Massive bleeding induced by disintegration of breast cancer tumors for 4 yrs (88 yrs-old lady) was treated by applying Mohs' paste (75g/45mL of Zinc complex). This huge site healed quickly and the healing effects were sustained for 48 days. Based on this clinical evidence, the hemostatic mechanism of Mohs' paste was evaluated using the hind limb ischemia model in mice. Unilateral hind limb ischemia was induced in 6 week-old male C57BL/6JJmsSlc mice by resecting the right femoral and saphenous arteries. Mice were painted once a day with Mohs' paste containing 0, 37.5, 50 and 62.5g/45mL of zinc after the surgery. Hind limb blood perfusion was measured by using a laser Doppler perfusion imaging system (moorLDI2-Ⅱλ : S/N 5489). To identify capillaries, the thigh muscles were harvested on Day 1 to make a tissue section and immunostained for CD31. One day after painting of Mohs' paste, blood flow suppression to the ischemic leg was significantly enhanced in a dose dependent manner. Furthermore, anti-CD31 immunostaining revealed that Mohs' paste decreased capillary density in the ischemic muscle. The results of both the clinical and animal studies suggest that Mohs' paste suppressed angiogenesis due to the acute effects of aggregated protein and the sustained effects of decreases of CD31.

We developed an international journal club, using an online meeting tool, as an evidence-based medicine training program intended for healthcare professionals and students. This study aimed to evaluate the outcome of the journal club via a questionnaire survey. We administered the questionnaire to 53 healthcare professionals and students who participated in the journal club between February 2019 and April 2020. Respondents showed positive improvement scores after participating in the journal club, compared with those before, as conveyed through the use of a seven-point Likert scale. The median scores of critical thinking skills, discussion skills in English, evidence-based practice skills, and the sense of freedom to ask questions and make comments after participating in the journal club were significantly improved from 4 (interquartile range [IQR]: 2 - 5) to 5 (IQR: 4 - 6); 3 (IQR: 2 - 4) to 3 (IQR: 2 - 5); 3 (IQR: 2 - 5) to 4 (IQR: 3 - 6); and 3 (IQR: 1 - 5) to 3 (IQR: 2 - 6), respectively. Moreover, they expressed interest in the opportunity to share articles on topics of interest with other professionals from different disciplines, to exchange clinical practice ideas concerning topics of interest, and to explore differences in clinical settings between Asia and the MD Anderson Cancer Center. They also expressed great appreciation for the online meeting format, novelty, and overall qualities of the journal club. These findings suggest that an international journal club for healthcare professionals and students could be useful in an evidence-based medicine training program and communication in English.

The purpose of this study was to analyze factors that influence the outcome of nonsurgical treatment for partial-thickness rotator cuff tears (PTRCTs). Sixty-one patients with PTRCT included 34 women and 27 men with a mean age of 55.7 years old were evaluated.43 patients had a history of trauma such as a fall or traffic accident. The types of PTRCT were revealed by MRI. A bursal-sided tear of the cuff was observed in 32patients, a joint-sided tear in 20 and an intratendinous tear in 9. All the patients were treated nonsurgically for a mean duration of 5.7 months. Satisfactory results were achieved in 28 patients.33 patients with unsatisfactory results eventually required surgery. Factors of the 33 nonresponsive patients to the 28 responsive patients to treatment were compared. The factors were as follows; characteristics of the patients, range and strength of the shoulder motion, and radiographic factors, such as size of the subacromial spur and acromial morphology. All data were statistically evaluated using a logistic analysis. The significance level was set at P < 0.05, and odds ratios were figured. Identified factors that had a significant difference between the two groups were age, size of the subacromial spur, bursal side tear, and range of forward elevation (FE) and external rotation (ER). Positive correlation was demonstrated in age and range of the motions, and negative correlation in size of the spur. Each ratio of age, size of the spur, bursal. side tear, FE and ER was 3.33, 2.55, 33.33, 1.1 and 1.11respectively. This study suggested that the younger the age, a bursal-sided tear, a large subacromial spur and motion restriction of the shoulder are factors that induce a poor outcome of nonsurgical treatment for PTRCTs.
