[Purpose] This study assessed the relationships between peak toe flexor muscle strength, ankle dorsiflexion range of motion, and countermovement jump height. [Subjects and Methods] Eighteen healthy volunteers participated in the study. Each participant completed tests for peak toe flexor muscle strength, ankle dorsiflexion range of motion, and countermovement jump height. [Results] The results showed (1) a moderate correlation between ankle dorsiflexion range of motion and countermovement jump height and (2) a high correlation between peak first toe flexor muscle strength and countermovement jump height. Peak first toe flexor muscle strength and ankle dorsiflexion range of motion are the main contributors to countermovement jump performance. [Conclusion] These findings indicate that the measurement of peak first toe flexor muscle strength and ankle dorsiflexion range of motion may be useful in clinical practice for improving jump performance in athletes training for sports such as volleyball and basketball.

[Purpose] The purpose of this study was to identify the effects of extracorporeal shock wave therapy on the pain and function of patients with degenerative knee arthritis. [Subjects and Methods] Twenty patients with degenerative knee arthritis were divided into a conservative physical therapy group (n=10) and an extracorporeal shock wave therapy group (n=10). Both groups received general conservative physical therapy, and the extracorporeal shock wave therapy was additionally treated with extracorporeal shock wave therapy after receiving conservative physical therapy. Both groups were treated three times a week over a four-week period. The visual analogue scale was used to evaluate pain in the knee joints of the subjects, and the Korean Western Ontario and McMaster Universities Osteoarthritis Index was used to evaluate the function of the subjects. [Results] The comparison of the visual analogue scale and Korean Western Ontario and McMaster Universities Osteoarthritis Index scores within each group before and after the treatment showed statistically significant declines in scores in both the conservative physical therapy group and extracorporeal shock wave therapy group. A group comparison after the treatment showed statistically significant differences in these scores in the extracorporeal shock wave therapy group and the conservative physical therapy group. [Conclusion] extracorporeal shock wave therapy may be a useful nonsurgical intervention for reducing the pain of patients with degenerative knee arthritis and improving these patients’ function.

[Purpose] The aim of this study was to investigate the relationship between physical capacity and fear avoidance beliefs in patients with chronic low back pain. [Subjects and Methods] This cross sectional study included 131 male university students with chronic low back pain. All the patients completed a fear avoidance beliefs questionnaire. Each participant performed a physical capacity test, which included hand grip force, leg strength, abdominal muscle endurance, flexibility, and cardiopulmonary endurance testing. [Results] Negative correlation was observed between physical capacity (leg strength, abdominal muscle endurance) and fear avoidance beliefs regarding work. Physical capacity (hand grip force, leg strength, cardiopulmonary endurance) showed a negative correlation with fear avoidance beliefs about physical activity. Abdominal muscle endurance and cardiopulmonary endurance were predictors of fear avoidance beliefs. [Conclusion] Physical capacity showed a negative correlation with fear avoidance beliefs in patients with chronic low back pain. The results of this study suggest that physical capacity is an important factor for predicting fear avoidance beliefs in patients with chronic low back pain.

[Purpose] To verify the immediate effects of exercise therapy on cancer-related fatigue (CRF) in cancer patients. [Subjects and Methods] Eighteen cancer patients who performed exercise therapy targeting a rating of 4 (somewhat strong) on the Borg category-ratio scale (CR-10) were enrolled. CRF was evaluated using the Cancer Fatigue Scale (CFS). CFS was evaluated in clinical practice immediately before and after exercise therapy on the 1st or 2nd day of physiotherapy for CRF management. CFS scores before and after exercise were compared to determine how CRF changed due to exercise therapy. [Results] CFS physical, CFS affective, CFS cognitive, and CFS total all decreased following exercise therapy, and the changes in CFS physical and CFS total were statistically significant. The effect sizes for CFS physical and CFS total were “medium”, and for CFS affective and CFS cognitive “small.” [Conclusion] These findings suggest that exercise therapy targeting a rating of 4 (somewhat strong) on the CR-10 can immediately reduce CRF in cancer patients.

