[Purpose] This study assessed the relationship between hamstring length and gluteus maximus (GM) strength with and without normalization by body weight and height. [Subjects and Methods] In total, 34 healthy male subjects volunteered for this study. To measure GM strength, subjects performed maximal hip joint extension with the knee joints flexed to 90° in the prone position. GM strength was normalized for body weight and height. [Results] GM strength with normalization was positively correlated with hamstring length, whereas GM strength without normalization was negatively correlated with hamstring length. [Conclusion] The normalization of GM strength by body weight and height has the potential to lead to more appropriate conclusions and interpretations about its correlation with hamstring length. Hamstring length may be related to GM strength.

[Purpose] Changes in oxidative stress severity and antioxidant potential are routinely used as oxidative stress markers. While several studies have reported the relationship between these markers and exercise, little is known about the dynamic nature of these markers during muscle atrophy and reloading. Therefore, we examined changes in oxidative stress severity and antioxidant potential during muscle atrophy and reloading. [Subjects and Methods] Muscle atrophy was induced in mice by casting the limb for 2 weeks. Mice were then subjected to reloading for 2 weeks. The severity of oxidative stress (hydroperoxide) and antioxidant potential (degree of reduction) were quantified. [Results] Muscle atrophy was induced by cast immobilization. The muscle mass of mice recovered to similar levels as the control group following 2 weeks of reloading. The degree of oxidative stress was within the normal range throughout the experimental period. The antioxidant potential decreased to the clinical borderline level 2 weeks after immobilization, further decreased after 1 day of reloading, and then recovered to within the normal range. [Conclusion] Performing d-ROMs and BAP tests may contribute to the understanding to atrophic process of skeletal muscle in clinical practice of physical therapy.

[Purpose] To present the case of the resolution of right temporomandibular joint dysfunction (TMJD) following the correction of a right lateral head translation posture. [Subject and Methods] A 24 year old female reported facial pain and jaw clicking in the right TMJ. Radiography revealed a 19 mm right head (shift) translation posture. TMJ vibration analysis showed characteristic abnormalities for the right TMJ. The patient was treated with CBP® technique mirror image® left sided exercises, and traction methods as well as spinal manipulative therapy (SMT). [Results] After 36 treatments over a 12-week time period, a complete correction of the lateral head posture was achieved corresponding with a complete resolution of jaw pain and clicking. TMJ vibration analysis demonstrated normal right side TMJ characteristics following treatment. [Conclusion] Abnormal head/neck postures, such as lateral head translation, may be an unrealized source of TMJD and may be explained through the ‘regional interdependence’ model or by how seemingly unrelated anatomy may be associated with a primary complaint.

[Purpose] This study was to observe the influence of Pilates training on the quality of life in chronic stoke patients. [Subjects and Methods] Forty chronic stroke patients participated in this study. They were divided into same number of experimental group (EG) and control group (CG). EG participated in a 60-min Pilates training program, twice a week for 12 weeks, while the CG did not participate in any exercise-related activities for the duration and participating in general occupational therapy without any exercise-related activities. Then the MMSE-K was performed before and after Pilates training to observe the influence of Pilates training on the quality of life in chronic stroke patients. [Results] Statistically significant improvement in the physical, social, and psychological domains was found in EG after the training. No statistically significant difference was found in all three quality of life domains for the CG. EG experienced a statistically significant improvement in all quality of life domains compared with that of CG. [Conclusion] Therefore, participation in Pilates training was found to effectively improve the quality of life in stroke patients. Pilates training involves low and intermediate intensity resistance and repetition that match the patient’s physical ability and can be a remedial exercise program that can improve physical ability and influence quality of life.

