[Purpose] This study aimed to investigate the perceived discomfort and trunk muscle activity in three different 1-hour sitting postures. [Subjects] A repeated-measures design study was conducted on 10 healthy subjects. [Methods] Each subject sat for an hour in three sitting postures (i.e., upright, slumped, and forward leaning sitting postures). Subjects rated perceived body discomfort using Borg’s CR-10 scale at the beginning and after 1 hour sitting. The electromyographic activity of the trunk muscle activity was recorded during the 1-hour period of sitting. [Results] The forward leaning sitting posture led to higher Borg scores in the low back than those in the upright (p = 0.002) and slumped sitting postures (p < 0.001). The forward leaning posture was significantly associated with increased iliocostalis lumborum pars thoracis (ICL) and superficial lumbar multifidus (MF) muscle activity compared with the upright and slumped sitting postures. The upright sitting posture was significantly associated with increased internal oblique (IO)/transversus abdominis (TrA) and ICL muscle activity compared with the slumped sitting posture. [Conclusion] The sitting posture with the highest low back discomfort after prolonged sitting was the forward leaning posture. Sitting in an upright posture is recommended because it increases IO/TrA muscle activation and induces only relatively moderate ICL and MF muscle activation.

[Purpose] To evaluate the relationship between stride length during a pitch and maximal ball velocity and define the determinants of stride length in collegiate baseball pitchers. [Participants and Methods] Eighteen collegiate baseball pitchers participated in this study. Using a standard tape measure, the distance between each participant’s legs was measured after maximal abduction of both hip joints in a standing position. The pitchers threw fastballs on an indoor pitching mound at an official pitching distance. The throwing motion was recorded with four high-speed video cameras (240 Hz), while the ball velocity was measured using a radar gun. The stride length at foot contact was calculated at the maximal ball velocity using a three-dimensional motion analysis system. [Results] The maximal ball velocity (mean 129.8 km/h) had a significant positive correlation with the absolute stride length during a pitch (r=0.55), but no correlation with the relative stride length (% body height, % lower extremity length, and % maximal open legs’ width). Stride length during a pitch did not significantly correlate with the pitcher’s body height, lower extremity length, and maximal open legs’ width. [Conclusion] A longer stride length contributes to a higher ball velocity in collegiate baseball pitchers.

[Purpose] The purpose of this study was to show the effects of wearing a tight skirt on the VMO, VL, and TFL muscles during walking. [Subjects] Twelve female subjects were recruited. [Methods] The muscle activities of the VMO, VL, and TFL muscles were measured using a surface EMG system during walking while wearing a typical skirt and tight skirt. [Results] The VL and TFL muscle activities significantly increased, and VMO muscle activity significantly decreased when wearing a tight skirt. [Conclusion] We thought that a tight skirt might limit the normal gait. The abnormal gait movements resulting from wearing a tight skirt could cause imbalance of the VMO and VL.

The purpose of this study was to determine the effect of ankle taping and the combined effect of ankle taping and exercise on ankle stabilization under four different conditions (pre-taped, taped, walking on the treadmill, removal of tape) in drop landing from a 40 cm height. The subjects consisted of 14 healthy adult (four male, ten female) volunteers with ages ranging from 22-28 years. Preamplified EMG electrodes were attached to the soleus, tibialis anterior, vastus medialis, and biceps femoris. EMG and force plate data were collected at 500 Hz. While standing on one leg, subjects hopped from a 40 cm height onto a force plate. The ankle of the right leg was then taped followed by a repeat of the hopping task, walking on a treadmill, and a second repetition of the hopping task. The hopping task was again repeated following the removal of the tape. Significant increases occurred in vertical ground reaction forces for the taped conditions compared with those in pretaped and untaped conditions. In the taped conditions, the soleus displayed a significant decrease in mean iEMG from impact to peak vertical ground reaction force (p<0.05). Results suggest that taping can provide enough mechanical support to limit ankle motion even after 30 minutes of walking on the treadmill. Additionally, diminished soleus iEMG activity may be related to an increase in the ground reaction forces upon landing from a jump.