[Purpose] It is clinically important to accurately evaluate the alignment of hindfoot alignment from the body surface in weight-bearing position. In general, measurement is carried out using a goniometer, but the measurement with goniometer requires experience, and the reliability in this measurement is also in question. Therefore, it is necessary to establish a clinical measurement method to replace the goniometer. As a solution, we devised a hindfoot alignment measurement method with laser beam (HAML), which measures hindfoot alignment along the body surface using a laser beam. This study aimed to study the correlation between the measurement result along the body surface with laser or goniometer and that of radiography with regard to hindfoot alignment. [Subjects and Methods] Subjects included 23 individuals with knee osteoarthritis. Their hindfoot alignment using radiography and goniometry were measured. The correlation between the measurement result along the body surface with or without laser and radiography was investigated. [Results] The correlation between HAML and radiography was better than that between HAML and goniometry. [Conclusion] The correlation between radiography and HAML for measuring hindfoot alignment was good. This finding implies that HAML can be used as a screening test or epidemiological survey in regional medicine.

[Purpose] The purpose of this study was to investigate whether transcutaneous electrical nerve stimulation simultaneously combined with local heat and cold applications enhances pain relief compared with transcutaneous electrical nerve stimulation alone in patients with knee osteoarthritis. [Subjects and Methods] Fourty-five patients with knee osteoarthritis participated in this study. They were randomly assigned to the following three interventions: transcutaneous electrical nerve stimulation simultaneously combined with local heat using a hot pack; combined with local cold using a cold pack; and transcutaneous electrical nerve stimulation alone. In each intervention, the knee pain level during walking and standing up from a chair, as well as dynamic balance and gait ability were evaluated immediately before and after a single intervention using the visual analogue scale and the timed up & go test, respectively. [Results] A significant improvement in dynamic balance and gait ability was only observed immediately after transcutaneous electrical nerve stimulation simultaneously combined with local heat application, although the degree of pain relief during standing and walking were comparable among the three interventions. [Conclusion] These results suggest that transcutaneous electrical nerve stimulation simultaneously combined with local heat application can immediately improve not only knee pain during standing and walking but also dynamic balance and gait ability in patients with knee osteoarthritis.

[Purpose] The aim of this study was to examine the effects of balance training and muscle training around the ankle joints on the gait of elderly people who have experienced a fall. [Subjects] Twenty-six elderly people with a risk of falling and a Berg Balance Scale score of 37 to 50 points who had experienced a fall in the last year were randomly and equally assigned to either a balance training group or an ankle training group. The balance training group received training on a hard floor, training while maintaining balance on a cushion ball in a standing position, and training while maintaining balance on an unstable platform in a standing position; the ankle training group received training to strengthen the muscles around the ankle joints and conducted stretch exercise for the muscles around the ankle joints. [Results] There were significant changes in gait velocity, step length, and stride length in the balance training group after the intervention; there were significant changes in gait velocity, cadence, step time, cycle time, step length, and stride length in the ankle training group after the intervention. In a between-group comparison, the gait velocity of the balance training group showed a significant improvement compared with the ankle training group. [Conclusion] Both balance training and ankle joint training are effective in enhancing the gait ability of elderly people with a risk of falling; in particular, balance training is effective in improving the gait velocity of elderly people who have experienced a fall compared with ankle joint training.

[Purpose] The purpose of this study was to examine the effects of interferential current therapy on the shoulder muscles of subjects with forward head posture. [Subjects and Methods] Thirty volunteers (15 forward head posture, 15 control) participated in this study. Interferential current therapy treatment was conducted for 10 min, three times per week, for 3 weeks on both the upper trapezius and levator scapulae. Subjects were measured anterior both acromion distance and posterior both acromial distance using a ruler, and the forward head posture angle using an electronic application. [Results] Both groups showed significant increases in anterior acromial distance, and decreases in posterior acromial distance and forward head posture angle. [Conclusion] Subjects were concluded that electrical therapy could improve structural alignment; therefore, the implementation of interferential current therapy is expected to be effective for the treatment of forward head posture.

