加齢に伴う筋力,身体機能の低下と定義されるサルコペニアは各種疾患でその概念が普及し,治療アウトカムにおける影響が盛んに報告されている.われわれも高齢胃癌患者におけるサルコペニア症例が非サルコペニア症例にくらべて術前摂取エネルギー量,タンパク質量が有意に少なく,サルコペニアが胃癌術後の重篤な合併症発生の独立したリスク因子であることを報告している.つぎのステップとして,初診時にサルコペニアと診断された65 歳以上の高齢胃癌術前患者を対象にして,エクササイズとしてハンドグリップ,ウォーキング,レジスタンストレーニング,栄養介入として1 日28kcal/kg(IBW)のエネルギー量と1.2g/kg(IBW)のタンパク質の摂取およびβ-hydroxy-β-methylbutyrate(HMB)の補充を推奨する,「術前栄養+エクササイズプログラム」を作成・実践しているので,その概要について報告する.

This paper presents actual performance of high-reflectivity panels by enviromental exposure test and cover performance of cloth. It was indicated that baking finishing and photocatalyst coating panels could maintain initial performance over a long period compared with conventional panels. On the other hand, spectral reflectivity and transmissivity of the clothes commercially available that influence on human thermal sensation of the exterior space. The spectral characteristic of solar shielding near infrared region is significant.

胃癌患者は高齢者や低栄養症例も多いため,術前からサルコペニアの有病率が高いことが予想される.2012年7月~2014年2月の期間で当科にて行った65歳以上の胃癌手術症例69例中38例(55.1%)にサルコペニアを認めた.サルコペニア群で有意に摂取エネルギー量が少なく(25.6 vs. 29.9kcal/(IBW)kg, p=0.0060),摂取タンパク質量が少なかった(0.95 vs. 1.12g/(IBW)kg, p=0.0041).Clavien-Dindo分類Grade Ⅲ a 以上の重篤な合併症発生率はサルコペニア群で有意に高かった(23.7 vs. 6.5%, p=0.043).サルコペニア有りは重篤な合併症発生に関する独立した危険因子であった(OR 5.86, 95%CI 1.06? 51.65, p=0.042).サルコペニア合併高齢胃癌手術症例に対し,適切な栄養介入が重篤な合併症を回避するうえで有用であると考える.