[Purpose] The present study investigated the effect of an exercise program for posture correction on musculoskeletal pain. [Subjects] Between September 2, 2013 and November 3, 2013, an exercise program was performed in 88 students from S University in K city (male students, n = 34; female students, n = 54). [Methods] The exercise program for posture correction was performed for 20 minutes per session, 3 times a week for 8 weeks. Pain levels were measured using a pain scale, and pain levels before and after the exercise program were compared. [Results] Overall, pain levels of the participants were lower after the exercise program than before the program, and significant differences in pain levels were noted in the shoulders, middle back, and lower back. [Conclusion] In conclusion, shoulder pain, mid back pain, and low back pain were relieved with the exercise program for posture correction. Therefore, the findings of this study can be used to improve the work efficiency of students as well as people engaged in sedentary work.

[Purpose] This study aims to describe a protocol based on neurocognitive therapeutic exercises and determine its feasibility and usefulness for upper extremity functionality when compared with a conventional protocol. [Subjects and Methods] Eight subacute stroke patients were randomly assigned to a conventional (control group) or neurocognitive (experimental group) treatment protocol. Both lasted 30 minutes, 3 times a week for 10 weeks and assessments were blinded. Outcome measures included: Motor Evaluation Scale for Upper Extremity in Stroke Patients, Motricity Index, Revised Nottingham Sensory Assessment and Kinesthetic and Visual Imagery Questionnaire. Descriptive measures and nonparametric statistical tests were used for analysis. [Results] The results indicate a more favorable clinical progression in the neurocognitive group regarding upper extremity functional capacity with achievement of the minimal detectable change. The functionality results are related with improvements on muscle strength and sensory discrimination (tactile and kinesthetic). [Conclusion] Despite not showing significant group differences between pre and post-treatment, the neurocognitive approach could be a safe and useful strategy for recovering upper extremity movement following stroke, especially regarding affected hands, with better and longer lasting results. Although this work shows this protocol’s feasibility with the panel of scales proposed, larger studies are required to demonstrate its effectiveness.

[Purpose] External focus (EF) instructions demonstrate a learning effect on motor performance enhancement. However, the effective EF distance during standing long jump performance of non-athletes has not been clarified. This study aimed to determine the effects of EF at different distances on jumping performance. [Participants and Methods] A total of 40 non-athlete participants were randomly divided among four groups. The no attention line group performed a standing long jump without the attention line on the floor; those in the −20-cm EF group, the ± 0-cm EF group, and +20-cm EF group performed the jump attention line with an attention line 20-cm posterior, at ± 0 cm, and 20-cm anterior as the reference jump distances, respectively. [Results] The mean rate of increase between the first to second jump distances in the +20-cm EF group was higher than that in the no attention line group. The rates at which the jumpers reached the attention line in the ± 0-cm EF group and the +20-cm EF group were lower than the rate in the −20-cm EF group. [Conclusion] Instructions are more effective when the distance to the attention line exceeds jumping performance.

[Purpose] Specific exercises and brace treatment are the two evidence-based modes of treatment for patients with scoliosis. The purpose of this paper is to present the first end-results from a prospective cohort that commenced treatment in 2011 with a CAD based Chêneau derivate and is then compared to the published results achieved with the Boston Brace. [Participants and Methods] Inclusion criteria for the study, refers to the SRS inclusion criteria on bracing, except the range of Cobb angles which was extended to curvatures of up to 45°. Twenty-eight patients were weaned from their CAD Chêneau style brace. The results of this cohort have been compared with the BRAIST study by Weinstein et al. with the help of the Z-test. [Results] A success rate of 92.9% has been achieved. This was compared to the success rate of 72% in the BRAIST study. The differences were highly significant in the Z-test. [Conclusion] The results achieved with the GBW are significantly and better than the results achieved with the Boston brace. Therefore, the standards for bracing should be reviewed with the results that symmetric compression with Boston bracing is not as successful, when compared to asymmetric high correction bracing results, which allow a standardized classification-based corrective approach.