[Purpose] This study was conducted to identify and analyze the degree of effect of massage therapy on the range of motion of the shoulder. [Subjects and Methods] The database search was conducted using PubMed, CINAHL, Embase, PsycINFO, RISS, NDSL, NANET, DBpia, and KoreaMed. The meta-analysis was based on 7 studies, covered a total of 237 participants, and used a random-effects model. [Results] The effect size estimate showed that massage therapy significantly improved the shoulder range of motion, especially the flexion (SMD: 18.21, 95% CI 1.57–34.85) and abduction (SMD: 22.07, 95% CI 5.84–38.30). [Conclusion] The review findings suggest that massage therapy is effective in improving the shoulder flexion and abduction.

[Purpose] Sarcopenia may be associated with malnutrition in patients with vertebral compression fractures which may affect a patient’s functional prognosis. This study investigated the association between sarcopenia, malnutrition, and activities of daily living at the time of hospital discharge in patients with vertebral compression fractures. [Participants and Methods] The study included 36 patients who were hospitalized with vertebral compression fractures. Sarcopenia was assessed by measuring grip strength and calf circumference. The nutritional status was assessed at the time of hospital admission and at discharge using the Mini Nutritional Assessment Short Form screening tool. Activities of daily living were assessed using the Barthel Index. [Results] The prevalence of sarcopenia at the time of admission was 47.2%. The Barthel Index and Mini Nutritional Assessment Short Form scores in patients with sarcopenia at the time of admission were significantly lower at discharge than to those in patients without sarcopenia. Overall, at discharge, weight and calf circumference decreased significantly with a consequent increase in the prevalence of sarcopenia (55.6%). Multivariate analysis showed that the Mini Nutritional Assessment Short Form score, calf circumference loss, and age affected the Barthel Index at discharge. [Conclusion] Patients with vertebral compression fractures often show sarcopenia and malnutrition, which are conditions that may be exacerbated during hospitalization. These conditions can subsequently affect a patient’s activities of daily living; thus, nutritional rehabilitation is important in patients with vertebral compression fractures, as demonstrated in this study.

[Purpose] To investigate the effects of pre-induced muscle damage vibration stimulation on the pressure-pain threshold and muscle-fatigue-related metabolites of exercise-induced muscle damage. [Subjects and Methods] Thirty healthy, adult male subjects were randomly assigned to the pre-induced muscle damage vibration stimulation group, post-induced muscle damage vibration stimulation group, or control group (n=10 per group). To investigate the effects of pre-induced muscle damage vibration stimulation, changes in the pressure-pain threshold (lb), creatine kinase level (U/L), and lactate dehydrogenase level (U/L) were measured and analyzed at baseline and at 24 hours, 48 hours, and 72 hours after exercise. [Results] The pressure-pain thresholds and concentrations of creatine kinase and lactate dehydrogenase varied significantly in each group and during each measurement period. There were interactions between the measurement periods and groups, and results of the post-hoc test showed that the pre-induced muscle damage vibration stimulation group had the highest efficacy among the groups. [Conclusion] Pre-induced muscle damage vibration stimulation is more effective than post-induced muscle damage vibration stimulation for preventing muscle damage.

[Purpose] The purpose of this study was to examine the effects of extracorporeal shock wave therapy (ESWT) on upper extremity muscle tone in chronic stroke patients. [Subjects and Methods] For this study, 30 stroke patients participated in this study and they were divided into ESWT group and sham-ESWT group, each group consisted of 15 patients. ESWT and sham-ESWT was performed by the patients for two times a week, for eight weeks. MyotonPRO was used to measure muscle tone. [Results] According to the results of the comparisons between the groups, after intervention, upper extremity muscle tone were significantly higher in the ESWT group than in the sham-ESWT group. [Conclusion] This study showed that ESWT is effective for improving decrease of muscle tone in chronic stroke patients.

