Objectives: To elucidate the factors that influence occupational physicians’ decision to issue an employer warning.Methods: The interview was conducted with 10 Japan Society for Occupational Health certified occupational physicians (COPs) and certified senior occupational physicians (CSOPs) to create nine fictive scenarios in which an occupational physician may need to consider issuing a warning. Sixteen CSOPs assessed the seriousness of the problem in each of nine scenarios where they may need to consider issuing an employer warning. Next, using a survey questionnaire, 597 COPs and CSOPs were asked to rate how likely they were to issue a warning in each of the nine scenarios, and answer items on their characteristics and number of previously issued warnings. A multilevel logistic regression analysis nested for various scenarios was used to assess the odds ratio (OR) of being likely to issue a warning.Results: Valid questionnaires were obtained from 117 participants (19.6%). The ORs and 95% confidence intervals (CIs) were as follows: mean score of seriousness of the problem, 5.90 (4.50-7.75); years of experience as occupational physician, 1.04 (1.02-1.06); women, 1.75 (1.20-2.54); being a part-time occupational physician without in-house experience, 2.08 (1.31-3.29); and having previously issued two or more times warnings, 1.99 (1.29-3.06), compared with those who had never issued a warning.Conclusions: Occupational physicians’ likelihood to issue a warning was associated with the seriousness of the problem as assessed in various scenarios, years of experience as occupational physician, gender, employment type, experience as in-house occupational physician, and number of past warnings.

Objectives: Stress management for human service professionals remains an international issue. Methods: We conducted a primary prevention program using Dohsa-hou designed to provide early-career nurses with an understanding of stress and physical relaxation skills that can be utilized to reduce negative and increase positive mood states. Results: A total of 56 nurses working at a general hospital in Japan participated in the program. As a result, negative mood states were significantly reduced at post-test from the intervention. Also, vigor as a positive mood state showed a marginal increase compared to pre-test. These findings were largely consistent with a previous study implementing Dohsa-hou movement tasks in a stress management program. Conclusions: Although there were some limitations, this study suggested that the stress management program using Dohsa-hou may be a useful approach for preventing burnout by promoting mood state regulation for nurses.

Objectives: This retrospective cohort study evaluated the impact of the Stress Check Program, a recently introduced national policy and program aimed at reducing psychological distress among Japanese workers. Methods: A baseline survey was conducted from November 2015 to February 2016, the period when Japan began enforcing the Stress Check Program. A one-year follow-up survey was conducted in December 2016. In the follow-up survey, two exposure variables were collected: having taken the annual stress survey, and experiencing an improvement in the psychosocial work environment. Psychological distress was assessed using the Brief Job Stress Questionnaire (BJSQ) at baseline and 1-year follow-up. The two exposure variables were used to define four groups: "Neither", "Stress survey (SS) only", "Psychosocial work environment improvement (WI) only", and "Both". BJSQ results were analyzed using repeated measures general linear modeling (GLM). Results: The study included 2,492 participants: 1,342 in the "Neither" group, 1,009 in the "SS only" group, 76 in the "WI only" group, and 65 in the "Both" group. Overall time-group interaction effects were not significant. The "Both" group showed significantly greater improvements in psychological distress than the "Neither" group (p = 0.02) at the 1-year follow-up, although the effect size was small (d = -0.14). Conclusions: Combination of the annual stress survey and improvement in psychosocial work environment may have been effective in reducing psychological distress in workers, although the effect size was small.

Objectives: This study aimed to examine the potential effects of perceived organizational support (POS) and psychological capital (PsyCap) on combating depressive and anxious symptoms among Chinese underground coal miners. Methods: A cross-sectional survey was conducted in a coal-mining population in northeast China. The Center for Epidemiologic Studies Depression (CES-D) Scale, the Zung Self-Rating Anxiety Scale (SAS), the Survey of Perceived Organizational Support (SPOS) scale and the Psychological Capital Questionnaire (PCQ), which measure depressive and anxious symptoms, POS and PsyCap were distributed to 2,500 underground coal miners (1,925 effective respondents). Hierarchical linear regression was performed to examine the associations of POS and PsyCap (self-efficacy, hope, resilience and optimism) with depressive and anxious symptoms and the moderating roles of PsyCap and its components. The mediating roles of PsyCap and its components were examined using asymptotic and resampling strategies. Results: The mean levels of depressive and anxious symptoms were 19.91 and 49.69, respectively. POS, PsyCap, hope, resilience, optimism and POS × PsyCap were negatively associated with depressive symptoms. POS, PsyCap, resilience, POS × PsyCap and POS × resilience were negatively associated with anxious symptoms. PsyCap, hope, resilience and optimism partially mediated the association between POS and depressive symptoms. PsyCap and resilience partially mediated the association between POS and anxious symptoms. Conclusions: POS, PsyCap, hope, resilience and optimism could be effective resources for reducing depressive and anxious symptoms. PsyCap, hope, resilience and optimism act as moderators and mediators in the associations of POS with depressive and anxious symptoms. Managers should promote supportive settings and investment in PsyCap to improve workers' mental health.(J Occup Health 2015; 57: 58–68)