[Purpose] The objective of the present study was to examine the factors, including physical and mental functions, that are associated with the prognosis for activities of daily living in patients with stroke in a recovery rehabilitation unit. [Subjects and Methods] The subjects were 13 patients with stroke who were admitted to the recovery rehabilitation unit of Tokai Memorial Hospital between March and October 2014 and followed up for 2 months. Their physical and mental functions were evaluated at the time of admission and the relationship between these functions and activities of daily living was assessed 2 months later. [Results] The activities of daily living at 2 months after admission showed a significant correlation not only with the severity of motor paralysis (R=−0.80) but mental function (R=−0.69) at the time of admission. No significant correlation was noted between the severity of motor paralysis and the mental function. [Conclusion] It is important to evaluate both mental function and severity of motor paralysis immediately after patients’ admission to the recovery rehabilitation unit. Mental health intervention should be considered in the future to effectively improve subsequent activities of daily living.

[Purpose] This study examined the effects of the intrinsic foot muscle exercise combined with interphalangeal flexion exercise on metatarsalgia with Morton’s toe. [Subject] A 38-year-old male with Morton’s toe, who complained of pain in his left metatarsophalangeal joints was the subject. [Methods] The pressure pain threshold, peak contact pressure of the metatarsophalangeal region during gait, and the navicular drop were measured before and after the intrinsic foot muscles exercises combined with interphalangeal flexion exercise. [Results] After exercising for 2 weeks, the pressure pain threshold increased from 1 to 1.5 kg, while the peak contact pressure decreased from 0.63 to 0.50 kg/cm2, and the navicular drop improved from 5 to 8 mm. [Conclusion] The results show that the combined exercises alleviated the pain while walking by reducing the excessive pressure on the metatarsophalangeal region, and the improvement of gait with Morton’s toe.

[Purpose] The purpose of this study was to investigate the changes in postural sway according to surface stability in post-stroke patients. [Subjects] Fifty-eight subjects were recruited on a voluntary basis from the local rehabilitation unit (stroke patients) and community (age-matched older adults). Twenty-nine stroke patients and twenty-nine age-matched older adults who were healthy participated in the study. Participants were classified as the stroke group (28 stroke patients) and control group (28 age-matched older adults). [Methods] This study applied a cross-sectional design. The Good Balance force platform system (Metitur Ltd., Finland) was used to measure the postural sway velocity, area, and distance of subjects under the unstable surface (foam) condition and stable surface condition in the standing posture. [Results] The postural way velocity, area, and distance were significantly higher in under the unstable surface conditions compared with under the stable surface conditions in both groups. In particular, the changes in postural sway velocity (anteroposterior), area, and distance (anteroposterior) under the foam and stable surface conditions were significantly different between the stroke group and control group. [Conclusion] We believe that these results provide basic information on the improvements in balance ability and may be useful in balance training to prevent future falls after stroke.

[Purpose] The purpose of this study was to measure gluteal pressure and center of force (COF) during sitting in children with cerebral palsy (CP). [Subjects and Methods] The participants were five children with CP and five typically developing controls. We measured peak gluteal pressure and COF using the TekScan system. During the measurements, all children sat on the therapeutic table for 1 minute without back and foot support. [Results] The peak gluteal pressure and COF in children with CP were significantly higher than values in controls. [Conclusion] This study suggests that it is important to distribute hip pressure evenly and transfer COF in a forward direction during rehabilitation in children with CP.

[Purpose] To investigate the effect of heel height on the distribution of plantar foot force and heel pain in patients with a heel spur. [Subjects and Methods] Plantar force was measured using 8 force sensors in 16 patients (3 men, 13 women), with symptomatic heel spur for 4 heel heights (0–4 cm). Sensors were located at the hallux (T1); medial to lateral metatarsals (M1 through M3), mid-foot (MF); and at the central, lateral, and medial heel (CH, LH, and MH). Pain was evaluated using the minimum compression force that caused pain and was measured using an algometer. [Results] Load bearing shifted from the heel (CH) to the mid-foot (MF) and hallux (T1) with increasing heel height. Raising the heel from 2 to 3 cm reduced the magnitude of load bearing, relative to the minimum compression force for pain, by 3.70% at the LH and 2.35% at the MH. Excellent clinical outcomes, defined by a 70–100% decrease in pain, were achieved in 10/16 participants with the use of a 2-cm and 3-cm heel height in men and women, respectively. [Conclusion] Increasing heel height effectively decreases the plantar force on the heel during weight-bearing activities.