[Purpose] This study evaluated the immediate and short-term effects of a combination of prolonged passive muscle stretching (PMS) and whole body vibration (WBV) on the spasticity, strength and balance of children and adolescents with cerebral palsy. [Subjects and Methods] A randomized two-period crossover trial was designed. Twelve subjects with cerebral palsy aged 10.6 ± 2.4 years received both PMS alone as a control group (CG) and a combination of PMS and WBV as an experimental group (EG). After random allocation to the trial schedules of either EG-CG or CG-EG, CG received prolonged PMS while standing on a tilt-table for 40 minutes/day, and EG received prolonged PMS for 30 minutes, followed by 10 minutes WBV. Both CG and EG received the treatment 5 days/week for 6 weeks. [Results] Immediately after one treatment, EG resulted in better improvement in scores on the Modified Ashworth Scale than CG. After the 6-week intervention, EG also showed significantly decreased scores on the Modified Ashworth Scale compared to CG. Both CG and EG showed significantly reduced the performance times in the five times sit to stand test, and EG also showed significantly increased scores on the pediatric balance scale. [Conclusion] This study showed that 6 weeks of combined prolonged PMS and WBV had beneficial effects on the spasticity, muscle strength and balance of children and adolescents with CP.

[Purpose] Counteracting the systemic cytokine release and its inflammatory effects by improving respiratory muscle strength and controlling lung inflammation may be important for improving immune system in patients with chronic obstructive pulmonary disease, So the aim of the present study was to evaluate the effect of low level laser therapy and inspiratory muscle training on interleukin-6 (IL-6) as a marker of inflammation and CD4+/CD8+ ratio as a marker for T Lymphocytes in these patients. [Subjects and Methods] Forty male patients with stable COPD participated in the study, their ages ranged between 55−65 years. They were randomly divided into group (A) who received inspiratory muscle training and group (B) who received low level laser (LLL) acupuncture stimulation for about 8 week. [Results] There was a reduction in the concentration of plasma IL-6 associated with an increase in CD4+/CD8+ ratio in both groups, but laser was superior to inspiratory muscle training. IL-6 and CD4+/CD8+ were negatively correlated. [Conclusion] Both inspiratory muscle training and low level laser therapy are effective physical therapy modalities in promoting immune disturbances. The results also supported the superior role of LLLT over IMT in managing immune disturbances.

[Purpose] The purpose of this study was to determine the effect of whole-body vibration exercise in a sitting position prior to therapy in stroke patients. [Subjects and Methods] Fourteen chronic stroke patients were included in this study. Prior to occupational therapy, whole-body exercise was performed for 10 minutes, 5 times per week, for a total of 8 weeks. Muscle tone and upper extremity function were measured. The Modified Ashworth Scale (MAS) was used to measure muscle tone, and the Manual Function Test (MFT) and Fugl-Meyer Assessment scale (FugM) were used to measure upper extremity function. [Results] MAS score was significantly decreased, and MFT and FugM were significantly increased. [Conclusion] These results indicate that whole-body vibration exercise in a sitting position prior to therapy had a positive effect on muscle tone, and upper extremity function in stroke patients.

[Purpose] To investigate the prevalence of neck and shoulder symptoms during the use of tablet computer, and to identify the risk factors associated with these symptoms. [Subjects and Methods] A cross-sectional survey was conducted to study tablet computer usage, posture during use, and neck and shoulder symptoms in 412 participants in a school setting. Significant risk factors for musculoskeletal symptoms during tablet computer use were identified. [Results] Overall prevalence of musculoskeletal symptoms during tablet computer use was 67.9% with greater prevalence of neck symptoms (neck: 84.6%; shoulder/upper extremity: 65.4%). Significant risk factors associated with symptoms during use were: current musculoskeletal symptoms, gender, roles, and postural factors including: sitting without back support, sitting with device in lap, and lying on the side and on the back during tablet computer use. A multivariate analysis further showed that the odds for females to have symptoms were 2.059 times higher than males. [Conclusion] The findings revealed that female gender and other postural factors were significantly associated with musculoskeletal symptoms during the use of tablet computer. Among all postural factors, sitting without back support was identified as the most important risk factor for having musculoskeletal symptoms.