Objective: We reviewed studies on pulmonary, reproductive, and developmental toxicity caused by carbon nanotubes (CNTs). In paricular, we analyzed how CNT exposure affects the several processes of pulmonary toxicity, including inflammation, injury, fibrosis, and pulmonary tumors. Methods: In pulmonary toxicity, there are various processes, including inflammation, injury, fibrosis, respiratory tumor in the lungs, and biopersistence of CNTs and genotoxicity as tumor-related factors, to develop the respiratory tumor. We evaluated the evidence for the carcinogenicity of CNTs in each process. In the fields of reproductive and developmental toxicity, studies of CNTs have been conducted mainly with mice. We summarized the findings of reproductive and developmental toxicity studies of CNTs. Results: In animal studies, exposure to CNTs induced sustained inflammation, fibrosis, lung cancer following long-term inhalation, and gene damage in the lung. CNTs also showed high biopersistence in animal studies. Fetal malformations after intravenous and intraperitoneal injections and intratracheal instillation, fetal loss after intravenous injection, behavioral changes in offsprings after intraperitoneal injection, and a delay in the delivery of the first litter after intratracheal instillation were reported in mice-administered multi-walled carbon nanotubes (MWCNTs). Single-walled carbon nanotubes (SWCNTs) appeared to be embryolethal and teratogenic in mice when given by intravenous injection; moreover, the tubes induced death and growth retardation in chicken embryos. Conclusion: CNTs are considered to have carcinogenicity and can cause lung tumors. However, the carcinogenicity of CNTs may attenuate if the fiber length is shorter. The available data provide initial information on the potential reproductive and developmental toxicity of CNTs.

Objective: To assess the usefulness of the Laboratory of Physical Science (LOPS) protocol for detecting exaggerated blood pressure (BP) response as a risk factor for future hypertension when controlling for work and personal factors. Methods: Subjects were 815 healthy normotensive men (mean age, 43.1 ± 6.76 yr; range, 29-64 yr) who participated in the LOPS protocol, a graded 4-stage exercise test undertaken for the measurement of 40-70% of maximum oxygen consumption. A hypertensive response was defined as systolic blood pressure (SBP) of 250 mmHg or diastolic blood pressure (DBP) of 120 mmHg during the exercise test. Results: New-onset hypertension or the initiation of antihypertensive drug treatment had occurred in 108 men (13.3%) after 7 yr of follow-up. Cox proportional survival analysis revealed significantly increased risks of developing hypertension were associated with exaggerated BP response to exercise (hazard ratio, 2.3; 95% confidence interval, 1.4-3.7) and higher frequency of business trips (hazard ratio, 1.7; 95% confidence interval, 1.2-2.5) after multivariable adjustments for work and personal-related risk factors. Conclusions: These results suggest that the LOPS protocol is effective for detecting exaggerated BP response as a risk factor for future hypertension when controlling for work and personal-related risk factors. Exaggerated BP response to exercise and higher frequency of business trips are risk factors for developing future hypertension.