[Purpose] This study aimed to investigate the correlation between the diaphragm thickness and diaphragm excursion, and pulmonary function in individuals with stroke. [Subjects and Methods] One hundred fourteen patients who were clinically diagnosed with ischemic or hemorrhagic stroke were included. The diaphragm thickness and excursion were assessed using ultrasonography, and the diaphragm thickening ratio was standardized using a formula. To analyze pulmonary function, we measured the forced vital capacity, forced expiratory volume in one second, and peak expiratory flow. [Results] A statistically significant correlation was found between the diaphragm thickness, thickness ratio, and diaphragm excursion; and the forced vital capacity, forced expiratory volume in one second, and peak expiratory flow. [Conclusion] This study demonstrated that there is a relationship between respiratory function and diaphragm thickness and diaphragm excursion, especially in the paretic side of the diaphragm. Therefore, the role of the respiratory muscles of the paretic side is important in rehabilitation programs to improve the respiratory function of stroke patients.

[Purpose] We investigated the effects of upper extremity resistance exercise with elastic bands on respiratory function in children with cerebral palsy. [Subjects and Methods] Fifteen children were divided into two groups: the experimental (n=8) and control (n=7) groups. Both groups performed general exercises for 30 minutes per session, two times a week during the intervention period. The experimental group performed an additional upper extremity resistance exercise with elastic bands for 20–30 minutes per session, twice weekly for 8 weeks. Pulmonary function, and respiratory muscle and grip strength were measured before and after the exercise. [Results] There was no significant difference in forced vital capacity, forced expiratory volume in one second, the ratio of forced expiratory volume in one second to forced vital capacity, and peak expiratory flow before and after the intervention in either group. The increment of maximal expiratory pressure was significantly greater in the experimental group, compared with the control group. In addition, grip strength was significantly increased in the experimental group after the intervention than before. [Conclusion] We found that upper extremity resistance exercise with elastic bands has a positive effect on expiration and improves grip strength in children with cerebral palsy.

[Purpose] To determine whether repetitive trampoline or hard surface jumping affects lower extremity alignment on jump landing. [Subjects and Methods] Twenty healthy females participated in this study. All subjects performed a drop vertical jump before and after repeated maximum effort trampoline or hard surface jumping. A three-dimensional motion analysis system and two force plates were used to record lower extremity angles, moments, and vertical ground reaction force during drop vertical jumps. [Results] Knee extensor moment after trampoline jumping was greater than that after hard surface jumping. There were no significant differences between trials in vertical ground reaction force and lower extremity joint angles following each form of exercise. Repeated jumping on a trampoline increased peak vertical ground reaction force, hip extensor, knee extensor moments, and hip adduction angle, while decreasing hip flexion angle during drop vertical jumps. In contrast, repeated jumping on a hard surface increased peak vertical ground reaction force, ankle dorsiflexion angle, and hip extensor moment during drop vertical jumps. [Conclusion] Repeated jumping on the trampoline compared to jumping on a hard surface has different effects on lower limb kinetics and kinematics. Knowledge of these effects may be useful in designing exercise programs for different clinical presentations.