[Purpose] The purpose of the study was to determine the effects of a sciatic nerve mobilization technique on improvement of lower limb function in patient with poststroke hemiparesis. [Subjects] Twenty- two stroke patients participated in this study. [Methods] They were randomly selected based on selection criteria and divided into two groups. In the subject group (n=10), sciatic nerve mobilization with conventional physical therapy was applied to patients. In the control group (n=10), only conventional physical therapy was applied to stroke patients. [Results] There were significant differences between the two groups in pressure, sway, total pressure, angle of the knee joint, and functional reaching test results in the intervention at two weeks and at four weeks. [Conclusion] The present study showed that sciatic nerve mobilization with conventional physical therapy was more effective for lower limb function than conventional physical therapy alone in patient with poststroke hemiparesis.

[Purpose] The purposes of this study were to strengthen gluteus maximus and trunk muscles depending upon the hip joint direction by bridging exercise on an unstable and stable surface, and to suggest an intervention method for efficient and selective exercise. [Participants and Methods] The test measured the muscle activities with the external rotation and internal rotation of 25 degrees and hip joint neutral position of 0 degree on a stable and unstable surface each exercise 3 times for 9 seconds. [Results] External oblique abdominis showed a higher muscle activity on the unstable surface and internal rotation. Erector spinae showed no significant difference on the surfaces and demonstrated the highest muscle activity at the internal rotation. Gluteus maximus showed a higher muscle activity on the stable surface and external rotation and the interaction effect between the surface and the angle indicated a statistical significance as well. [Conclusion] Muscle activities appear different depending upon the change of surface and joint angle, and it can be said that the correct mobilization of muscle fiber relying on the muscle arrangement direction and muscle contraction direction is the most important factor for gluteus maximus.

[Purpose] The purpose of this study was to investigate the effect of pelvic tilt angles and lung function in participants performing pelvic tilts on a ball. [Subjects and Methods] Eighteen subjects participated in this study. While they performed pelvic tilt on sitting at a ball, the peak expiratory flow (PEF) and forced expiratory volume in one second (FEV1) were measured at 10 degrees of anterior and posterior pelvic tilt, respectively, and neutral position. The repeated measure ANOVA was performed, and the Bonferroni correction was used for post-hoc analysis. [Results] The PEF of the participants was significantly higher at neutral position, compared with an anterior pelvic tilt at 10 degrees. The FEV1 was also higher in neutral position, compared with anterior and posterior pelvic tilt. [Conclusion] This study underlines the need for the standardization of the FVC testing protocol for positioning the pelvic angle in a neutral position in patients with respiratory disorders to promote reliable interpretation of intervention outcomes.

[Purpose] Limited literature has investigated the relationships between acceleration-based gait characteristics and kinematic information from motion analysis systems in gait analysis. The purpose of this study is to determine whether acceleration-based gait characteristics were associated with gait characteristics by motion analysis systems in patients with stroke. [Participants and Methods] Seventeen patients with stroke walked along a 10-m-long walkway at their comfortable speed. Trunk acceleration was measured with an accelerometer. Several reflective markers over bony landmarks on the lower extremities were used to capture movements. We evaluated the correlations of variables calculated between the trunk accelerometers and the motion analysis system. [Results] Walking speed was positively correlated with harmonic ratios along the anteroposterior axis and stride regularity along the vertical and anteroposterior axes. Harmonic ratios were associated with the stance phase percent on the unaffected side. Stride regularity was associated with the stance phase percent on both sides. Smaller interstride variability was associated with smaller peak ankle plantarflexion during both phases and greater peak ankle dorsiflexion during swing phase. Stride regularity is positively associated with maximal knee flexion during swing phase. [Conclusion] Relationships with spatiotemporal and joint kinematic parameters from the motion analysis system support the potential use of accelerometers.