<b>目的:</b>警察官は他の公務員と比べて心疾患による休業率が高く,在職死亡においても常に心疾患は死因の上位を占めることから重要な課題となっている.本研究では,A県の男性警察官を対象に,近年増加傾向にある虚血性心疾患に焦点をあて,各種危険因子の保有状況とその背景要因としての勤務状況や生活様式の特徴を明らかにすることを目的として検討を行った.<b>対象と方法:</b> 症例対照研究により,虚血性心疾患の発症に関連する危険因子について検討した.対象はA県警察の男性警察官で,1996–2011年に新規に虚血性心疾患を発症した58名を症例群,脳・心血管疾患の既往がない者の中から年齢と階級をマッチさせて抽出した116名を対照群とした.虚血性心疾患の発症5年前の健診データを用いて,肥満,高血圧,脂質異常症,耐糖能障害,高尿酸血症,喫煙の有無を両群で比較するとともに,多重ロジスティック回帰分析を用いて調整オッズ比を算出した.続いて,男性警察官1,539名と一般職員153名を対象に,横断的な資料に基づいて,各種危険因子の保有率およびメタボリックシンドローム (MetS) の有所見率,勤務状況および生活様式の特徴を年齢階層別に比較検討した.<b>結果: </b>虚血性心疾患を発症した症例群では,対照群に比べて発症5年前での高血圧,耐糖能障害,高LDL-C血症,高尿酸血症の保有率が有意に高かった.多重ロジスティック回帰分析では高血圧(オッズ比 [95%信頼区間]:3.96 [1.82–8.59]),耐糖能異常 (3.28 [1.34–8.03]),低HDL-C血症 (2.26 [1.03–4.97]),高LDL-C血症 (2.18 [1.03–4.61]) が有意な独立の危険因子となった(モデルχ<sup>2</sup>:<i>p<</i>0.001,判別的中率:77.0%).また,警察官は一般職員に比べて腹部肥満者 (腹囲85 cm以上)の割合が有意に高く(57.3% vs. 35.3%, <i>p<</i>0.001),年齢階層の上昇に伴う脂質異常症や耐糖能障害の有所見率の増加もより顕著であった.45–59歳の年齢階層では個人における危険因子の集積数(1.8個 vs. 1.4個, <i>p<</i>0.01)が有意に高く,MetS該当者の割合も高率であった(25.0% vs. 15.5%, <i>p<</i>0.1).さらに,MetS該当者では,交替制勤務者が多く (33.6% vs. 25.4%, <i>p<</i>0.01),熟睡感の不足を訴える者 (42.5% vs. 33.7%, <i>p</i><0.01),多量飲酒者 (12.8% vs. 6.3%, <i>p</i><0.01)の割合が高くなっていた.<b>結論:</b>警察官においても高血圧,耐糖能障害,脂質異常症などの既知の危険因子が虚血性心疾患の発症と関連することが示された.また,警察官は加齢による各種危険因子の保有率およびMetS該当者の割合の増加が一般職員よりも顕著であり,その背景要因として,交替制勤務や長時間労働などの勤務形態と,飲酒や睡眠の状態などの生活様式の影響が示唆された.

Objectives: The objectives of this study were to determine the prevalence of and differences in certain health complaints, including physical symptoms and psychological distress, among workers in different occupational categories and to explore the associations between occupational category and each complaint. Methods: A cross-sectional study was conducted using a self-administered questionnaire. A total of 1,628 workers representing seven occupational categories from ten companies in different industries submitted completed questionnaires. The self-administered questionnaire contained three parts: personal data, physical symptoms and psychological distress. Physical symptoms were measured using nine questions developed by a panel of ten general practitioners; the questions included nine common physical symptoms across main organ systems. Psychological distress was measured using the Chinese Health Questionnaire. Results: Muscle pain (44.7%) and dizziness (30.1%) were the most common symptoms reported by participants, and 16.6% of participants suffered from psychological distress. Significant differences in physical symptoms and psychological distress were found among workers in different occupational categories (χ2=53.59, pConclusions: The study merits attention in terms of prevention of health problems in the workplace by focusing on a set of physical symptoms and psychological distress (not merely morbidity). Occupational category should be taken into consideration when planning workplace health promotion. Our findings highlight the need for health promotion programs that specifically target office workers and managers.

We investigated the relationship between burnout and communication skill training among Japanese hospital nurses to improve the mental health of human service workers. The subjects were forty-five registered nurses referred to a self-expression skill intervention program by their section superiors, with each superior choosing from two to five nurses. The hospital was located in the Kyushu area and staffed by about four hundred nurses. The subjects were divided into an intervention group (19 nurses) and a reference group (26 nurses). The intervention group received the communication skill training in July and August, 2001. The communication skill training was carried out in accordance with the assertiveness training (AsT) precepts of Anne Dickson. In June, 2001, we delivered a set of questionnaires including age, gender, working years, a burnout scale, and a communication skill check-list as a baseline survey. The baseline questionnaires were returned at the end of June, 2001. In January, 2002, we delivered the same questionnaire again to the two groups and collected them at the end of the month. Excluding the only male and insufficient answers, twenty-six nurses (58%) returned complete answers in the initial and subsequent surveys. We found that the personal accomplishment and the two communication skills such as “accepting valid criticisms” and “negotiation” of the intervention group had improved significantly five months after the training as compared with that of the reference. Our results implied that communication skill training might have a favorable effect on burnout among Japanese hospital nurses.

