We report the clinical study of 9 Judo players who had painful finger joints and complaints of difficulty in performing. There were 18 joints in all 16 of which (88.9%) had a history of trauma (ex. sprain and collateral ligament injury), but no history of fractures or dislocations. Instability of the joints was not found on clinical examination. Almost all players complained of a limitation in ROM and pain when they gripped a rival's sleeve with finger full flexion. X-ray examination of the finger joints revealed osteoarthritic changes in 15 joints (83.3%) and fragments in 7 joints (38.9%). No statistical differences were found in the techniques and experience between the 9 players and the other 11 players who had no finger complaints. The 2 joints with fragments had no history of trauma, therefore suggesting that they developed due to the distinctive overuse reaction in Judo.

慢性呼吸器疾患患者の入浴に関する報告は慢性閉塞性肺疾患(COPD)を対象としたものが多く症例数も少ない.本研究の目的はCOPDを含む慢性呼吸器疾患患者の入浴中の経皮的動脈血酸素飽和度(SpO2)の変動を後方視的に調査し負担のかかりやすい動作を明らにすることで効率的な動作指導を検討することである.作業療法士が入浴評価を実施した61名を対象に入浴を構成する各動作項目(脱衣,洗体,洗髪,体拭き,着衣)後のSpO2値を調査した.加えて入浴評価と6分間歩行試験(6MWT)を同じ酸素量で実施した25名を対象に入浴時と6MWT時のSpO2最低値を比較した.入浴時のSpO2値は体拭きで最低値を示し,約35%の患者は6MWTのSpO2最低値を下回った.入浴時は体拭きでSpO2が低下しやすいことを考慮し指導を行う必要がある.また労作時の酸素流量設定は6MWTに加え入浴評価も実施した上で決定することが望ましい.

職業性腰痛の疫学を各職種に従事している2,778名を対象にアンケート調査を行い,特に職種と腰痛の関係について検討した.アンケートの結果から腰痛歴の既往を約半数に認め,職場での発症が最も多かった.現在の腰痛に関しては,運輸職,看護職で腰痛との因果関係があると回答した者が多かった.発症状況では,徐々に発症したものは看護職や事務職に多く,急に発症したものは保安職や運輸職に多かった.腰痛発症の要因として,特に中腰作業,運転作業や重量物の取り扱いや介護作業が考えられた.腰痛発症と従事年数との関係では,看護職は初年度から腰痛の発生が高く,また業務との因果関係がありとの回答が多かった.この結果から,特に作業姿勢や作業関係などに関する指導や腰痛の予防に対する啓発を行う必要がある.

Middle aged athletes in Japan were studied to evaluate changes in their hematological indices. Ninety-two male and twelve female athletes (average age, male: 53.3 years, female 43.8 years) were examined for blood characteristics at the world veterans championship athletic meeting. Sixty-five male and nine female marathon runners all showed elevation of creatine kinase (CK) and HDL-Cholesterol before competition (CK, male: 220.3IU/L, female: 135.3IU/L, HDL-Cholesterol, male: 80.7mg/dl, female: 83.5mg/dl). Twenty-three of twenty-five male marathon runners showed a significant increase of CK (580IU/L), LDH (626.4IU/L), WBC (10940.9/mm) and GOT (38.5IU/L) after the competition. In conclusion, it was shown that long distance training in middle aged athletes caused elevation of HDL-Cholesterol and CK.