[Purpose] To investigate the inter-rater and test-retest reliability of the sitting-rising test (SRT), the correlations of sitting-rising test scores with measures of strength, balance, community integration and quality of life, as well as the cut-off score which best discriminates people with chronic stroke from healthy older adults were investigated. [Subjects and Methods] Subjects with chronic stroke (n=30) and healthy older adults (n=30) were recruited. The study had a cross-sectional design, and was carried out in a university rehabilitation laboratory. Sitting-rising test performance was scored on two occasions. Other measurements included ankle dorsiflexor and plantarflexor strength, the Fugl-Meyer assessment, the Berg Balance Scale, the timed up and go test, the five times sit-to-stand test, the limits of stability test, and measures of quality of health and community integration. [Results] Sitting-rising test scores demonstrated good to excellent inter-rater and test-retest reliabilities (ICC=0.679 to 0.967). Sitting-rising test scores correlated significantly with ankle strength, but not with other test results. The sitting-rising test showed good sensitivity and specificity. A cut-off score of 7.8 best distinguished healthy older adults from stroke subjects. [Conclusions] The sitting-rising test is a reliable and sensitive test for assessing the quality of sitting and rising movements. Further studies with a larger sample are required to investigate the test’s validity.

[Purpose] There is a lack of information evaluating specific markers of performance in patients awaiting bariatric surgery. We aimed to assess the postural control, functional performance, strength and endurance performance for morbidly obese patients awaiting bariatric surgery compared to lean controls. [Subjects and Methods] All parameters were assessed by modified Y-balance test, timed-up-and-go-test, maximum strength testing on resistance exercise equipment and cardio-pulmonary exercise testing on a cycle ergometer in 10 morbidly obese patients awaiting bariatric surgery and 10 age- and sex-matched lean controls. [Results] It was found that significant differences existed for overall modified Y-balance test in morbidly obese patients awaiting bariatric surgery versus lean controls (0.37 ± 0.03 vs. 0.47 ± 0.02 cm.cm−1), timed-up-and-go-test (9.33 ± 1.23 vs. 7.85 ± 1.73 sec) and several variables of cardio-pulmonary exercise testing. Overall absolute strength expressed in kilogram was similar, yet when relativized to body weight strength differences were notable (0.4 ± 0.17 vs. 0.83 ± 0.32 kg.kg−1). [Conclusion] The results of this study demonstrate the need for comprehensive functional assessment prior to surgery with an identified demand for subsequent tailored physical training prescription that should begin before surgery.

[Purpose] In a pilot investigation, extracorporeal shockwave therapy was applied to patients with idiopathic scoliosis. This case report aimed to describe the effects of repeated extracorporeal shockwave therapy on the clinical signs of a patient with scoliosis due to a functional tethered cord syndrome. [Subject and Methods] In June 2016, a 13-year-old girl presented with a left thoracic curvature. Radiograph showed a left thoracic curve of 24°. An angle of trunk rotation of 21° was measured using a scoliometer. Extracorporeal shockwave therapy was proposed to support conservative treatment. Five sessions of extracorporeal shockwave therapy were applied. Finger-floor distance and the angle of trunk rotation before and after each application of extracorporeal shockwave therapy were measured. [Results] The average finger-floor distances before and after extracorporeal shockwave therapy were 22.6 cm and 15.6 cm, respectively. The average angles of trunk rotation before and after the therapy were 13° and 10.2°, respectively. [Conclusion] The short-term effect of extracorporeal shockwave therapy was revealed in this study. The neural structures that inhibited free mobility of the spine were mobilized. Furthermore, deformity was reduced by applying extracorporeal shockwave therapy.

[Purpose] Bicycle saddle height is a critical factor for cycling performance and injury prevention. The present study compared the variance in cadence frequency after exercise fatigue between saddle heights with 25° and 35° knee flexion. [Methods] Two saddle heights, which were determined by setting the pedal at the bottom dead point with 35° and 25° knee flexion, were used for testing. The relative variances of the cadence frequency were calculated at the end of a 5-minute warm-up period and 5 minutes after inducing exercise fatigue. Comparison of the absolute values of the cadence frequency under the two saddle heights revealed a difference in pedaling efficiency. [Results] Five minutes after inducing exercise fatigue, the relative variances of the cadence frequency for the saddle height with 35° knee flexion was higher than that for the saddle height with 25° knee flexion. [Conclusion] The current finding demonstrated that a saddle height with 25° knee flexion is more appropriate for cyclists than a saddle height with 35° knee flexion.