Objectives: The objective of this study was to estimate a risk of cardiovascular and cerebrovascular diseases for each worker and to determine whether this risk is associated with the incidence and costs of presenteeism, absenteeism, and medical/drug treatments.Methods: Established risk equations were used to estimate the 10-year probability of developing coronary artery disease and ischemic stroke in male workers aged 40-65 years who were recruited from four pharmaceutical companies in Japan. The incidence of presenteeism was defined as existence of presenteeism for the past a month, and the incidence of absenteeism was defined as existence of sick-leave for the past three months by a self-administered questionnaire. Each cost was calculated based on the human capital method. Data on medical/drug treatments were collected from health insurance claims.Results: The risks were calculated for 6047 workers. Individuals at moderate and high risk of coronary artery disease had a significantly higher rate of presenteeism and absenteeism than workers at low risk. Workers at moderate and high risk of ischemic stroke also had a significantly higher rate of presenteeism and absenteeism than workers at low risk. Mean costs for absenteeism and medical/drug treatments increased with the risk of developing coronary artery disease or ischemic stroke, while costs for presenteeism did not.Conclusions: To prevent the costs of presenteeism, workers not only at high risk but also at low and moderate risk of developing cardiovascular and cerebrovascular diseases should receive health care services.

Lead oxide is used extensively in the construction and repair of wooden boats in Thailand, but the behaviors of boatyard workers that could place them at risk of contamination have not previously been documented. Baseline data on practices and behaviors of boatyard workers and on the level of worker and workplace contamination with lead were therefore collected. Fifty workers in two boatyards participated in this study. Lead exposure of workers was assessed by determining airborne and blood lead levels. A questionnaire was administered to collect information on work history, suspected exogenous lead sources, personal behavior and knowledge about lead. Evidence obtained by the study indicated that safety behavior and personal hygiene were poor-workers used no mask, gloves or hood, wore open sandals, smoked, drank, chewed and ate during work and did not wash their hands before drinking or eating. Some workers had lunch in the working area. The mean personal airborne lead of caulkers (36.4 μg/m3) was higher than that of carpenters (8.3 μg/m3). Forty-eight percent of all workers and 67% of caulkers had a blood lead level (BLL) exceeding 40 μg/dl. Multiple linear regression indicated that blood lead levels of workers were significantly related to job and education level, with significant differences between boatyards. In addition, the potential for "take-home" contamination was high; none of the workers took a shower or changed their clothes prior to going home. These results indicate a problem of lead exposure of sufficient magnitude to be a public health concern.

Objectives: To narratively review the presence and treatments of mental health problems among high-level political leaders. These questions have been noted in few epidemiologically sound studies and in the media. Methods: The literature search was performed and it resulted well-described cases and case series, but lacks properly designed studies focusing on medical issues. Results: High-level political leadership is a high-risk occupation, especially during crises and wartime, but also stressful in modern, democratic society. Leadership positions do not necessary facilitate the early detection of and intervention in mental disorders. In the media, psychiatrists should ensure that leaders with mental disorders are treated in a manner that preserves their dignity. Commonly accepted ethical principles stress that psychiatrists should not make announcements to the media about presumed psychopathology and diagnosis of any individuals. Conclusions: Current top leaders are mostly in midlife and rarely seriously mentally ill, but many are prone to anxiety, depression, addictions or stress-related disorders. The care of these eminent persons presents a clinical challenge that requires experience, clinical skills and multidisciplinary team work, usually within the occupational health system which is familiar with the working conditions and state of health of each patient.