[Purpose] This study aimed to examine the impact of changing the drop vertical jump stance time on kinematic and kinetic parameters by ordering to high jump or quick jump for consistent stance time and a more accurate assessment of anterior cruciate ligament injury risk. [Participants and Methods] The participants were 20 healthy female students. The drop vertical jump was started by instructing the participants to stand on a 30-cm platform with both legs stationary. The task was performed while the participants were instructed to perform high jump or quick jump. [Results] Stance time was significantly shorter with quick jump than with high jump. Quick jump showed significantly higher knee abduction angles at initial contact and peak vertical ground reaction force, and lower hip flexion, knee flexion, and ankle dorsiflexion angles at the lowest point of the center of mass. Quick jump showed a significantly higher peak vertical ground reaction force. The knee abduction moment at initial contact was not significantly different between the 2 conditions. [Conclusion] Quick jump was better than high jump for making stance time consistent, and the differences in kinematic and kinetic characteristics by oral instructions should be considered when using drop vertical jump.

[Purpose] The purpose of this study was to examine the effects of cervical traction treatment, cranial rhythmic impulse treatment, a manual therapy, and McKenzie exercise, a dynamic strengthening exercise, on patients who have the neck muscle stiffness of the infrequent episodic tension-type (IETTH) headache and frequent episodic tension-type headache(FETTH), as well as to provide the basic materials for clinical interventions. [Subjects] Twenty-seven subjects (males: 15, females: 12) who were diagnosed with IETTH and FETTH after treatment by a neurologist were divided into three groups: (a cervical traction group (CTG, n=9), a cranial rhythmic contractiongroup (CRIG, n=9), and a McKenzie exercise group (MEG, n=9). An intervention was conducted for each group and the differences in their degrees of neck pain and changes in muscle tone were observed. [Results] In the within-group comparison of each group, headache significantly decreased in CTG. According to the results of the analysis of the muscle tone of the upper trapezius, there was a statistically significant difference in MEG on the right side and in CRIG on the left side. According to the results of the analysis of the muscle tone of the sternocleidomastoid muscle, there was a statistically significant difference in MEG on the right side and in CRIG on the left side. [Conclusion] In the comparison of the splenius capitis muscle between the groups, there was a statistically significant difference on the right side. Hence, compared to the other methods, cervical traction is concluded to be more effective at reducing headaches in IETTH and FETTH patients.

[Purpose] This study examined the effectiveness of extracorporeal shock wave therapy versus ultrasound therapy, combined with the mobilization and therapeutic exercise in both groups, in participants with diabetic frozen shoulder. [Participants and Methods] Twenty participants with diabetic frozen shoulder were divided into an experimental group who received extracorporeal shock wave therapy, mobilization and exercises (n=10, Mean: 43.70) and the control group who received ultrasound, mobilization and exercises (n=10 Mean: 45.50). The clinical outcomes, i.e., a) pain b) active range of motions of the shoulder, c) disability scores by Disabilities of the Arm, Shoulder and Hand scale and d) global rating of change was measured weekly for four weeks. [Results] Significant improvements in pain, all active range of motions and disability scores were observed at the end of the 4th week in both groups. Additionally, the experimental group benefitted significant pain reduction (median difference: 7 in experimental versus 6 in control group), reduced number of therapy sessions and thus the costs of treatment compared to the control group. [Conclusion] Extracorporeal shock wave therapy significantly reduced pain in people with diabetic frozen shoulder with a reduction of treatment cost compared to the control group.

[Purpose] The aim of this study is to compare patients with failed back surgery syndrome (FBSS) with those with non-specific chronic back pain (NSCBP) in terms of pain, depression, and quality of life levels to explain the effects of surgery experience on low-back pain. [Subjects and Methods] A total of 50 patients with FBSS and 51 patients with NSCBP who consecutively applied to the outpatient clinic from September 2012 to April 2013 were included in the study. Patients completed questionnaires on demographics, body mass index, education level, work history, and pain duration (in months). Lumbar pain at rest, during movement, and at night were measured with a visual analogue scale (VAS). The Short Form 36 scale was used for evaluating quality of life, and the Beck Depression Inventory (BDI) was used for assessing depression. [Results] VAS scores reporting pain at rest and at night and BDI scores were significantly higher in the FBSS group than in the NSCBP group. Role limitations due to physical functioning, which is one of the measures of quality of life, were significantly higher in the FBSS group than in the NSCBP group. [Conclusion] These assessments show that surgery experience in patients with ongoing low-back pain makes their pain and depression worse.