[Purpose] This study aimed to evaluate the sustainability and efficacy of exercise intervention, as implemented by professionally trained volunteers, on the health-related quality of life among Japanese community-dwelling older females. [Subjects and Methods] We conducted a non-randomized observational prospective study of a six-month exercise intervention delivered by volunteers or health professionals. Health-related quality of life was scored using the Short Form 36 Health Survey before and after the intervention. Participants were classified into three groups, comprising those with improved health, maintained health, and reduced health. [Results] Among 127 Japanese community-dwelling females aged >65 years, 50 of 62 (80.6%) females involved in the intervention conducted by volunteers, and 55 of 65 (84.6%) females involved in the intervention conducted by health professionals, completed the six-month intervention program. Scoring revealed that interventions by both volunteers and health professionals had a maintaining or improving effect on scores in >70% of participants instead of an expected six-month aging decline. [Conclusion] Exercise intervention by trained volunteers was sustainable and effective for improving the health-related quality of life among Japanese community-dwelling older females. Professionally trained volunteers could benefit communities in helping older persons avoid the need for daily nursing care through promoting health activities.

[Purpose] This study compared the activity of the quadriceps femoris muscle according to the angle of ankle abduction during a lunge exercise. [Subjects and Methods] Fifteen male healthy volunteers participated in the study. All participants performed the lunge exercise with different angles of ankle abduction (0°, 20°, 40°, 60°). The activity of the rectus femoris (RF), vastus lateralis (VL) and vastus medialis oblique (VMO) muscles were recorded for the four angles by using a surface electromyography system. [Results] There were significant differences between the rectus femoris (RF), vastus lateralis (VL) and vastus medialis oblique (VMO) with every angle of the ankle joint. The most significantly increased muscle activity was evident in the vastus medialis oblique (VMO) when the ankle abduction was 60°. [Conclusion] These findings suggest that as the ankle abduction angle increases during the lunge exercise, the muscle activity of the quadriceps femoris muscle increases and this could be helpful in the selective muscle strengthening of the vastus medialis oblique muscle (VMO) with the ankle in the 60° abduction position.

[Purpose] To present a case demonstrating the reduction of progressive thoracolumbar scoliosis by incorporating Chiropractic BioPhysics® (CBP®) technique’s mirror image® exercises, traction and blocking procedures based on the ‘non-commutative properties of finite rotation angles under addition’ engineering law. [Subject and Methods] A 15-year-old female presented with a right thoracolumbar scoliosis having a Cobb angle from T5–L3 of 27° and suffering from headaches and lower back pains. Her curve had progressed over the last two years despite being under traditional chiropractic care. [Results] The patient was treated using CBP structural rehabilitation protocols incorporating mirror image traction, home blocking, corrective exercises and spinal manipulation. The patient was treated 24 times (including 45 home self-treatment blocking sessions) over the course of 15-weeks. Her thoracolumbar curve reduced from 27° to 8° and her headache and low back pain disability improved significantly. [Conclusion] CBP mirror image exercises and traction are consistent with other successful non-surgical approaches and show promise in treating adolescent idiopathic scoliosis.

[Purpose] This case series presents the unique application of the non-commutative property of finite rotation angles under addition to the CBP® mirror image® treatment protocol for adult lumbar and thoraco-lumbar scoliosis. [Subjects and Methods] Five adult patients having lumbar/thoraco-lumbar scoliosis and back pain, and having at least two prominent thoracic postural abnormalities according to Harrison’s rotations and translations of thoracic postures were included. After initial assessment, mirror-image stress x-rays were taken. For each patient, one stress film was taken using the order of mirror image movements with the largest displacement followed by the second largest (primary + secondary) and one stress film was taken in the opposite order (secondary + primary). The consecutive ordered movements that resulted in the largest reduction of curvature were chosen as the order-specific series of movements all exercises and postural traction were to be performed for each patient. Spinal manipulation was also performed. [Results] All patients had a reduction of curvature concomitant with a reduction in pain levels. [Conclusion] This unique treatment approach offers a patient-specific, targeted structural rehabilitative procedure to stress the spine towards a more straightened configuration. Adult lumbar and thoraco-lumbar curves can be reduced and improved by these non-invasive CBP methods.