Objectives: We prospectively examined the combined effect of high stress (i.e., being under great work-related stress), as defined in the Japanese Stress Check Program manual using the Brief Job Stress Questionnaire (BJSQ), and job dissatisfaction on long-term sickness absence lasting 1 month or more. Methods: Participants were 7,343 male and 7,344 female financial service company employees who completed the BJSQ. We obtained personnel records covering a 1-year period to identify employees with long-term sickness absence, which was treated as a dichotomous variable. Participants were classified into four groups (high-stress+dissatisfied, high-stress+satisfied, not high-stress+dissatisfied, and not high-stress+satisfied groups) to calculate the hazard ratios (HRs) of long-term sickness absence for these groups using Cox’s proportional hazard regression analysis. Furthermore, to examine whether the combined effect of high stress and job dissatisfaction is synergistic or additive, we calculated relative excess risk due to interaction (RERI), attributable proportion due to interaction (AP), synergy index (SI), and their 95% confidence intervals (CIs). Results: After adjustment for covariates, the HR of long-term sickness absence was highest among the high-stress+dissatisfied group (HR 6.49; 95% CI, 3.42–12.3) followed by the high-stress+satisfied group (HR 5.01; 95% CI, 1.91–13.1). The combined effect of high stress and job dissatisfaction was additive (95% CIs of RERI and AP included 0 and that of SI included 1). Conclusions: Our findings suggest incorporating high stress with job dissatisfaction improves the predictability of long-term sickness absence. However, employees reporting high stress but satisfaction with their jobs may still at increased risk of developing long-term sickness absence.

Objectives: Decreased workforce productivity has a significant economic impact on healthcare systems. Presenteeism, the practice of working at reduced potential, is more harmful than absenteeism. Present workers most often experience musculoskeletal pain that is not mitigated by general exercise or stretching. We aimed to assess whether a regimen of pain neuroscience education (PNE) and exercise tailored to individual healthcare workers could reduce presenteeism and improve productivity.Methods: An independent investigator randomized 104 medical professionals into two groups (intervention and control). The control group received general feedback after answering a questionnaire, while the intervention group received a 6-month plan of exercises and PNE created by a physical therapist with 10 years of experience. Our primary outcome was the scores of the Japanese version of the World Health Organization Health and Work Performance Questionnaire (WHO-HPQ) to investigate presenteeism; and our secondary outcomes were pain intensity, widespread pain index (WPI), and EuroQol 5-dimension (EQ5D-5L).Results: In the intervention group, post intervention, we observed significant improvement in presenteeism, pain intensity, WPI, physical and psychological stress, and EQ5D-5L (P < .05). In the control group, we noted significant improvement only in the physical and psychological stress post intervention (P < .05). The results showed significant between-group differences in presenteeism post-intervention (P < .05).Conclusion: We demonstrated that a combination of PNE and exercise decreases presenteeism of healthcare workers. Our findings will help healthcare facilities carry out better employee management and ensure optimal productivity.

Objectives: Problematic internet use (PIU) has been suggested in relation to psychological symptoms among schoolteachers, but the relationship with physical symptoms remains unclear. We examined whether PIU or longer Internet usage time is associated with neck pain in schoolteachers.Methods: We conducted a cross-sectional study among 2582 teachers aged 20 years or older (35.6% women) in Shimane and Tottori, Japan in 2018. Neck pain was defined as ≥5 points on the Neck Disability Index. The Compulsive Internet Use Scale (CIUS) was used to assess PIU. Internet usage time on weekdays and weekends was divided into five groups: 0, 1-29, 30-59, 60-119, and ≥120 min/day. Logistic regression analysis was conducted to examine the association of the CIUS score and Internet usage time on weekdays or weekends with neck pain, adjusting for sex, age, position at school, insomnia, and psychological distress.Results: We observed 800 (31.0%) teachers with neck pain. The median (interquartile range) of their CIUS scores was 7 (2, 14). A higher CIUS score was independently associated with a higher prevalence of neck pain (odds ratio of 4th vs. 1st quartiles, 1.41; 95% confidence interval, 1.06-1.87; trend P = .006). Compared with non-Internet users, Internet users on weekdays had almost double the odds of neck pain although the difference did not reach the customary level for designating statistical significance.Conclusions: In conclusion, teachers with higher scores in CIUS were associated with a higher prevalence of neck pain in Japan, suggesting adults with PIU being at risk of physical disorders.

