, on the shoulders of Shadows, to the fairyland or the border of the afterworld. Ralph sees a group of mysterious Shadows there. Although The Shadows consists of very complicated symbolical scenes, images, and words, my close analysis of the workings of Shadows and the visions of Ralph gives the following conclusion: The Shadows is a declaration of MacDonald's idea of salvation(Universalism), because all Shadows try to lead not only Ralph but also all people to salvation in the story.

Atypical polypoid adenomyoma (APAM) is an endometrial tumor that arises from the uterine corpus or cervix in premenopausal woman. Although APAM is pathologically benign, it should be distinguished from atypical endometrial hyperplasia and complex invasive endometrial adenocarcinoma (G1) because these conditions are similar and often coexist. We report a patient with APAM treated with hysteroscopic transcervical resection (TCR). The 28-year-old nullipara patient had hypermenorrhea for 1 year and genital bleeding for 3 months. Ultrasound and MRI revealed a 27 mm×12 mm tumor in the uterine cavity, which was considered to be an endometrial polyp or submucosal myoma. TCR was performed after GnRHa administration for 3 months. Pathological diagnosis showed APAM. Dilatation and curettage was performed 1 month after surgery, showing several residual atypical glands of APAM. The patient was followed every 3 months, and no recurrence of APAM has been detected for 1.5 years. Although clinical management of APAM has not been established, TCR is a reliable procedure for conservative treatment under careful follow-up.

<B><緒言></B>中濃厚生病院救命救急センターは、2000年8月から約6年、岐阜県の狭義の中濃地域(関市、美濃市)で、救急診療を行っている。今回、当センターとして、岐阜県中濃地域の救急医療体制の検証を行ったので報告する。<BR><B><検証結果></B><BR> 1.当センターの現状:年間の総救急患者数は約2万人で、重症度別にみると、一次が約90%と多い。救急車搬送による救急患者は、開設後約2年間は、一次が60から70%と多かったが、その後の約4年は一次が約50%となり、それに伴い急性心筋梗塞及び脳卒中等の重症救急患者の中濃医療圏の他の病院からの紹介、転送も増えてきている。ときどき生じる問題は、(1)約1時間に、三つの消防組合からあわせて4から5例の救急車搬送の要請があることにより初療室が混雑したこと、(2)中濃消防組合から約1時間で心肺機能停止状態(CPA)2例、重症外傷1例症・中等症外傷3例のホットラインが4回あり、大混雑の中で、診療を行なったこと、(3)夜間の中濃消防組合からの救急車搬送が、他の三つの二次病院ではなく、ほとんどが当センターであったこと等である。中濃地域の救急の協議会等で病診連携や二次病院の救急診療の役割分担を要望しているものの、未だ改善されてはいない。今後とも、行政の協力も求めながら、システムの改善を目指している。また、夜間、休日に直接来院する軽症患者が多いため、救命救急センターの利用法についてという掲示を出して、軽症例については、開業医の受診を奨めている。平日夜間については、少し受診患者が減少した。しかしながら、休日の午前中は多くの小児患者が来院している。このことについても今後改善されるよう模索している。<BR> 2.メディカルコントロール:オフラインメディカルコントロールとして、(1)中濃消防組合の救命救急士に対する包括的指示下の除細動のトレーニングは、プレコース、本コースあわせて8時間行った。(2)中濃消防組合の救命救急士でない一般の救急隊員約120名には一次救急処置(BLS)、自動体外式除細動(AED)のトレーニングを1)に準じ計8時間のトレーニングを行った。(3)気管内挿管の研修を2005年2名の救命救急士、2006年3名の救急救命士に行った。いずれも消防組合からの評価は高かった。
