It is not uncommon for otolaryngologists to encounter laryngeal edema due to infection in routine practice. However, we rarely encounter laryngeal edema caused by allergic reactions to orally ingested food.We herein report two cases of oral allergy syndrome after consumption of manuka honey candy. Both cases developed laryngeal edema and subsequent steroid administration led to the prompt improvement of symptoms. A basophil activation test for manuka honey candy was negative in both cases.

外リンパ瘻はあぶみ骨直達外傷や鼻かみ等により外リンパ腔と中耳や頭蓋内の間に瘻孔が生じ,めまいや難聴をきたす疾患である.我々は当院を受診した急性感音難聴症例,くり返す難聴・めまい症例全例,計121例125耳にcochlin-tomoprotein(CTP)検査を行い,CTP陽性例がどの程度存在するか検討した.全症例に詳細な問診を行って,診断基準に従い外リンパ瘻を起こす誘因のある症例を外リンパ瘻疑い,誘因の無い症例は特発性症例とした.CTP陽性症例は全125耳中,外リンパ瘻疑い27耳中の4耳のみで,外傷によるカテゴリー1の3耳と内因性圧外傷によるカテゴリー3の1耳であった.特発性症例98耳は全て陰性であった.高齢者は蝸牛窓膜などの内耳構造が脆弱であるとの報告もあるが,60歳以上の高齢者の特発性例51耳は中間値の1耳を除き,全て陰性であった.以上より,急性感音難聴やめまい症例の中で外リンパ瘻を疑う明らかな誘因のない特発性症例では外リンパ瘻の可能性は低いと考えられた.

<p>Herein, we report a case of isolated oculomotor nerve paralysis caused by a sphenoid sinus cyst. An 80-year-old man presented with right-sided ptosis, diplopia and occipital headache. He had unilateral ptosis and disturbance of the upward, downward and inward ocular movements, however, ophthalmologic examination showed neither visual impairment, nor pupillary dilatation. The symptoms were clearly caused by isolated right oculomotor nerve paralysis. Computed tomography (CT) and magnetic resonance imaging (MRI) revealed a lateral sphenoid sinus lesion with a smooth margin compressing the right cavernous sinus and shaving down the surface of the clivus. Based on a diagnosis of sphenoid sinus cyst, we performed endoscopic surgery under general anesthesia. The ocular movements became normal approximately 2 months after the operation and the right-sided ptosis disappeared within 6 months of the sugery. Since oculomotor nerve paralysis due to sphenoid sinus cyst is highly amenable to cure by endoscopic surgery, operation should be aggressively pursued for its treatment.</p>

Chronic paranasal sinusitis encompasses a variety of pathologies, and eosinophil infiltration of nasal polyps is characteristic of chronic sinusitis in which allergy is a strong etiologic factor (allergic paranasal sinusitis). Suplatast tosilate is an anti-allergy agent that suppresses production of IgE antibody and inhibits tissue infiltration by eosinophils. In the present study we assessed the efficacy of suplatast tosilate immunohistochemically in surgical specimens of nasal polyps before and after drug therapy to evaluate its efficacy in allergic sinusitis.The subjects were 8 patients with allergic sinusitis associated with nasal polyps. These patients were treated with suplatast tosilate for about three months. Nasal polyps were collected before and after treatment, and 5-μm frozen serial sections were prepared and stained with HE and by immunochemical methods. The number of eosinophils and eosinophil activation were assessed by using EG2 antibody, which recognizes secretory type eosinophilic cationic protein (ECP) alone, as the primary antibody, to investigate eosinophil activation.Infiltration by numerous eosinophils, predominantly in the superficial layer of the lamina propria, was observed in the nasal polyps of allergic rhinitis patients. Significant decreases in the number of eosinophils and the number of EG2-positive cells were observed in the nasal polyps after treatment with suplatast tosilate, and a significant decrease was also observed in the ratio of the number of EG2-positive cells to the number of eosinophils. Thus, suplatast tosilate was shown to have eosinophil tissue infiltration suppressing activity in human nasal polyps, and to also possess inhibitory activity against eosinophil degranulation. Based on these findings, suplatast tosilate suppresses allergic inflammation, which is predominantly manifested by eosinophil infiltration, and was found to be effective in the treatment of allergic paranasal sinusitis.