[Purpose] The purpose of this study was to examine the effect of virtual reality-based eccentric training on lower extremity muscle activity and balance in stroke patients. [Subjects and Methods] Thirty stroke patients participated, with 15 patients allotted to each of two eccentric training groups: one using a slow velocity (group I) and one using a fast velocity (group II). The virtual reality-based eccentric training was performed by the patients for 30 minutes once a day, 5 days a week, for 8 weeks using an Eccentron system. Surface electromyography was used to measure the lower extremity muscle activity, while a BioRescue was used to measure balancing ability. [Results] A significant difference in lower extremity muscle activation and balance ability was observed in group I compared with group II. [Conclusion] This study showed that virtual reality-based eccentric training using a slow velocity is effective for improving lower extremity muscle activity and balance in stroke patients.

[Purpose] In this paper, hamstring stretching and nerve mobilization are conducted on patients with radicular lower back pain, and changes to pain levels, pressure thresholds, angles of knee joint extension, and disorder levels of lower back pain were studied. [Subjects and Methods] The subjects were divided into two groups: one group conducted hamstring stretches and was comprised of 6 male and 5 female subjects, and the other group received nerve mobilization treatment and was comprised of 5 male and 6 female subjects. [Results] Pain level and the disorder index of lower back pain were significantly alleviated after the intervention in both groups. Pressure threshold and angles of knee extension were significantly increased after the intervention in both groups. Comparing the two groups, the alleviation of pain was more significant in the nerve mobilization group. [Conclusion] Patients with radicular lower back pain showed significant differences in pain level, pressure threshold, knee extension angle, and disorder index of lower back pain for both the hamstring stretching group and nerve mobilization group after the treatment. Hamstring stretching and nerve mobilization can be usefully applied for the therapy of patients with radicular lower back pain.

[Purpose] The aim of this study was to examine the effects of the application of elderly-customized hatha yoga on the vascular inflammation factors of elderly women. [Subjects and Methods] This research was conducted with 14 elderly women, between 70 and 80 years old, divided into an elderly-customized hatha yoga group (n=7) and a control group (n=7). The application group participated in a hatha yoga program designed to be elderly-friendly for 10 weeks. At the end of the program, the vascular inflammation factors were measured, including the albumin, white blood cell count, fibrinogen, high sensitivity C-reactive protein (hs-CRP), and erythrocyte sedimentation rate (ESR). [Results] In the hatha yoga group, the albumin increased significantly after the application, when compared to the level before the application, while the fibrinogen, hs-CRP, and ESR decreased significantly. In the control group, the vascular inflammation factor levels before and after the application period were not significantly different. [Conclusion] Based on the results of this study, the application of elderly-customized hatha yoga created positive changes in the vascular inflammation factors of elderly women.

[Purpose] The purpose of this study is to research the most effective knee flexion angle and ground condition in the squat position. [Subjects and Methods] The subjects of this study were 15 female college students who were able to perform squat movements and who had never previously experienced surgery, orthopedic disease, or musculoskeletal impairment. This study was conducted to examine changes of muscle activation of low-extremity muscles at different knee flexion angles of 70°, 90°, and 100°. Balance Pad (Aero Step, TOGU, Germany) was used as unstable ground. Surface electromyogram (4D-MT & EMD-11, Relive, Korea) was used for measuring muscle activation. Measured muscles were vastus medialis, biceps femoris, tibialis anterior, and gastrocnemius. Muscle activation was determined by the root mean square (RMS). [Results] There was a difference in muscle activation of the vastus medialis and tibialis anterior according to the change of the knee flexion angle with the stable ground. However, there was no difference in muscle activation of the lower extremity muscles according to the change of the knee flexion angle with the unstable ground. [Conclusion] These results suggest that changes in the angle of the knee flexion with the stable ground affect the muscle activation of the vastus medialis and tibialis anterior. It was found that as the joint angle increases, muscle activation also increases. However, ground condition does not affect muscle activation.