[Purpose] This study evaluated balance tests in users of a day care service who needed nursing care or support, and investigated the usefulness of the Two-step Test for evaluating balance. [Subjects and Methods] The subjects were users of a day care service, and had certified need for long-term care or support. All subjects were able to undergo the balance evaluations. Balance tests included the 3-m Timed Up and Go test (TUG), the one-leg standing time, and the Two-step Test. [Results] The Two-step Test and other balance tests were strongly correlated. [Conclusion] In this study of subjects who needed nursing care or support, the results were the same as in a previous study of subjects who did not need nursing care or support. The Two-step Test should be considered as an indicator of balance ability in elderly individuals requiring nursing care or support.

[Purpose] The purpose of this study is to examine the effectiveness of a stroke-related scale with regard to outcome, onset, and timing of stroke patients. [Participants and Methods] The participants included 583 out of 996 patients who were admitted to the stroke care unit.The outcomes and 3 stroke scale (National Institutes of Health Stroke Scale: NIHSS, Functional Independence Measure: FIM, modified Rankin Scale: mRS) scores immediately at hospitalization, on day 7 after onset, and on day 30 after onset were investigated. This study was analyzed using a generalization linear model with a binomial distribution. The comparisons between outcomes were made in terms of home discharge versus convalescence, and convalescence versus hospital transfer. [Results] Comparisons of home discharge versus convalescence hospital transfer showed a significant difference in the NIHSS and mRS scores at the time of hospitalization, and a significant difference in the NIHSS scale score on day 7 after onset. In comparisons between convalescence and hospital transfer, significant differences were observed in NIHSS and FIM scores at hospitalization, and the FIM scale score showed significant differences on day 7 and day 30. [Conclusion] The study suggested the efficacy of using multiple scales for prediction of stroke outcome with higher accuracy.

[Purpose] To present the case of the dramatic reduction in pain, disability, and neurologic symptoms following the reduction of forward head translation and increased cervical curvature in a patient suffering from post-surgical radiculopathy. [Subject and Methods] A 52-year-old male mechanic presented with chronic neck pain, unilateral paresthesia along the C5 and C6 dermatome distributions and diminished unilateral grip strength for 12 years following a C5–C6 cervical discectomy and fusion. Outcome measures included the neck disability index, the numerical pain rating scale, and the Zebris cervical range of motion system. Radiographs and computerized posture analysis revealed excessive forward head posture. Initial traditional ‘symptom-relief’ chiropractic rehabilitation was provided, followed by CBP® structural rehabilitation of head and neck posture with a 2.5 year follow-up. [Results] The initial traditional chiropractic rehabilitation did not improve posture or disability scores. CBP methods resulted in radiograph-verified postural alignment improvements corresponding with clinically significant improvements in the patient’s neurologic condition, pain and disability scores. These results were maintained at a 2.5 year follow-up with minimal treatment. [Conclusion] Patients with post-surgical axial symptoms and/or radicular complaints should be screened for altered cervical alignment and anterior head translation. Future studies should attempt to duplicate these positive results in a trial with long-term follow-up.

This study was undertaken to determine the relationship between quadriceps strength and balance to falls of institutionalized elderly people. The subjects comprised 25 elderly women aged 75.3 ± 3.4 years. The values of the postural sway in two standing positions, functional reach test, and quadriceps strength were compared between two groups of elderly adults, those with or without a history of fall. Quadriceps strength was measured using isometric contraction of the knee extension. There were no significant differences between the two groups in the postural sway in the two standing positions and the functional reach test. Quadriceps strength in the non-fall group was, however, significantly higher than that in the fall group (1.48 Nm/kg vs. 1.08 Nm/kg, p<0.01). The discriminating criterion for muscle strength between the two groups was 1.28 Nm/kg, with an apparent error rate of 24.8%. These findings suggest that (1) the degree of quadriceps strength may be a useful indicator for the prospect of fall in institutionalized elderly people, and (2) quadriceps strength should be maintained at least more than 1.28 Nm/kg to prevent fall.