Objectives: To determine the prevalence of burnout according to job category after the first wave of COVID-19 in Japan and to explore its association with certain factors.Methods: An online cross-sectional survey of health care workers (HCWs) from June 15 to July 6, 2020, was conducted at a tertiary hospital in Tokyo, Japan. Demographic characteristics, results of the Japanese version of the Maslach Burnout Inventory-General Survey, types of anxiety and stress, changes in life and work after the peak of the pandemic, and types of support aimed at reducing the physical or mental burden, were determined.Results: Of 672 HCWs, 149 (22.6%) met the overall burnout criteria. Burnout was more prevalent in women (OR, 3.11; 95% CI, 1.45-6.67, P = .003), anxiety due to unfamiliarity with personal protective equipment (PPE) (OR, 1.98; 95% CI, 1.20-3.27, P = .007), and decreased sleep duration (OR, 1.96; 95% CI, 1.20-3.20, P = .008). Conversely, participants who felt that the delivery of COVID-19-related information (OR, .608; 95% CI, .371-.996, P = .048) and PPE education opportunities (OR, .484; 95% CI, .236-.993, P = .048) and messages of encouragement at the workplace (OR, .584; 95% CI, .352-.969; p = .037) was helpful experienced less burnout.Conclusions: There is a need to focus on the above factors to maintain the mental health of HCWs. The delivery of COVID-19-related information and educational interventions for PPE and messages of encouragement at the workplace may be needed to reduce the mental burden.

Objectives: This study aimed to examine the factors for reducing monetary loss due to presenteeism by using a tailored healthcare web-application among office workers with chronic neck pain. Methods: The study was single-arm pre-post comparison study using secondary data of 130 Japanese office workers with chronic neck pain who used a tailored healthcare web-application (web-app) over 12 weeks. This data was obtained from BackTech Inc. (Kyoto, Japan), which manages the healthcare web-app. The primary outcome measure was the monetary loss due to presenteeism based on the quality and quantity method. Secondary outcome measures were intensity of physical symptoms measured by the Visual Analog Scale, frequency of web-app use obtained from the database, and the risk of depression score assessed by the Depression and Suicide Screen. Results: Eighty-six participants were included in the complete-case analysis. Monetary loss due to presenteeism and the risk of depression reduced, while physical symptoms improved significantly (p<0.01) after using the web-app. After covariate adjustment, decrease in neck pain intensity (β=0.25, confidence interval=2.34 to 32.66) and high frequency of web-app use (β=−0.24, confidence interval=−10.29 to −0.63) were significantly associated with a reduction in monetary loss due to presenteeism. Conclusion: Neck pain intensity and frequency of web-app use may be important factors for reducing monetary loss due to presenteeism among office workers with chronic neck pain who used a tailored health care web-app.

Objectives: The present study examined the relationship between health culture evaluated by the Health and Productivity Survey Sheets and the implementation status of infection control measures against COVID-19 in the workplace.Methods: This was a cross-sectional study using the corporate data (2518 companies) collected for the purpose of selecting the excellent company of health and productivity management by the Japanese government. The explanatory variable was the overall evaluation score, and the outcome was whether or not infection control measures against COVID-19 in the workplace. We used logistic regression analysis and calculated the odds ratio adjusted for the industry sector, the corporation size, and the operating profit ratio by the overall evaluation score category.Results: The odds ratio of all infection control measurements in the workplace increased as the evaluation score increased.Conclusions: This study revealed a new finding that the presence of a healthy culture in the workplace will lead to the appropriate implementation of infection control measures during a pandemic. The company's ordinary commitment to employee health will be effective even in times of health crisis, such as during the outbreak of the pandemic.

With the rapidly increasing number of work-related suicides in Japan (Karojisatsu, in Japanese), both applications for worker's compensation insurance and civil suits are proliferating. The phenomenon of work-related suicide is examined along with the process and related factors. With informed consent from bereaved families, two certified psychiatrists independently reviewed and summarized 22 insurance and legal reports filed by psychiatrists on employee suicides that were related to heavy workloads. A clinical epidemiologist participated in discussions with psychiatrists to reach a consensus concerning the cause of the suicides. Only one case involved a female. Seventeen had experienced personnel changes, such as a promotion or transfer. Low social support was recognized in 18, high psychological demand in 18, low decision latitude in 17, and long working hours in 19 cases (more than 11 hours per day for 3 months or more, and without a day off in 9). The subjects had depressive episodes by the ICD-10 criteria and showed suicidal signs. Ten of them saw a general practitioner because of unspecified somatic complaints, but no effective measures were taken. None of them had a history of psychiatric consultation or had received mental health education dealing with job stress management. Although causality cannot be made from this case series report, we hypothesize that long working hours, heavy workloads, and low social support may cause depression, which can lead to suicide. Appropriate countermeasures are urgently needed and the present findings suggest some of them are possible.
